High blood cholesterol is a leading risk factor in the development of atherosclerosis and coronary heart disease (CHD) (1,2). The risks associated with high blood cholesterol can be reduced by screening and early intervention (3). Current clinical practice guidelines provide evidenced-based standards for detection, treatment, and control of high blood cholesterol (4). Healthy People 2020 monitors national progress related to screening and controlling high blood cholesterol through the National Health Interview Survey and the National Health and Nutrition Examination Survey (NHANES). State-level estimates of self-reported cholesterol screening and high blood cholesterol prevalence are available using Behavioral Risk Factor Surveillance System (BRFSS) data. To assess recent trends in the percentage of adults aged ≥18 years who had been screened for high blood cholesterol during the preceding 5 years, and the percentage among those who had been screened within the previous 5 years and who were ...

In the literature on Sen's capability approach, studies focusing on the empirical measurement of conversion factors are comparatively rare. We add to this field by adopting a measure of 'conversion efficiency' that captures the efficiency with which individuals convert their resources into achieved functioning. We use a non-parametric efficiency procedure ... read more borrowed from production theory and construct such a measure for a set of basic functionings, using data from the 2005 wave of the British Household Panel Survey. In Great Britain, 49.88% of the individuals can be considered efficient while the mean of the inefficient individuals reaches one-fifth less functioning achievement. An individual's conversion efficiency is positively affected by getting older, being self-employed, married, having no health problems and living in the London area. On the other hand, being unemployed, separated/divorced/widowed and (self-assessed) disabled decrease an individual's conversion efficiency ...
Over a third of people with HIV have non-alcoholic fatty liver disease (NAFLD) in the absence of hepatitis B or C, according to the results of a meta-analysis and systematic review published in AIDS. Metabolic disorders including high body mass index (BMI), diabetes and elevated lipids were key risk factors. The study also revealed high prevalences of non-alcoholic steatohepatitis (NASH) and liver fibrosis, possible outcomes of NAFLD, with metabolic disorders once again shown as the most important factors.. "NAFLD is common in HIV-monoinfected patients. Metabolic disorders are key risk factors for NAFLD independent of HIV parameters and predict its complications," comment the authors. "Our systematic review underlines the need for additional data on NAFLD in HIV infection as well as a better standardised assessment and management of the disease.". Liver disease is now a leading cause of serious illness and death in people with HIV. Until now, much of the liver disease occurring in people living ...

The roles of different dietary proteins in the aetiology of type 2 diabetes (T2D) remain unclear. We investigated the associations of dietary proteins with the risk of incident T2D in Finnish men from the prospective Kuopio Ischaemic Heart Disease Risk Factor Study. The study included 2332 men aged 42-60 years at the baseline examinations in 1984-1989. Protein intakes were calculated from 4-d dietary records. Incident T2D was determined by self-administered questionnaires, fasting blood glucose measurements, 2-h oral glucose tolerance tests, and with national registers. The multivariable-adjusted risk of T2D on the basis of protein intakes was compared by the Cox proportional hazard ratios (HR). During the mean follow-up of 19·3 years, 432 incident T2D cases were identified. Total, animal, meat or dairy product protein intakes were not associated with risk of T2D when the potential confounders were accounted for. Plant (multivariable-adjusted extreme-quartile HR 0·65; 95 % CI 0·42, 1·00; P ...
This study examined the psychometric properties of the Revised Illness Perception Questionnaire adapted for a clinical sample of low-income Latinos suffering from depression. Participants (N = 339) were recruited from public primary care centers. Their average age was 49.73 years and the majority was foreign born females of either Mexican or Central American descent. Confirmatory factor analysis was used to test the factor structure of this measure. Construct and discriminant validity and internal consistency were evaluated. After the elimination of three items because of low factor loadings (| .40) and the specification of seven error covariances, a revised model composed of 24 items had adequate goodness-of-fit indices and factor loadings, supporting construct validity. Each of the subscales reported satisfactory internal consistency. Intercorrelations between the 5 illness perception factors provided initial support for the discriminant validity of these factors in the context of depression. The

This study was undertaken to investigate the associations of blood vessel invasion (BVI), lymphatic vessel invasion (LVI) or other variables and long-term survival in 173 Japanese and 184 British patients with primary invasive breast cancer, and whether they are associated with survival differences between Japanese and British patients. BVI was detected by objective methods, using both factor VIII-related antigen (F-VIII) staining and elastica van Gieson (E v G) staining. BVI was classified into three subtypes. 1) BVI e, BVI detected by E v G staining alone, 2) BVI f, BVI detected by F-VIII staining alone, 3) BVIef, BVI evaluated by combining BVIf and BVIe. LVI was also detected by objective methods, using lymphatic vessel endothelial hyaluronan receptor-1 (LYVE-1) staining alone. There was a borderline significance between the frequencies for BVIef of British patients and those of Japanese patients (8.2% vs 3.5%; P = 0.06) but not for LVI (P = 0.36). British patients had a significantly worse relapse

Hearing problems are a significant public health concern. It has been suggested that psychological distress may represent both a cause and a consequence of hearing problems. Prospective data that allow testing such potential bi-directionality have thus far been lacking. The present study aimed to address this knowledge gap. Random (RE) and fixed effects (FE) panel regression models estimated the association of psychological distress (GHQ-12) and participant-reported hearing problems. Data from 18 annual waves of the British Household Panel Survey were used (n?=?10,008). Psychological distress was prospectively associated with self-reported hearing problems in women (multivariable odds ratios (ORs) ≥1.44; one-year time lag?≥?1.16) and men (ORs?≥?1.15; time lag?≥?1.17). Conversely, self-reported hearing problems were associated with increases in psychological distress in both sexes (OR?≥?1.26; time lag?≥?1.08). These associations were independent of the analytical strategy and of ...
Neighbourhood characteristics affect the social and economic opportunities of their residents. While a number of studies have analysed housing adjustments at different life stages, little is known about neighbourhood quality adjustments. Based on a model of optimal housing consumption we analyse the determinants of residential mobility and the neighbourhood quality adjustments made by those who move, drawing on data from the British Household Panel Survey and Indices of Multiple Deprivation. We measure neighbourhood quality both subjectively and objectively and find that not all life-course events that trigger moves lead to neighbourhood quality adjustments. Single people are negatively affected by leaving the parental home and couples by a husband s unemployment. Couples having a new baby move into better neighbourhoods.. ...
3 Department of Epidemiology, School of Public Health, Loma Linda University, Loma Linda, CA. Whole blood viscosity, plasma viscosity, hematocrit, and fibrinogen are considered independent risk factors for coronary heart disease and can be elevated by dehydration. The associations between fatal coronary heart disease and intake of water and fluids other than water were examined among the 8,280 male and 12,017 female participants aged 38-100 years who were without heart disease, stroke, or diabetes at baseline in 1976 in the Adventist Health Study, a prospective cohort study. A total of 246 fatal coronary heart disease events occurred during the 6-year follow-up. High daily intakes of water (five or more glasses) compared with low (two or fewer glasses) were associated with a relative risk in men of 0.46 (95% confidence interval (CI): 0.28, 0.75; p trend = 0.001) and, in women, of 0.59 (95% CI: 0.36, 0.97). A high versus low intake of fluids other than water was associated with a relative risk of ...

Whither managed care on all this? Mike Taylor, a principal at Towers Perrin in Boston, thinks that managed care should be all over this issue. After all, he says, many of the objectives established by Healthy People 2000, such as immunizations and other types of preventive care, mesh well with what managed care is all about: establishing benchmarks and improving quality while containing costs.. While no one can cite concrete evidence of managed care's effect on Healthy People's results, there is consensus on this: Managed care probably will play an even larger role in helping the country meet Healthy People 2010.. "There are some key goals that should be met," says Taylor. "We also need to reassess where the bar needs to be placed." Healthy People 2000's goals were "grounded in the expectation of moderate managed care penetration," and that fee-for-service plans would still be around. "We've come a long way," he says, "and probably, we should set the bar higher.". Goals also should be ...

There are about 198 participants (170 in the UK) who are enrolled for examination of any or all of five categories of specimen (distribution types). These are detection of rubella antibody (128 UK participants), detection of hepatitis B surface antigen (130 UK participants), general virus serology (86 UK participants), virus identification (85 UK participants), and electron microscopy (56 UK participants). Specimens of a sixth category (rubella IgM antibody), not yet formally established, have also been distributed to 67 UK participants. Specimens in each distribution type are sent out once or twice a year, and, except for rubella IgM antibody, participants have been given a score of 2, 1, 0 or -1 marks for their reports on each specimen. Their cumulative scores and performance ratings are calculated retrospectively over a 12 month period for each distribution type separately and for ...
PurposeThe purpose of this study was to conduct the initial psychometric analyses of the Communicative Participation Item Bank-a new self-report instrument designed to measure the extent to which communication disorders interfere with communicative participation. This item bank is intended for community-dwelling adults across a range of communication disorders.. MethodA set of 141 candidate items was administered to 208 adults with spasmodic dysphonia. Participants rated the extent to which their condition interfered with participation in various speaking communication situations. Questionnaires were administered online or in a paper version per participant preference. Participants also completed the Voice Handicap Index (B. H. Jacobson et al., 1997) and a demographic questionnaire. Rasch analyses were conducted using Winsteps software (J. M. Linacre, 1991).. ResultsThe results show that items functioned better when the 5-category response format was recoded to a 4-category format. After ...

N2 - Background: Cross-sectional relationships between physical activity and health have been explored extensively, but less is known about how physical activity changes with time in older people. The aim of this study was to assess baseline predictors of how objectively measured physical activity changes with time in older people. Methods: Longitudinal cohort study using data from the Physical Activity Cohort Scotland. A sample of community-dwelling older people aged 65 and over were recruited in 2009-2011, then followed up 2-3 years later. Physical activity ...
A total of 2316 participants (mean age of 32.2 ± 10.7 years; 62.3% male; 37.7% female) were included (participation proportion 86.5%). Gender and age distributions imperfectly matched those of the national population, requiring adjustments for these to prevalence estimates. Observed 1-year prevalence of all headache was 77.2%, reducing to 65.8% when adjusted. For headache types, adjusted 1-year prevalences were migraine 25.0%, TTH 34.1%, pMOH 2.0% and other headache on ≥15 days/month 2.3%. Adjusted 1-day prevalence of any headache was 11.5%. Migraine and pMOH were associated with female gender (ORs: 1.7 and 4.7; p < 0.0001). Migraine was negatively associated with age > 45 years (OR: 0.4; p = 0.0143) while pMOH was most prevalent in those aged 46-55 years (OR: 2.7; p = 0.0415). TTH reportedly became more common with increasing level of education ...
A cross sectional descriptive study was done of 373 women who attended the antenatal clinic and welfare units of a primary health center in Ile-Ife. The objective of this study was to determine, among a sample of women attending a primary health center in Ile-Ife, the socio-demographic factors associated with intimate partner violence. Respondents were aged 18-37 years; the majority of them (73.8%) were aged 21-30 years (mean age was 24.9 ± 4.09). Three quarters (73.5%) were married in a monogamous setting and well over half (60.1%) were employed. The prevalence of intimate partner violence in the previous twelve months was 36.7%. Significant socio-demographic correlates of intimate partner violence were the age of the respondents (younger), marital status (single and separated), marriage type (polygamous), employment (being employed), level of education (secondary school education) and having children. Also, Respondent's and partner's use of alcohol were significantly associated with
Children with conduct disorders (CD) and their families are in contact with multiple agencies, but there is limited evidence on their patterns of service utilization. The aim of this study was to establish the patterns, barriers and correlates of service use by analysing the cohort of the 2004 Great Britain child mental health survey (N = 7,977). Use of social services was significantly higher by children with CD than emotional disorders (ED) in the absence of co-morbidity, while use of specialist child mental health and paediatric was significantly higher by children with hyperkinetic disorders (HD) than CD. Children who had comorbid physical disorders used more primary healthcare services compared to those without physical disorders. Utilization of specialist child mental heath and social services was significantly higher among children with unsocialized CD than socialized CD and oppositional defiant disorders. Services utilization and its correlates varied with the type of service. Overall, ...
Purpose: : Neither the etiology nor the natural history of age-related maculopathy (ARM) and age-related macular degeneration (ARMD) is clearly known. Epidemiologic studies suggest that the number of risk factors may be associated with ARM. Possibly associated with ARM are personal characteristics e.g. race, age, smoking, and cardiovascular diseases. In addition recent studies indicate that the density of macular pigment (MP) may play a central role in development and progression of ARM. In this study we investigate central MP density and the different pattern of MP distribution in ARM patients with normal versus with high blood cholesterol. Methods: : All Patients underwent MP density measurements (MPD) with a modified confocal scanning laser ophthalmoscope (HRA, Heidelberg Engineering, Germany) following a standard protocol. Inclusion criteria included age above 50 years, the presence of drusen in at least one eye, no evidence of advanced ARMD in the study eye, and no other concomitant eye ...
