BACKGROUND: Infants and children undergoing cardiopulmonary bypass become substantially hemodiluted secondary to the volume used to prime the oxygenator. Fresh-frozen plasma has been included in the prime to lessen dilution of clotting factors and correspondingly minimize blood loss and transfusions. METHODS: We prospectively randomized 56 patients weighing 10 kg or less who required cardiopulmonary bypass to receive either one unit of fresh-frozen plasma or 200 mL of albumin 5% in the prime. After protamine administration, samples for prothrombin time, fibrinogen, platelet count, and thromboelastogram were obtained. We analytically and computationally model the interaction of intense laser pulses with different types of plasmas. Such interactions not only excite various forms of collective motion of plasma particles but also open up the possibility to access states of matter only found in massive astrophysical objects. We study the non-linear density structures that are excited due to the laser-driven spatial charge separation between the electrons and ions, forming and sustaining TeraVolts per meter scale electric fields which are applied to particle acceleration and to the generation of x-ray and gamma-ray radiation. The acceleration structures in the plasma are however not completely stable and also affect the lasers exciting them, thus the interaction physics has to be carefully analyzed to ensure stability of the acceleration and transport of particle beams. Treatment failure is a key challenge in the management of HIV-1 infection. We conducted a mixed-model survey of plasma nevirapine (NVP) concentrations (cNVP) and viral load in order to examine associations with treatment and adherence outcomes among Kenyan patients on prolonged antiretroviral therapy (ART). Blood plasma was collected at 1, 4 and 24 hours post-ART dosing from 58 subjects receiving NVP-containing ART and used to determine cNVP and viral load (VL). Median duration of treatment was 42 (range, 12-156) months, and 25 (43.1%) of the patients had virologic failure (VF). cNVP was significantly lower for VF than non- VF at 1hr (mean, 2,111ng/ml vs. 3,432ng/ml, p = 0.003) and at 4hr (mean 1,625ng/ml vs. 3,999ng/ml, p = 0.001) but not at 24hr post-ART dosing. Up to 53.4%, 24.1% and 22.4% of the subjects had good, fair and poor adherence respectively. cNVP levels peaked and were | = 3μg.ml at 4 hours in a majority of patients with good adherence and those without VF. In the present work we have evaluated the effect of decreasing concentrations of normal plasma in TC 199 on leukocyte capillary migration. From the 1st to the 4th hour of migration, normal plasma at 100%-80%-60%-40% concentration, significantly inhibits leukocyte migration. The inhibitory effect is lost, only by a few plasmas, at 60% and 40% concentration, during the following migration hours. Normal plasma at 20% and 10% concentration doesnt show any inhibitory effect on leukocyte capillary migration. Therefore, for clinical studies on pathological plasmas we recommend the use o plasma at 20% or 10% concentration to avoid aspecific inhibition.

To the Editor:. We were surprised to read the article by Blake et al1 on the relationship of soluble CD40 ligand (sCD40L) and lipid accumulation in carotid atheroma. We recently reported an association between inflammatory markers and early onset carotid atherosclerosis.2 Of the patients noted in that report, we had plasma samples from EDTA-anticoagulated blood, stored frozen for 1 to 3 years. Although the product information and manual of the Bender MedSystems ELISA kit clearly states that plasma preparations are not suitable for the essay,3 we decided to run a test on some of these plasma samples for sCD40L. We also used duplicate plasma samples, together with fresh serum and plasma of 5 healthy subjects obtained less than 4 hours before the ELISA test, in the same ELISA plate.
