Rising health care costs and increased cost sharing have resulted in significant medical expenses for many Americans. The goal of this study was to describe the prevalence of and risk factors for burdensome health care costs among non-elderly Americans. This was a cross sectional study of a nationally representative sample of non-elderly Americans. We used survey data previously collected by the Kaiser Family Foundation. We used logistic regression to identify key risk factors for burdensome health care costs and to assess whether risk factors differ according to age within our study population. For analyses comparing younger and middle-aged adults, we compared participants ages 18-39 (younger Americans) to those ages 40-64 (middle-aged Americans). Our study population included 5,493 participants. Twenty seven percent of participants reported difficulty paying medical bills, a prevalence that did not differ by age. Statistical Process Monitoring (SPM) is not typically used in traditional quality assurance of inpatient care. While SPM allows a rapid detection of performance deficits, SPM results strongly depend on characteristics of the evaluated process. When using SPM to monitor inpatient care, in particular the hospital risk profile, hospital volume and properties of each monitored performance indicator (e.g. baseline failure probability) influence the results and must be taken into account to ensure a fair process evaluation. Here we study the use of CUSUM charts constructed for a predefined false alarm probability within a single process, i.e. a given hospital and performance indicator. We furthermore assess different monitoring schemes based on the resulting CUSUM chart and their dependence on the process characteristics. Beginning with an historical reminiscence, this paper examines the peer review process as experienced by authors currently seeking publication of their research in a highly controversial area. A case study of research into the events of 9/11 (11 September 2001) illustrates some of the problems in peer review arising from undue influences based on financial and political considerations. The paper suggests that ethical failures, rather than flaws in the process itself, are mainly responsible for perceived problems. The way forward lies in improved ethics and a more open process. Peer review is considered the gold standard of scientific publishing. Trust in the traditional system of editor - blind-reviewer - author is still high, but its authority is in decline and alternative methods are on the rise. The current study investigates opinions of alternative peer review methods, the arguments for and against, and the reasons why academics are searching for new approaches. The opinions were analysed by applying qualitative content analysis to online discussions. The findings were interpreted using two different sociological theories: the Mertonian sociology of science and social constructivism. I understand that this piece isnt specific to VBACs, but wanted to share an important piece of information in case any women reading this might fall into the same category as me. I am part of a Facebook group called Special Scars, Special Women - we have all had cesareans that left us with not the typical scars on our uterus, meaning the low transverse. There are classicals, Ts, inverted Ts and Js and Im sure a few variations. What makes this matter is that typically we are COMPLETELY DENIED the option of a VBAC - and seen as a BIG uterine rupture risk. That said, Id say the MAJORITY of our ever-growing group has fought for or is fighting for VBACs. Its a *very* informed, educated decision - and the *truth* is that even in our numbers, the rupture rate is STILL LOW/VERY LOW! The struggle is very uphill for most of us - but we arm ourselves with the true numbers - and are working on getting accurate numbers/research done via our stories since were an organized group sharing these ...
