Retrograde perfusion (retroperfusion) is an artificial method of providing blood supply to an organ by delivering oxygenated blood through the veins. It may be performed during surgery that interrupts the normal arterial supply of blood to that organ. For instance, when performing surgery that interrupts the cerebral arteries, a hose placed into the femoral artery and the superior vena cava can redirect blood up the internal jugular vein to supply the brain. This technique was pioneered by Oscar Langendorff, who perfused mamallian hearts ex vivo for research applications. Thus, it is often called Langendorff perfusion. Introduction: Increasing the usage of hearts from marginal donors and from donors after circulatory death (DCD) has the potential to expand the donor pool. Such hearts may be resuscitated when subjected to ex vivo perfusion. In addition, this may provide an opportunity for viability testing prior to transplant. We describe a novel ex vivo perfusion system designed to reanimate porcine DCD hearts. The same system was subsequently tested using a human heart from a marginal brainstem-dead donor.. Method: In the first phase of the study 23 porcine hearts were procured following circulatory death. The effects of acetylcholine on glucose and lactate balance and on perfusion flow were studied in isolated rat livers perfused simultaneously via the hepatic artery (100 mmHg, 25-35% of flow) and the portal vein (10 mmHg, 75-65% of flow) with a Krebs-Henseleit bicarbonate buffer containing 5 mM-glucose, 2 mM-lactate and 0.2 mM-pyruvate. Arterial acetylcholine (10 microM sinusoidal concentration) caused an increase in glucose and lactate output and a slight decrease in arterial and portal flow. These effects were accompanied by an output of noradrenaline and adrenaline into the hepatic vein. Portal acetylcholine elicited only minor increases in glucose and lactate output, a slight decrease in portal flow and a small increase in arterial flow, and no noradrenaline and adrenaline release. Intestinal segments (5 ± 1 cm) were everted according to the method described by Wilson & Wiseman. After being everted, segments of guts were blotted with a piece of whatman filter paper no. 40 and weighted. A 1 g glass weight was fixed and tied to the end of the everted gut segment to make an empty gut sac. This was important to prevent peristaltic muscular contractions, which may otherwise alter the shape and internal volume of the sac. The 1 g glass weight was the minimum weight to secure the above-mentioned conditions. After weighing, the empty sac was filled with 0.5 mL of Krebs-Henseleit bicarbonate buffer (KHB). To overcome the present shortage of liver donors by expansion of the existing donor pool and possibly lengthening of the storage time, hypothermic machine perfusion of the liver as a dynamic preservation method is revisited. The three most important aspects are defined to be the type of preservation solution, the characteristics of perfusion dynamics, and the oxygen supply. Reviewing hypothermic liver machine perfusion experiments, the University of Wisconsin machine preservation solution is the solution most used. It is also found that nothing conclusive can be said about the optimal perfusion characteristics, since either perfusion pressure or perfusion flow is reported. The best estimation is perfusion of the liver in a physiological manner, i.e. pulsatile arterial perfusion and continuous portal venous perfusion. Esophageal manometric systems with water-filled catheters have been characterized by the use of model experiments. The examined parameters have been: catheter dimension, catheter compliance, catheter resistance, pump type, pump compliance, and perfusion flow. Accurate static pressure measurements have been obtained for perfused systems independently of the investigated parameters. The dynamic characteristics vary with catheter diameter and perfusion flow. For catheters with low diameter, a narrow bandwidth is obtained for the investigated perfusion flows. The results have been expressed in terms of an electric model of the measurement system. BACKGROUND AND PURPOSE: To examine radiation-induced changes in regional lung perfusion per dose level in 58 non-small-cell lung cancer (NSCLC) patients treated with intensity-modulated radiotherapy (IMRT).. MATERIAL AND METHODS: NSCLC patients receiving chemo-radiotherapy (RT) of minimum 60 Gy were included prospectively in the study. Lung perfusion single-photon emission computed tomography (SPECT/CT) was performed before and serially after RT. Changes (relative to baseline, %) in regional lung perfusion were correlated with regional dose. Toxicity outcome was radiation pneumonitis (RP) CTC grades 2-5.. RESULTS: Perfusion changes were associated with dose. Dose-dependent reduction in regional perfusion was observed at 3, 6 and 12 months of follow-up. Organ preservation plays a major role in improving the clinical outcome of transplantation. Extracorporeal perfusion systems have been developed for solid-organ transplants (e.g. liver) and recently, investigators have demonstrated incremental improvements in preserving porcine vascularized composite allografts (VCA) viability through ex vivo perfusion. Yet, there is a burgeoning need for novel technologies capable of non-invasive, real-time monitoring of the tissue viability prior to transplantation, which would significantly increase the efficacy and availability of life-changing VCA. Thereby, following tissue procurement, lactate, a key biomarker of stress increases and the main source of metabolically-produce acid responsible for tissue acidosis. Lactate levels have also been found to correlate with the severity of injury and decrease in oxygen availability, making lactate a surrogate for oxygen debt. Small molecules such as lactate and glucose exists in equilibrium between the interstitial and
