PURPOSE OF THE STUDY
The authors present a group of patients treated for pelvic fractures in a period of 6 years and they evaluate radiographic findings and clinical outcomes following surgical management of type B and type C fractures.
MATERIAL AND METHODS
Between July 1998 and June 2004, a total of 271 patients with pelvic fractures, 162 men and 109 women (average age, 42 years; range, 15 to 93 years) were hospitalized at the authors departments. Of these, 141 patients were operated on (94 men, 47 women; average age, 37 years; range, 15 to 72 years) and 130 were treated conservatively (average age, 47 years; range, 15 to 93 years). The clinical outcome assessment in patients with type B and type C fractures treated surgically was based on the Majeed scoring system, and the radiographs were evaluated as described by Matta and Tornetta.
RESULTS

Well, I should tell you some more about me. On Apr. 23rd of this year, I received my diagnosis from my Urologist in Truro. He told me that I had aggressive Stage 4 Prostate Cancer that had metastasized to my lymph nodes and pelvic bone area ( a very painful area). I had many trips to the Truro Hospital within a couple of months prior to my diagnosis for many tests, a body scan and a biopsy of the prostate. Also, months before that I knew something was wrong, I just didnt know what. Finally, mid-Feb. I made an appointment to see my family doctor and like I said, I had many trips to Truro since then. I was in a lot of pain at the time in my right hip, upper leg, lower back and right pelvic bone area. I needed crutches at times to get around and then by mid-March I just basically stayed home as simple things like grocery shopping were difficult to do. When they checked me again (after all those hours of labor) I was only dilated to 8-9cm. Once the epidural kicked in, I was in a lot less pain! I was able to take a nap for about an hour or so, and then I read for a while. Finally, they checked again and this time I was finally at 10cm, so I was able to start pushing. Well, I pushed for 2 1/2 hours but baby Micah was not fitting through my pelvic bone. It was pretty exhausting and close to the end of that time my epidural had worn off. The doctor had been checking in on me the whole time, and by the end of the 2 1/2 hours of pushing, he checked again to see where the baby was, and he still wasnt coming through the pelvic bone. He also saw when he checked that there was some meconium in the uterus, which means the baby had pooped in my uterus, and this is very scary because it is poison that he could swallow. Recently, an increasing number of patients with traumatic hemipelvectomy are admitted to trauma centers alive due to improvements of the pre-hospital care. Successful management requires prompt recognition of the nature of this injury and meticulous surgical technique. We present our successful experiences on four cases of traumatic hemipelvectomy in the past nine years. Four cases with traumatic hemipelvectomy were admited to our hospital from June 21, 2002 to September 3, 2011. All injuries occurred due to vehicle accident and all patients were in a state of severe hypotension, with two of them having anal lacerations. These four cases were treated immediately with resuscitation, control of hemorrhage, early amputation, repeated debridement and closure of the wounds. Vascular damage in polytrauma patients is associated with high mortality and morbidity. Therefore, specific clinical implications of vascular damage with fractures in major trauma patients are reassessed. This comprehensive nine-year retrospective single center cohort study analyzed demography, laboratory, treatment and outcome data from 3689 patients, 64 patients with fracture-associated vascular injuries were identified and were compared to a control group. Results: Vascular damage occurred in 7% of patients with upper and lower limb and pelvic fractures admitted to the trauma room. Overall survival was 80% in pelvic fracture and 97% in extremity fracture patients and comparable to non-vascular trauma patients. Additional arterial damage required substantial fluid administration and was visible as significantly anemia and disturbed coagulation tests upon admission. Preoperative x-rays normally do not reveal sufficient information about the amount of osteolysis, but in the course of the operation, huge localized osteolytic lesions in the pelvic bone may appear. X-ray examinations often underestimate the extent of osteolysis, and CT-scans have not yet overcome the metal artifacts and high radiation doses to evolve into a useful tool in this matter. Wear-particles from polyethylene are suspected to play a role in the development of osteolysis. HA-coating of implants seems to reduce the risk by sealing off the bone-implant interface.
