The aim of this study was to improve the level of diagnosis and differential diagnosis of lymphomatoid papulosis (LyP). Two cases of type B LyP were identified and the literature was reviewed to summarize the clinical outcomes and pathology of LyP and its treatment. The two patients exhibited symptoms with papulonodular lesions, the centers of which gradually underwent ulceration and necrosis. CD30, a helper T-cell marker specifically expressed in tumor cells was analyzed by immunohistochemical staining and the result showed that CD30-negative or only scattered CD30-positive cells were present. Therefore, a diagnosis of type B LyP was made. A fairly good curative effect was achieved following treatment with retinoic acid, glucocorticoids and immunomodulatory drugs. A 38-year old Japanese woman visited our dermatological department on June 24, 2013, with a three-day history of erythematous lesions on the trunk and extremities. She had been treated orally with 50 mg of diaminodiphenyl sulfone (dapson) daily for pityriasis lichenoides et varioliformis acuta (PLEVA) for 38 days. Physical examination on admission revealed fever (38.4℃), bilateral cervical lymphadenopathy, facial edema with erythema, and round erythematous and purpuric plaques on the trunk and extremities. These include a polymorphous eruption with histopathologic findings of Mucha-Habermanns disease, large ulceronecrotic skin lesions, intermittent high fever, and constitutional symptoms. The patient was unique in that he also had malabsorption and eosinophilia. This disease may represent a hypersensitivity reaction. We conducted a prospective study of 20 patients with pigmenting pityriasis alba (PPA) over a period of two years. Characteristic morphology revealed a central zone of bluish hyperpigmentation surrounded by a hypopigmented, slightly scaly halo of variable width. All patients displayed lesions on the face. Concomitant extrafacial involvement was uncommon. A significant finding was an associated dermatophyte infection in 13 patients (65%). These patients all received griseofulvin 10 mg/kg/day for eight weeks, resulting in the resolution of PPA in seven within 4 to 20 weeks. These were also treated with 1% hydrocortisone. Biopsy specimens from two patients showed similar features, namely, a subacute dermatitis with variable pigment incontinence. Immunohistochemical labeling revealed a preponderance of T lymphocytes. Pigmenting pityriasis alba seems to be a variant of classic pityriasis alba showing a strong association with dermatophyte infection, especially tinea capitis. It may be related to ...
To the Editor,. I read with interest the the article Hypopigmented pityriasis versicolor developing on a pre-existing Becker′s naevus by Bhogal, et al (IJDVL 2002;68:43--44). With regards to that article, I have a few comments to make. The authors have reported an interesting, rare occurrence, which may be a future therapeutic application.. In their report, it is mentioned that the association was first reported by Wright in 1979 and then by Signal, et al in 1998. However present authors have not mentioned previous report[1] published in Indian literature, in which hypopigmented pityriasis versicolor occured over pre-existing Becker′s naevus. They also observed reappearance of pigmentation following treatment of pityriasis versicolor, and suggested therapeutic trial with azelaic acid for Becker′s naevus..
