There is a growing interest in high income countries to control expenditure on medicines by improving the rationale for their selection. However, in middle income countries with differing priorities and needs, little attention has been paid to this issue. In this paper we explore the policies and processes for the selection and use of medicines in a group of hospitals in Chile, a middle income country which has recently joined the OECD. A combination of qualitative and quantitative methods was used. A national survey questionnaire was distributed to investigate the role and operation of PTCs (Pharmacy and Therapeutics Committees). Interviews were conducted with key actors in the selection of medicines in large urban public hospitals. The national survey had an overall response rate of 42% (83 out of 196), whilst 7 out of 14 hospitals participated in the qualitative study. BACKGROUND Pain management experts have recommended a diminished role for pethidine because of lack of superiority for any indication and greater potential for adverse effects compared with morphine. OBJECTIVES To evaluate the impact of a multifaceted education initiative on prescribing patterns and to minimize pethidine use in the ED. METHODS This was a pre- and post-intervention study. The intervention was an education initiative, introduced between December 2003 and January 2004. It comprised ED pharmacist in-services for medical and nursing staff, championing by senior medical staff (ED consultants, surgical units, Drug and Therapeutics Committee) and concurrent prescriber feedback by ED pharmacists. The pre-intervention prescribing practices guided the development of in-services and the educational programme. The intervention was evaluated by a medical record review of analgesic prescribing in March-April of both 2003 (pre-intervention period) and 2004 (post-intervention period). Outcome

It is the job of the stewardship pharmacist to understand these trends in a quantitative way and to respond in a dynamic fashion. In addition to the active management of restricted antibiotic agents, the stewardship team is responsibility for the development of a large share of the hospitals policies and procedures regarding appropriate antimicrobial use. One of the facts that seems sometimes to escape the notice of university faculty (especially early-career faculty) is that a sizable proportion of university administrators used to be regular faculty members. Fortuitously, my early encounters with administrators kept underlining this fact for me.. When I started my appointment at my university, the Associate Dean of my college was a member of the Philosophy Department. He wasnt teaching any courses for the department then (because Associate Dean-ing was a full-time gig), but he was very much connected to the culture of the department. By engaging in this search, designers not only improve solution quality but also begin to learn what operational patterns might improve the solution in future iterations. Previous work in psychology has demonstrated that humans can fluently and adeptly learn short operational sequences that aid problem-solving. This paper explores how designers learn and employ sequences within the realm of engineering design. Specifically, this work analyzes behavioral patterns in two human studies in which participants solved configuration design problems. Behavioral data from the two studies is first analyzed using Markov chains to determine how much representation complexity is necessary to quantify the sequential patterns that designers employ during solving.
