Primary care clinicians often address multiple patient problems, with a range of possible outcomes. There is currently no patient-reported outcome measure (PROM) which covers this range of outcomes. Consequently, many researchers use PROMs that do not capture the full impact of primary care services. In order to identify what outcomes a PROM for primary care would need to include, we conducted interviews with patients and practitioners. This paper reports these patient and practitioners views on the outcomes arising from primary care consultations. Semi-structured interviews were held with 30 patients and eight clinicians across five sites in Bristol. Interviews were audio-recorded, transcribed and analysed thematically. We used a broad definition of health outcome as the impacts of healthcare on health, or a patients ability to impact health to identify outcomes through this process. 10 outcome groups were identified. These occupied 3 domains: Health Empowerment: These are the internal and external
Family physicians demonstrate competence in the patient-centred clinical method; they integrate a sensitive, skillful, and appropriate search for disease. They demonstrate an understanding of patients experience of illness (particularly their ideas, feelings, and expectations) and of the impact of illness on patients lives.. Family physicians use their understanding of human development and family and other social systems to develop a comprehensive approach to the management of disease and illness in patients and their families.. Family physicians are also adept at working with patients to reach common ground on the definition of problems, goals of treatment, and roles of physician and patient in management. They are skilled at providing information to patients in a manner that respects their autonomy and empowers them to "take charge" of their own health care and make decisions in their best interests.. Family physicians have an expert knowledge of the wide range of common problems of ...
to the editor: The article1 and editorial2 on care of patients with human immunodeficiency virus (HIV) and acquired immunodeficiency syndrome (AIDS) in the January 15, 2006, issue of American Family Physician emphasized the important medical services that family physicians can provide for patients infected with HIV, including health care maintenance, baseline evaluation and laboratory studies, and monitoring to guide the initiation of antiretroviral therapy and prophylaxis against opportunistic infections.. The role of family physicians will need to shift toward routine HIV screening for nearly all of our patients. The Centers for Disease Control and Prevention (CDC) recently released new guidelines that recommend making HIV testing routine.3 The new federal guidelines will recommend one-time HIV testing for everyone between 13 and 64 years of age, with annual testing for those with risk factors. The new CDC guidelines are not consistent with those issued by the U.S. Preventive Services Task ...
Many people work behind the scenes to ensure AFP remains the premiere resource for family physicians. Unfortunately, we rarely have the opportunity to recognize everyone who contributes to AFPs success. One such group is the Editorial Advisory Board, which was created decades ago to offer ongoing support and information to the AFP medical editors.. The physicians listed below have volunteered their time and expertise to AFP. They serve as liaisons to their specialties and the organizations they represent. According to Jay Siwek, M.D., AFP editor, these experts, many of whom are family physicians, are responsible for informing him when their organizations develop or revise guidelines that are of interest to family physicians. In addition, they are asked to solicit or write one article each year. Dr. Siwek also relies on them to review manuscripts if the content specifically relates to their specialty.. It is the contributions of these people, in these varied specialties, that help keep AFP up to ...

Now, at the turn of the 21st century, advances in diagnostic techniques mean that very few cases are real diagnostic mysteries. Furthermore, advances in treatment methods mean that physicians are now more focused on formulating appropriate treatment plans than they are on formulating differential diagnoses. These changes in the practice of medicine mean that cases involving diagnostic mysteries suitable for the traditional CPCs are becoming more and more esoteric and less relevant to practice ...
