This study attempted to give an insight in the implementation of clinical pharmacy service in the public hospitals of Ethiopia. Apart from assessing strength and limitations of the service, it also sheds light on the level of job satisfaction of newly graduated pharmacy professionals practicing in the surveyed hospitals.. Most of the new graduates were working in internal medicine and pediatrics wards. This may be due to the high prevalence of drug related problems in the internal medicine ward and vulnerability of the patient population in the pediatric ward. The graduates were also assigned in the dispensary due to shortage of pharmacists, which was compounded by implementation of workforce intensive initiatives. This in turn affected the clinical pharmacy service expected to be provided in the wards. Clinical pharmacy service has evolved steadily over the past few decades and is contributing to the patient care journey at all stages. The service improves safety and effectiveness of medicines, thereby avoiding medication errors. As part of this global shift in pharmacy education and practice, Ethiopian Universities revamped the undergraduate pharmacy curriculum and the first graduates came out in July 2013. These graduates were immediately deployed in public hospital settings, with the ultimate aim of providing clinical pharmacy services. As such an initiative is new to the Ethiopian pharmacy sector, there is a need to do assessment of the health care providers perception and satisfaction towards the service. A cross-sectional survey using self-administered questionnaire was conducted in six regions and one-city administration of the country. However, as pharmacists roles have expanded, so, too, have patient numbers. Given the fact that HIT predisposes strongly to new episodes of thrombosis, it is not sufficient to simply discontinue the heparin administration. Generally, an alternative anticoagulant is needed to suppress the thrombotic tendency while the generation of antibodies stops and the platelet count recovers. To make matters more complicated, the other most commonly used anticoagulant, warfarin, should not be used in HIT until the platelet count is at least 150 x 10^9/L because there is a very high risk of warfarin necrosis in people with HIT who have low platelet counts. Warfarin necrosis is the development of skin gangrene in those receiving warfarin or a similar vitamin K inhibitor. If the patient was receiving warfarin at the time when HIT is diagnosed, the activity of warfarin is reversed with vitamin K.
