A comparative study of the effects of plasma substitutes on apparent blood viscosity was made at low and high shear rates. Red cells from normal donors were resuspended in their own plasma diluted with plasma substitutes or saline. The haematocrit wa
OBJECTIVE: To compare the effects of different colloid plasma substitutes on blood coagulation and postoperative blood loss. DATA SOURCES: Relevant studies were obtained from the medical literature. STUDY SELECTION: Articles were selected that provided data on the effects of colloids on hemostasis and postoperative blood loss in humans. Studies comparing different colloids were looked for using MEDLINE and by searching through the references of studies as they were collected. DATA SYNTHESIS: Articles were reviewed and relevant data were extracted and partly presented in comparative tables. CONCLUSIONS: Dextran, gelatin, and hydroxyethyl starch (HES) all can induce a specific decrease of von Willebrand factor and factor VIII:c. Blood coagulation is most impaired by dextran and high molecular weight HES, both associated with increased postoperative blood loss. The effects of HES on blood coagulation have been shown to depend on its molecular weight and rate of elimination. Detrimental effects have ...
High-molecular weight dextran is a plasma volume expander made from natural sources of sugar (glucose). It works by restoring blood plasma lost through severe bleeding. Severe blood loss can decrease oxygen levels and can lead to organ failure, brain damage, coma, and possibly death. Plasma is needed to circulate red...
BioTime, Inc. was incorporated in 1990 in the state of California. It is a biotechnology company engaged in two areas of biomedical research and product development. The Company has historically developed blood plasma volume expanders and related technology for use in surgery, emergency trauma treatment and other applications. Its focus is in the field of regenerative medicine; specifically human embryonic stem ("hES") cell and induced pluripotent stem ("iPS") cell technology. Regenerative medicine refers to therapies based on stem cell technology that are designed to rebuild cell and tissue function lost due to degenerative disease or injury. hES and iPS cells provide a means of manufacturing every cell type in the human body and therefore show considerable promise for the development of a number of new therapeutic products. The Company plans to develop stem cell products for research and therapeutic use through its subsidiaries. OncoCyte Corporation is developing products and technologies to ...
Under normal conditions, blood flow in the renal arterial circulation is antegrade and maintained during diastole. When renal arterial vascular resistance increases or compliance lessens, a decrease in renal diastolic blood flow occurs, which is more pronounced than the decrease in the systolic component (11). This action results in an increased RI. Importantly, the Doppler waveform is altered not by vascular resistance alone but by the interaction of vascular resistance and compliance (e.g., large arterial distensibility, pulse pressure). Therefore, aging, atherosclerosis, and stiffening of the large arteries will result in an increased RI (12). This scenario likely explains the significantly lower baseline RI value in the cohort of healthy young subjects compared with HF patients.. Baseline levels of VII were not different between groups. After multivariate analyses, there was a poor but significant correlation between RVSP and baseline VII, but no significant correlation remained between ...
Octalbin: Albumin belongs to the group of medications known as plasma substitutes. Albumin is made by the liver and is a naturally-occurring protein found in plasma (the fluid that carries blood cells). It helps to transport a number of the bodys substances (e.g., bilirubin) via the blood by attaching itself to them.

Patients undergoing cardiac surgery with cardiopulmonary bypass require significant fluid administration. Fluids are used routinely to replace blood lost during and after surgery. Significant amounts of fluid are also used to prime the tubing and components of the cardiopulmonary bypass pump before and during its use. The use of Pentaspan - a synthetic pentastarch - was started because of the restriction of use for blood and blood products, particularly albumin. Pentaspan is usually used after surgery in the Intensive Care Unit (ICU). The impact of the use of pentastarch on coagulation, fluid balance and bleeding are very limited. This study will methodically evaluate the impact of using increasingly greater amounts of pentastarch during surgery on an open heart surgery patients recovery in particular - is there more bleeding, does his/her blood clot as well, and how much fluid overall is used and excreted ...
BACKGROUND AND OBJECTIVE: The effects of administering albumin 5%, hydroxyethylstarch 6% and succinylated gelatine 4% on oxygen transport and left ventricular function were prospectively investigated in different experimental conditions: baseline, fluid load, after 10 min of myocardial ischaemia and after reperfusion. METHODS: Twenty-seven rabbits received at random one of three colloids in escalating boluses over 10-15 min to achieve left ventricular end-diastolic pressures (LVEDP) between 8 and 10mmHg. A branch of the left anterior descending coronary artery was then temporarily occluded by a ligature and released after 10 min. Myocardial function was assessed using left ventricular pressure recordings and dimension data obtained from ultrasound crystals inserted onto the ventricular wall. Blood was sampled for the determination of oxygen delivery and consumption, the oxygen extraction ratio, acid-base status, and glucose and lactate concentrations. RESULTS: Administration of the colloids ...
Plasma Exchange (PLEX) if no improvement occurs with corticosteroids. The goal of PLEX is to lower the level of NMO-IgG in the blood.PLEX involves removing blood from the body through a needle and tubing. Through a series of steps, the plasma (the liquid part of the blood) is separated from blood cells and replaced with an artificial plasma substitute; the plasma substitute and blood cells are combined and returned to the body through an intravenous line. The procedure lasts several hours and may be repeated multiple times over a number of days ...

Blood transfusions are crucial for the successful treatment of many diseases and injuries. However, as we all know, donated blood can often be in short supply. Further, donated blood must always be screened for carrier diseases. In this study, the researchers aim to develop an encapsulation system for hemoglobin (a key component of blood transfusions) which can one day be used as a substitute for donated blood in the clinic. The researchers used PEG (polyethylene-glycol) and PCL (poly caprolactone) as starting materials to build what they term polymersomes, or triblock copolymers consisting of a water-insoluble PCL segment and two water-soluble PEG chains. The hemoglobin-polymer aggregates were mixed into blood plasma substitute solutions. Once encapsulated, the hemoglobin-containing particles were studied to determine that the hemoglobin continued to be reactive. Most importantly, the researchers demonstrated that the encapsulated hemoglobin could continue to bind oxygen. Ascorbic acid was ...
Plasma is a fluid that is part of the blood. It contains proteins that can make your kidneys inflamed. Plasma exchange involves removing some of the plasma from your blood.. During the procedure, youre connected to a machine that gradually removes some of your blood. The plasma is separated from the blood cells and removed. A plasma substitute is then added to the blood before its put back into your body.. Plasma exchange may be used in certain circumstances if your condition is severe.. Read more about plasma. ...
Hypotension is common during surgery and is often treated with intravenous fluid. The choice of fluid depends mainly on local tradition, and colloids were often selected over crystalloids because of their expected ability to expand plasma volume more effectively. Current guidelines do not recommend choosing colloids over crystalloids1 as improvements in outcome have never been established,2 3 but debate continues on the use of colloids in this setting.. In a linked study Opperer and colleagues (doi:10.1136/bmj.h1567) report a retrospective population based analysis in which they evaluated postoperative complications among patients given hydroxyethyl starch, albumin, or crystalloids during or soon after elective joint surgery.4 From a databases containing procedural codes, ICD-9 diagnosis codes, and prescription data, the authors identified a million patients who underwent elective hip and knee replacement surgery during 2009-13 in the United States. The use of either hydroxyethyl starch or ...

In this study, HS-HES and mannitol were both effective in reducing elevated ICP. HS-HES seemed to lower the ICP more effectively, but comparison between the 2 treatments has its limitations, since the optimum dose and infusion rate are largely unknown for both substances. Because of the lack of exact experimental or clinical data, dose, and mode of application, the end point of 10 minutes after the end of infusion for determining efficacy and the definition of treatment "success" in our clinical study are based primarily on personal experiences and general recommendations. Although mannitol is used in many patients with intracranial hypertension, larger dose-finding studies in humans have not been performed. Single doses of mannitol from 0.25 up to 2.27 g/kg body wt have been used.5 27 28 29 Marshall et al4 studied the effect of different mannitol doses in 8 patients and concluded that small doses (0.25 g/kg) were as effective as larger doses. Our dose of 40 g mannitol equals approximately 0.4 ...
Click the title to purchase the article.. Summary:. "We have evaluated the effect of a colloid solution on acute kidney injury in paediatric cardiac surgery. A total of 195 patients were ramdomly divided into an hydroxyethyl starch group and a control group. In the starch group, 6% hydroxyethyl starch 130/0.4 (Volulyte®) was used as the primary fluid for volume resuscitation but was limited to 30 ml.kg−1. In the control group, only crystalloid fluid was used during the peri-operative period. The incidence of acute kidney injury, peri-operative transfusion, clinical outcomes and laboratory data were compared. The incidence of acute kidney injury determined by Paediatric Risk, Injury, Failure, Loss, End-stage renal disease (pRIFLE) and Acute Kidney Injury Network (AKIN) criteria were no different between the two groups (starch group 40.8% vs. control group 30.0%; p = 0.150 using pRIFLE; 19.6% vs. 21.1% respectively, p = 0.602 using AKIN). There were no differences in clinical outcomes such as ...
In the acute normovolemic hemodilution group, intraoperative blood collection was performed to a target hemoglobin of 8.0 g/dL. Low central venous pressure anesthetic technique was used intraoperatively for both groups. A standardized transfusion protocol was applied to all patients intraoperatively and throughout the hospital stay.. The researchers randomized 63 patients to acute normovolemic hemodilution, and 67 to standard anesthetic management from 2004 to 2007.. Demographics, diagnoses, liver function, extent of resection, intraoperative blood loss, operative time, incidence and grade of complications, and length of hospital stay were similar between the 2 groups. The team found that acute normovolemic hemodilution reduced the overall allogeneic red cell transfusion rate by 50% compared with standard anesthetic management. Acute normovolemic hemodilution patients were less often transfused intraoperatively compared with the standard anesthetic management group.. The research team noted that ...

N2 - The objective of this study was to determine the effect of volume expansion with colloid on oxygen consumption (VO2), oxygen delivery (DO2) and the oxygen extraction ratio in patients with severe preeclampsia. The study was prospective and descriptive and took place in the tertiary care labor and delivery unit in a university hospital. Fourteen women with severe preeclampsia who received a pulmonary artery catheter and radial arterial line to monitor their hemodynamic and respiratory status took part in the study. Patients who had received prior medication or volume expansion were excluded. The intervention employed was acute volume expansion, using colloidal solutions (Dextran 70 or ...
Hydroxyethyl starch(HES) has been used for volume resuscitation. It is amylopectin+ hydroxyethyl groups in 0.9% saline or LR. 40-64% of it is excreted by the kidneys. It is reabsorbed via pinocytosis and no enzymes exist to metabolize the intracellular substances and this leads to accumulation of cellular water and osmotic damage. This can also be seen in IV sucrose or dextran induced injury. This is similar to some forms of IVIG induced injury. Risk factors associated with renal injury have been described as prior CKD, DM, old age, large HES volumes,1 liter. We as nephrologists have always been skeptical of this agent as its association with osmotic nephrosis. The first case evert described of osmotic nephrosis with HES was in a kidney transplant patient. A recent NEJM publication looked at a multi centered, blinded trial that randomly assigned patients with severe sepsis to either 6% HES or LR at a dose of 33ml/kg of IBW per day. The outcome was death and dependence of dialysis. The results ...

LEVOPHED should not be given to patients who are hypotensive from blood volume deficits except as an emergency measure to maintain coronary and cerebral artery perfusion until blood volume replacement therapy can be completed. If LEVOPHED is continuously administered to maintain blood pressure in the absence of blood volume replacement, the following may occur: severe peripheral and visceral vasoconstriction, decreased renal perfusion and urine output, poor systemic blood flow despite "normal" blood pressure, tissue hypoxia, and lactate acidosis.. LEVOPHED should also not be given to patients with mesenteric or peripheral vascular thrombosis (because of the risk of increasing ischemia and extending the area of infarction) unless, in the opinion of the attending physician, the administration of LEVOPHED is necessary as a life-saving procedure.. Cyclopropane and halothane anesthetics increase cardiac autonomic irritability and therefore seem to sensitize the myocardium to the action of ...

In the 200 ml group, 10 min after the end of volume expansion, the increase in MSAP and CVP disappears, whereas ITBV, CI and SI increases remain unchanged and even shows for ITBV a trend toward increase. This suggest a post-expansion effect which is not observed on CVP values. In group 2 who had a higher baseline CVP and a lower volume expansion, the increases in MSAP, CVP, ITBV and SI observed at the end of volume expansion disappear 10 min later, suggesting the absence of a post-expansion effect when the baseline CVP is high and/or the volume infused is low. ...
