The rapid development of new technology used in obstetrics and neonatology has been accompanied by new moral problems presented by the choices this technology generates. Ethics and Perinatology is a timely examination of the moral problems caused by clinical and research practices in perinatology. 14 major issues are each addressed by a leading clinician whose writing is then critically reviewed by a medical ethicist, a legal theorist, or an economist.

Introduction Pregnancy and childbirth are unique experiences in a familys lifetime. They are time periods of highest emotional sensitivity. Also, they may constitute substantial health risks for both the mother and the infant. With the advent of modern neonatal intensive care, ethical dilemmas have arisen more often in perinatology. Decisions on intensive vs. compassionate care for critically ill infants may have long-term emotional and mental health effects on parents. A questionnaire among parents of deceased newborns after delivery room resuscitation showed that predictions of morbidity and mortality were not central to their decision-making. However, religion, hope, spirituality and compassion were mentioned as being most valuable guidance to decision-making regarding delivery room resuscitation. Little is known about German professionals views regarding the role of religion and spirituality in perinatology. We therefore administered a cross sectional survey to medical professionals ...

Maternal-Fetal Medicine physicians, also called perinatologists, are obstetricians who specialize in the diagnosis, treatment, and ongoing care of expectant mothers and their unborn babies, who may be at high risk for special health problems.. Referral to a Maternal-Fetal Medicine specialist should not cause undue alarm for expectant mothers. It does not necessarily mean the pregnancy will be a difficult one. Nor does it mean the baby will have health problems. Most often, it is simply a precautionary measure designed to protect both mother and a baby, while putting the expectant parents minds at ease.. Services provided by Maternal-Fetal Medicine physicians include maternal and fetal medicine care, monitoring, ultrasound examinations, genetic testing, state-of-the-art diagnostic tests, tests for fetal well-being and more.. As the due date approaches, the Maternal-Fetal Medicine physician remains in close contact with the obstetrician who will deliver the baby and, if needed, the neonatologist ...

Birth Defects What is a birth defect? A birth defect is a health problem or physical change, which is present in a baby at the time he/she is born. Birth defects may be very mild, where the baby looks and acts like any other baby, or birth defects may be very severe, where you can immediately tell there is a health problem present. Some of the severe birth defects can be life threatening, where a baby may only live a few months, or may die at a young age (in their teens, for example). Birth defects ar ...

The main finding of the current study was the significant lower level of serum folic acid in women with PTB and folic acid level was associated with lower risk of PTB. This goes with the previous studies where Chen et al., have reported that higher plasma folate concentrations were associated with a longer gestational age and associated with lower risk of PTB [9]. Likewise Furness et al., have shown that women who had a lower Red blood cells (RBCs) folate level in early (12-14 weeks) pregnancy were 5.4 times at higher risk to have PTB [11]. In a large clinical trial, it has recently been shown consumption of folic acid/ other micronutrient in the first trimester lead to a 41%-45% risk reduction for PTB [17].. In contrast to our findings, it has recently been reported that supplemental folate intake was not significantly associated with the risk of PTB [12]. Interestingly, women who received folic acid supplementation more than 8 weeks before conception were at increased risk for PTB compared ...
Introduction: Polyhydramnios is a condition in which the amount of amniotic fluid increases more than 2 litres. The causes of polyhydramnios are multifactorial, and most are idiopathic. Here we report challenging management of polyhydramnios case without apparent risk factors.. Case: A 30-year-old woman G2P1001, 27-28 weeks of gestational age referred due to our tertiary health care center due to polyhydramnios. The patient chief complaint was progressively enlarged abdomen since a month ago. Her previous pregnancy, medical history and family history were unremarkable. The obstetric examination reflects the size of uterus equivalent to 36 weeks of gestational age. The fetomaternal ultrasound reveals a life, single fetus with normal heart rate and fetal movement, estimated fetal weight about 1057 grams, placenta corpus posterior grade I, Maximum Vertical Pocket 22.83 cm, no major congenital abnormalities were visible and bladder appears filled. Laboratory tests revealed blood glucose levels 90 ...

Soooo I asked him his recommendation on when we could start trying again. He said well THE recommendation is to wait a year...that being said, when you are ready, you should start when you want. Your body is healthy enough to carry another baby and I have no doubts you will have a healthy baby in the future. He said he would run a bunch of labs on me to screen for any possible things which would affect subsequent pregnancy. I had about 20 vials of blood drawn. The only thing that he found particularly interesting is the difference in weight between my two children. Kai was 8 lbs 12 oz...a big baby. Camille was 5 lbs 5 oz. She was measuring 2.5 weeks small on the ultrasound but that was still within normal limits. I figured my son was a large baby and my daughter was more the size my husband and I were. I weighed 6 lbs some odd ounces and my husband 7 lbs some odd ounces. Since she looked normal and the AFP screen came back negative (he said this is a good thing), it was probably not a ...

ANZARD includes data items that make it possible to follow a womans consecutive ART treatment cycles. A cohort of 16,355 women were followed from the start of their first autologous non freeze-all fresh cycle during 2013, through subsequent fresh and thaw cycles until December 2015 or until they achieved a live delivery. The cycle-specific live delivery rate per initiated autologous cycle for all women was 23.3% in their first cycle, and 14.5% after eight cycles. Of women who did not achieve a live birth in a specific cycle, approximately one in four did not return for further ART treatment. ...

Marginal structural models (MSMs) are causal models designed to adjust for time-dependent confounding in observational studies of time-varying treatments. MSMs are powerful tools for assessing causality with complicated, longitudinal data sets but have not been widely used by practitioners. The objective of this paper is to illustrate the fitting of an MSM for the causal effect of iron supplement use during pregnancy (time-varying treatment) on odds of anemia at delivery in the presence of time-dependent confounding. Data from pregnant women enrolled in the Iron Supplementation Study (Raleigh, North Carolina, 1997-1999) were used. The authors highlight complexities of MSMs and key issues epidemiologists should recognize before and while undertaking an analysis with these methods and show how such methods can be readily interpreted in existing software packages, including SAS and Stata. The authors emphasize that if a data set with rich information on confounders is available, MSMs can be used ...

Obstetricians (also called OBs) are medical doctors who specialize in the care of pregnant women, from the time of conception through delivery and the period following delivery (postpartum). Most obstetricians are also gynecologists who diagnose and treat diseases of the female reproductive system and provide care for women when they are not pregnant. An obstetrician can further specialize in perinatology (maternal-fetal medicine), which focuses on high-risk pregnancies, testing, and fetal development, or an obstetrician may specialize in reproductive endocrinology and infertility (care of women who have hormonal or fertility problems).. ...
Significant findings published in the American Journal of Obstetrics and Gynecology. Women with a short cervix should be treated with vaginal progesterone to prevent preterm birth, according to a landmark study by leading obstetricians around the world. Vaginal progesterone decreased the rate of preterm birth by 42%, and significantly reduced the rate of respiratory distress syndrome and the need for mechanical ventilation, as well as a composite of several complications of premature newborns (e.g. infection, necrotizing enterocolitis, intracranial hemorrhage, etc.). An early online version of the study was published today in the American Journal of Obstetrics and Gynecology (AJOG), and is available on the AJOG website free of charge.. Our analysis provides compelling evidence that vaginal progesterone prevents preterm birth and reduces neonatal morbidity/mortality in women with a short cervix, said lead investigator Dr. Roberto Romero, Chief of the Perinatology Research Branch and Head of the ...
New evidence that genetics play a significant role in some premature births may help explain why a woman can do everything right and still give birth too soon.. Research presented today at the 30th Annual Society for Maternal-Fetal Medicine (SMFM) meeting • The Pregnancy Meetingâ ¢ • showed that the genes of both the mother and the fetus can make them susceptible to an inflammatory response that increases the risk of preterm labor and birth.. Silent, undetected infections and inflammation are major risk factors for preterm labor and birth, says SMFM member Roberto Romero, MD, Chief of the Perinatology Research Branch at the National Institute of Child Health and Human Development. One of every three preterm births occurs to a mother who has an infection in her uterus, but has no symptoms.. Dr. Romero led a team of physicians and scientists studying a large number of genes involved in the control of labor that could help explain the complex process that triggers preterm birth. They ...

Their study, Placenta-on-a-Chip: A novel platform to study the biology of the human placenta, was published online in the Journal of Maternal-Fetal & Neonatal Medicine. We believe that this system may be used to address questions that are difficult to answer with the current placenta model systems and serve to enable research in pregnancy and its complications, said Roberto Romero, M.D., D.Med.Sci., chief of the Perinatology Research Branch, professor at the Center for Molecular Medicine and Genetics at Wayne State University, and an attending physician at the Detroit Medical ...

New York States system of regionalized peri-natal services includes a hierarchy of four levels of peri-natal care provided by the hospitals within a region and led by a Regional Peri-natal Center (RPC).. The regional system is led by an RPC that is capable of providing all the services and expertise required by the most acutely sick or at-risk pregnant women and newborns. The concentration of high-risk patients makes it possible to enhance and maintain the level of expertise in the care of high-risk obstetric and neonatal patients, as well as justify the substantial expense required to establish and maintain neonatal intensive care units and attending-level subspecialty consultation. RPCs provide or coordinate maternal-fetal and newborn transfers of high-risk patients from their affiliate hospitals to the RPC, and are responsible for support, education, consultation and improvements in the quality of care in the affiliate hospitals within their region.. The four levels of perinatal care within ...
