Phenoxybenzamine (PBZ) is an FDA approved α-1 adrenergic receptor antagonist that is currently used to treat symptoms of pheochromocytoma. However, it has not been studied as a neuroprotective agent for traumatic brain injury (TBI). While screening neuroprotective candidates, we found that phenoxybenzamine reduced neuronal death in rat hippocampal slice cultures following exposure to oxygen glucose deprivation (OGD). Using this system, we found that phenoxybenzamine reduced neuronal death over a broad dose range (0.1 µM-1 mM) and provided efficacy when delivered up to 16 h post-OGD. We further tested phenoxybenzamine in the rat lateral fluid percussion model of TBI. When administered 8 h after TBI, phenoxybenzamine improved neurological severity scoring and foot fault assessments. At 25 days post injury, phenoxybenzamine treated TBI animals also showed a significant improvement in both learning and memory compared to saline treated controls. Participants are not subjected to an experimental treatment of any kind, as we merely aim to describe in detail the perioperative course in general and, in particular, the intraoperative hemodynamic control in patients treated preoperatively with either phenoxybenzamine or doxazosin. A routine diagnostic work-up for pheochromocytoma will be performed in all participants. We examined the effects of dopamine and of a selective DA1 agonist, fenoldopam, on the levels of cyclic AMP in the trabecular meshwork, freshly excised from porcine and canine eyes. As measured by radioimmunoassay, fenoldopam at a concentration of 10(-5) M caused a 4-fold increase in the cyclic AMP content, from a basal level of 24.4 +/- 1.9 to 101.8 +/- 6.3 pmol/mg protein, of the trabecular meshwork samples from porcine eyes. In tissue samples from canine eyes, fenoldopam at a concentration of 10(-4) M increased the endogenous cyclic AMP level from a basal value of 26.0 +/- 4.6 to 64.2 +/- 5.7 pmol/mg protein. The dehydrogenase activity with either succinate or choline as substrate showed no change, whereas that with glycerol 1-phosphate increased. 2. The effect was obtained with noradrenaline, but not with dihydroxyphenylserine. 3. Phenoxybenzamine and yohimbine, but not propranolol, prevented the response to noradrenaline treatment. 4. We investigated the distinct ability of various antihypertensive drugs to modulate the extent (%) of rapid (15 minutes) resetting of the baroreceptors of normotensive rats to hypotensive levels. In one protocol, hemorrhage produced a complete resetting to hypotension in rats chronically treated (6 days) with captopril. Also, hemorrhage produced only partial resetting in rats acutely treated (10-15 minutes before baroreceptor recording) with captopril and in control (untreated) rats (73 +/- 7% and 49 +/- 5%, respectively). In another protocol, all vasodilators produced hypotension in normotensive rats. Nifedipine produced complete (93 +/- 4%) resetting to hypotension, whereas prazosin produced near-maximal (83 +/- 3%) resetting. The remaining drugs studied (phenoxybenzamine, trimethaphan, and MgSO4) induced a partial resetting (63 +/- 7%, 63 +/- 9%, and 50 +/- 5%, respectively) that did not differ significantly from the extent observed with hemorrhage in control (untreated) rats.
