Following the very successful joint meeting in 1994 of the North American Society for Pediatric Gastroenterology and Nutrition (NASPGN) and the European Society of Pediatric Gastroenterology, Hepatology and Nutrition (ESPGHAN) in Houston, Texas, a feeling of growing unity among the international pediatric gastroenterology societies emerged. The concept of holding a World Congress of Pediatric Gastroenterology, Hepatology and Nutrition, at which these societies could hold a combined meeting, spawned simultaneously in North and South Americas, Australia, Asia, and Europe. The strategy of improving childrens health through training more and more pediatric clinicians can only be successful if two conditions are met. The first is that the new clinicians must selectively serve the populations in greater need. Unfortunately, the 24% growth in child health physicians per capita14 (pediatricians and 0.25 family practitioners per 100 000 age 0-17 years) in the past 10 years has not remedied the geographic disparities in distribution. There is a fivefold variation in the per capita supply of child health physicians (unpublished data from the Dartmouth Atlas Working Group that is available from authors). Regions with a notably high number of child health physicians per capita include San Francisco (162 per 100 000 age 0-17 years), Hackensack, New Jersey (144 per 100 000), and eastern Long Island (133 per 100 000). These rates are almost twice as high as Minneapolis (76 per 100 000) and Wichita (67 per 100 000), regions suggested as physician supply benchmarks.15-17 ...

Control of pain, other symptoms, and psychological, social, and spiritual problems is paramount. Multiple perspectives, obtained through a variety of research methods, are necessary to provide a greater understanding of the whole person. The goal of palliative care is to achieve the best quality of life for patients and their families. In pediatric palliative care, where systematic knowledge development is only in its infancy, such perspectives become even more critical. OBJECTIVES: The aim of this study was to provide an assessment of pediatric residency training in adolescent medicine. METHODS: We conducted 2 national surveys: 1 of pediatric residency program directors and the other of faculty who are responsible for the adolescent medicine block rotation for pediatric residents to elicit descriptive and qualitative information concerning the nature of residents ambulatory care training experience in adolescent medicine and the workforce issues that affect the experience. RESULTS: Required adolescent medicine topics that are well covered pertain to normal development, interviewing, and sexual issues. Those least well covered concern the effects of violence, motor vehicle safety, sports medicine, and chronic illness. Shortages of adolescent medicine specialists, addictions counselors, psychiatrists, and other health professionals who are knowledgeable about adolescents frequently limit pediatric residency training in adolescent medicine.
