In the present study, C. pneumoniae was detected in throat swab specimens from pediatric patients with lower respiratory tract infections by PCR-EIA and by isolation in cell culture. The organism was recovered in culture from only 1 of 368 specimens tested. The reasons for the low sensitivity of culture in this study are unknown. Sensitivity of cell culture depends on a number of factors, such as sufficient numbers of viable chlamydiae, collection method, transport and storage conditions of specimens, and choice of cell lines. We examined throat swab specimens, and it is possible that these samples did not include enough cells which harbored the organism. We cannot exclude the possibility that the isolation rate ofC. pneumoniae might have been higher with other types of specimens. Comparative studies on the relative efficacy of throat swab, nasopharyngeal swab, and sputum samples for recovery ofC. pneumoniae from culture revealed that positive results were most frequently obtained with sputum ...

We thank Bokov and Delclaux for their thoughtful comments1 on our paper2 and offer the following comments.. The homothety factor, as defined by Bokov and Delclaux,1 describes a structural aspect of the airway tree not directly addressed in our study, which we agree is important and likely also influences airflow in asthma and COPD. However, we believe the suggestion that the two factors, tracheal area and homothety factor, alone characterise the whole bronchial tree omits important structural characteristics, such as … ...
Functionally feeding and swallowing are complex processes divided into 4 phases, as shown in Figures 23-1 and 23-2. The first phase is the preoral phase, which is dependent upon the infant/childs sensation and communication of hunger. The second phase, the oral phase, is the oral cavity-food processing phase where food or liquid is formed into a bolus, enabling safe passage through the pharynx. The next steps in the swallow process are reflexive, and therefore, involuntary. In the ensuing pharyngeal phase, the bolus contacts the tonsillar pillars and pharyngeal wall, resulting in elevation of the larynx, closure of the vocal cords, and relaxation of the esophageal sphincter. Then, contraction of the pharynx deposits the food bolus in the esophagus. During the passage of the food bolus through the pharynx, coordination between breathing and swallowing is essential to prevent aspiration. The final phase, the esophageal phase, moves the food bolus into the stomach and then small intestine, where ...

An oral airway consists of an elongate tubular member having a leading distal end and a proximal end, the leading distal end leading the elongate tubular member as it is inserted into the mouth and pharynx of a patient. An opening at the leading distal end of the elongate tubular member is inclined so that a posterior portion of the distal portion extends beyond an anterior portion of the distal end. A flexible tip portion extends distally beyond the opening to aid in insertion of the oral airway into the mouth and pharynx of a patient. The tip is configured to flex toward the opening as it abuts tissue within the mouth and pharynx of a patient.

When reconstructing combined defects of the cervical spine and the posterior pharyngeal wall the goals are bone stability along with continuity of the aerodigestive tract. We present a case of a patient with a cervical spine defect, including C1 to C3, associated with a posterior pharyngeal wall defect after excision of a chordoma and postoperative radiotherapy. The situation was successfully solved with a free fibula osteo-adipofascial flap. The reconstruction with a fibula osteo-adipofascial flap provided several benefits in comparison with a fibula osteo-cutaneous flap in our case, including an easier insetting of the soft tissue component at the pharyngeal level and less bulkiness of the flap allowing our patient to resume normal deglutition.. ...
The current study demonstrates that MCA, determined by acoustic pharyngometry, can significantly differentiate between those with mild/no-OSA versus moderate-to-severe OSA. When analyzed alongside other variables such as gender, age, and neck circumference, acoustic pharyngometry was the only independent predictor of detecting the absence of moderate-to-severe OSA. This easily obtained measurement thus has potential utility as a component in a diagnostic algorithm for OSA. With healthcare reform, sleep testing may become less readily available, making decisions to prioritize testing in certain patients important.. Acoustic pharyngometry may also be a useful objective tool in occupational health clinics. The population in certain environments may minimize subjective symptoms in sleep questionnaires, leading to the need for simple and cost-effective tools for objective assessments. In conjunction with basic anthropometric characteristics, pharyngometry can help objectively determine those at high ...

chambers of the nasal cavity to prevent damage to the lungs. The nasal and oral cavities are separated by the palate. The anterior, rigid portion is called the hard palate, and the posterior fleshy part is called the soft palate. The mouth and nose serve as secondary respiratory structures. Pharynx The pharynx, or throat, serves both the respiratory and digestive systems and aids in speech. It has a mucous membrane lining that traps microscopic particles in the air and aids in adjusting temperature and humidifying inspired (inhaled) air. The pharynx connects with the mouth and nasal chambers posteriorly. According to its location, the pharynx is referred to as the nasopharynx posterior to the nasal chambers), the oropharynx (posterior to the mouth), or the laryngopharynx (posterior to the pharynx). Epiglottis The epiglottis is a lidlike, cartilaginous structure that covers the entrance to the larynx and separates it from the pharynx. Anatomical optical coherence tomography: a safe and effective tool for quantitative long-term monitoring of upper airway size and shape
The upper airway consists of the pharynx and the nasal cavities; however, some authors include the larynx and trachea as well. The pharynx is can be divided into the nasopharynx, oropharynx, and laryngopharynx.. The nose is composed of bone and cartilage, which are in turn attached to the facial skeleton. It is a pyramidal structure that is divided by a midline septum into two nasal cavities. The nasal cavities are lined with mucosa that can function to heat and humidify inspired gas. The paranasal sinuses drain into the nasal cavity. The posterior portion of the mouth opens into the oropharynx. When a patient is supine and unconscious, the tongue and lower jaw may slide posteriorly leading to airway obstruction within the oropharynx.. The pharynx is a U-shaped fibromuscular tube that extends from the base of the skull to the cricoid cartilage. It is bounded anteriorly and superiorly by the nasal cavity, followed more inferiorly by the mouth, and then the larynx. These borders divide the pharynx ...

Hello again, I wanted to let you know something I learned about throat cultures. They do not always show what is growing in the lungs so it is better to do a sputum culture. It took me about 17 years to realize this, I do not know why my doctors never thought to do sputum cultures and test them against the throat cultures, as they, for me yielded entirely different results. Just for the future you might want to start having him do sputum cultures (when hes older) because it comes directly from the lungs and not so much of the flora in the back of the throat persay. I say when hes older because I can imagine it would be a lot harder to get a baby to hack or cough up a culture ...
After registering the before and after cone-beam volumetric tomography data using the basion region as a reference point, we can see the changes that have taken place in the pharyngeal airway, the maxilla and mandible, and the inferior turbinate. The green area shows the pre-treatment airway and tooth position. (Please note the lower central incisor as well as the airway.). We see that the maxilla and the mandible have come forward along with the tongue. Furthermore, we see that the posterior pharyngeal wall has toned up to widen the airway. We have a 360 degree improvement of the airway space. The lower turbinate has increased in size.. ...
In people suffering from obstructive apnoea, there is repetitive upper airway collapse during sleep. This condition affects four per cent of males and two per cent of females. The reason why airway collapse occurs in some, and not in others, is due to differences in airway anatomy - e.g., narrow passage, extended length of the pharynx [throat], and poor control of upper airway muscles that are responsible for keeping the airways open [dilators].. During wakefulness, our airways remain open, due to the high activity of upper airway muscles. However, when we sleep, this activity is reduced, or lost. It may also result in partial, or complete upper airway collapse. Other unknown reasons may also contribute to collapse.. During airway obstruction, there is no breathing taking place. The levels of oxygen in blood fall, and those of carbon dioxide rise. The body tries to compensate this change by increasing the force of breathing, but this doesnt work. Hence, such episodes often lead to arousal from ...
If you are wondering whether your painful sore throat is from a cold, allergies or strep here is help with how to tell the difference.. Sore throats can be caused by viruses or bacteria. The most common cause of sore throats are viruses. Viral sore throats are often accompanied by other cold symptoms that may include a runny nose, cough, red or watery eyes and sneezing. Other causes of sore throats include smoking, pollution or irritants in the air, allergies and dry air.. A sore throat is often the first sign of a cold or allergy congestion. Often a sore throat from a cold will get better or go away after the first two or three days.. Strep throat, which is an infection due to streptococcus bacteria, is another cause of a sore throat. With strep throat the soreness in the throat is often more severe and persists. In general the common symptoms of strep throat include a sore throat, fever, red and inflamed throat and swollen or tender lymph nodes on the sides of the upper neck.. People with ...
In most cases, it is hard to tell the difference between a viral sore throat and a strep throat based on physical exam. It is important, though, to know if the sore throat is caused by strep bacteria, as this calls for antibiotics to help prevent the complications that can happen with these bacteria.. As a result, most people, when they have the above symptoms, will get a strep test and throat culture to find out if it is an infection caused by strep. This will involve a throat swab in the healthcare providers office.. Quick tests, called rapid strep tests, may be done. This may immediately show as positive for strep and antibiotics can be started. If it is negative, part of the throat swab will be kept for a throat culture. This will further identify strep in 2 to 3 days. Your healthcare provider will discuss the treatment plan with you based on the findings. ...
These Perspectives (SIG 19) articles provide with information relevant to speech science research and education. Lulich and Pearson present two demonstrations in this technical report to illustrate the utility of 3D/4D ultrasound technology. First, the authors report that not only can structures be imaged which previously were impossible to identify from 2D ultrasound alone (e.g., piriform sinuses and posterior pharyngeal wall), but questions involving non-sagittal structures and asymmetrical tongue shapes, such as the pervasiveness and extensiveness of lateral contact between the tongue and the palate-teeth, can now be addressed non-invasively. Second, they also conclude that the fusion of ultrasound data with MRI images further enhances the utility of 3D/4D ultrasound, since it combines the strengths of ultrasound with the complementary strengths of the other modality, while mitigating the weaknesses of each. Richardson et al., compare various acoustical measures of sustained vowels ...
Lets discus the common and not so common neck cancer symptoms.. Neck cancer could occur in the pharynx, larynx, oropharynx or the Hypopharynx. Pharynx is a 5 to 6 inch hollow pipe and starts behind the nose and extends to the oesophagus. It enables the passage of food through its lumen. Larynx is also called as the voice box and contains the vocal cords and helps in phonation. The oropharynx is the part of the pharynx which connects the pharynx to the top of the throat. Hypopharynx is a part of the pharynx that is present behind and on either sides of the larynx. Neck cancer is more common in men and commonly affects people over the age of 50 years. Tobacco and alcohol use are the most common cause of oral and throat cancer. Eighty five percent of neck cancers are caused due to tobacco or alcohol usage. Those using tobacco and alcohol are at a higher risk of developing neck cancer as against those using either one of them. Nasopharynx cancers are more common among those of Asian origin, those ...
The present issue of the European Respiratory Review strongly emphasises the relevance of small airways disease in respiratory medicine. This fascinating topic is fast-growing and certainly represents a key aspect of current respiratory research. As stated by P.J. Sterk and E.H. Bel in their editorial, there is a big challenge with small airways! Thus, it is timely to investigate novel tools that can accurately evaluate airway anatomy and function in health and disease, as well as responses to therapy.. Imaging of the airways is the focus of the textbook CT of the Airways edited by P.M. Boiselle and D.A. Lynch. Imaging of the airways is a true art in medicine and this book is certainly an important help to any health professional requiring up-to-date information.. The introductory section of the book addresses airway anatomy and physiology, as well as pathology. The fi ve chapters in this section are an excellent introduction to the fi eld. The second part of the book discusses very important ...

Obese children have more respiratory symptoms than their normal weight peers and respiratory related pathology increases with increasing weight. Some will need specialist assessment (box 1). Obesity produces mechanical effects on respiratory system performance. Breathlessness, wheeze, and cough are not related to increased airway responsiveness and may respond more to weight loss than bronchodilator therapy. A significant number of obese children have signs and symptoms of obstructive sleep apnoea largely related to the effect of obesity on upper airway dimensions. It seems likely that unless action is taken soon, increasing numbers of children will experience preventable respiratory morbidity as a result of nutritional obesity ...

The main findings of this study were that older males had greater pharyngeal calibre measured using acoustic reflection, and a longer combined retropalatal and retroglossal pharyngeal length, and larger soft palate and parapharyngeal fat pad size measured using MRI than younger males. These age-related pharyngeal morphological changes were independent of BMI and neck circumference.. The impact of age on pharyngeal calibre has been debated, with some [14]-[16] but not all [10], [17]-[20] studies finding greater pharyngeal calibre in older compared with younger males. This equivocal evidence may be due to poor screening and exclusion of undiagnosed OSA, as the prevalence of OSA is known to increase with age [1], [2]. In the present study, appropriate screening ensured the exclusion of undiagnosed OSA, with groups well matched for BMI, neck circumference, subjective daytime sleepiness, AHI, ODI and craniofacial morphology. This approach created a model to establish the effect of age per se on ...

With the weather being so wacky, the kids are all getting sick. We go from 40/50 degrees down to -9 pretty much over night. My daughter slowly got a sore throat, and a lite cough. While my nieces got sore throats, fevers, and pains in their stomachs. I am not sure why my daughter has not got strep but both my nieces currently have it.. Do you know how to tell if your child has strep throat, or just a common cold? Strep throat usually gives you certain symptoms. The main being of course a sore throat. My nieces have been fussing for the past few days of sore throats, followed with a temperature, I knew pretty much right away it was strep.. You cannot tell a child has strep just by looking at their throat, in my experience with strep, it is usually a sore throat followed by a fever. My nieces have been running a fever off and on for the past 4 days. If strep throat is left untreated they can also develop a nasty ear infection.. With the weather changing so much, the kids are bound to keep getting ...

However, these findings are not diagnostic as they can be seen in viral infections as well.. COVID-19 viral nucleic acid is extracted from cough throat swab samples. Real-time reverse transcriptase PCR (RT-PCR) is performed by use of 1-step reverse transcription of RNA from the sample and then amplified by PCR. Internal positive and negative controls are included to ensure that each test performs correctly. The US Centers for Disease Control and Prevention (CDC) designed RT-PCR assays for the detection of 2019-nCoV. Our test uses the same primers and probes used in CDC assays and has comparable in sensitivity and specificity to CDC assays. Sensitivity is ~98% and specificity is ~99.9%. This test is recommended by the FDA only under the current EUA (for emergency use) guidance, unless the authorization is terminated or revoked sooner. Aperiomics has submitted an application for FDA EUA (for emergency use) for Xplore-COVID-19™ testing. The testing is only for the detection of RNA from COVID-19 virus and not for any other viruses or pathogens.. ...

Patients with a moderate pharyngeal collapsibility and low loop gain are likely to respond to MAD therapy, i.e. characteristics that have been identified as beneficial with respect to the mechanism of action of MAD treatment [8, 14]. Milder OSA patients have also been recognised as responders. The common use of the AHI to identify this complex disease and make recommendations for treatment has, however, been questioned [15]. The AHI has been found to be a weaker predictor to identify MAD treatment responders than previously believed and severe patients might also have a good outcome [16-18]. Instead, more advanced ways to interpret sleep apnoea recordings are under development [19, 20]. This will allow the identification of various phenotypes of sleep apnoea patients and provide a more promising way forward to find patients who respond to OSA treatment in different ways.. Non-responders to MAD therapy are more likely among severe OSA patients with non-positional OSA [21-23]. Evaluation of the ...
