Effective patient handoffs are an important factor for ensuring quality and patient safety. TeamHealth physicians Rohit Uppal, MD, and Dennis Deruelle, MD, spoke with Hospital Peer Review about how to ensure good handoffs between service lines.
Objective: To propose approaches to improve nursing handoffs for surgical patients, including standardization. Background: Handoffs, or the transfer of accountability and patient information, can generate potential risks for patient safety. Standardization has been proposed to help improve handoffs. Methods: After observing 333 nursing handoffs in the surgical wards of our institution, we conducted a thematic content analysis, comparing and contrasting the observations. Results: Handoff processes, including the use of support tools, varied among the observations. Common themes in the handoff content suggested possibilities of standardization. About half of the 51 interruptions occurring during the observed handoffs were by healthcare professionals. Conclusions: Standardization to improve handoffs should address both the content and the process. Interruptions were common and should be avoided whenever possible. Traditional handoff algorithms generally cannot keep both the average number of unnecessary handoffs and the handoff decision delay low. They do not exploit the relative constancy of path loss and shadow fading effects at any given location around a base station. This information can in fact be used to improve the efficiency of handoff algorithms, as we do in our new handoff algorithms using statistical pattern recognition. Over the last decade, increased attention has been focused on the transfer of care between clinical teams (handoffs) as a potentially modifiable factor in preventing adverse patient outcomes. Handoffs occurring in the perioperative setting may be particularly prone to error due to the need to exchange complex information and an environment that emphasizes efficiency and rapid turnovers. In such environments, use of a handoff protocol increases the duration of a handoff only slightly, while decreasing missed surgical information and improving nursing satisfaction.1 A 2012 meta-analysis of best practices for perioperative handoffs by Segall et al.2 identified more than 500 articles on this topic. PURPOSE: To systematically review and evaluate the effectiveness of interventions in order to improve the safety and efficiency of patient handover between intensive care unit (ICU) and general ward healthcare professionals at ICU discharge. METHODS: PubMed, CINAHL, PsycINFO, EMBASE, Web of Science, and the Cochrane Library were searched for intervention studies with the aim to improve clinical handover between ICU and general ward healthcare professionals that had been published up to and including June 2013. The methods for article inclusion and data analysis were pre-specified and aligned with recommendations outlined in the PRISMA guideline. Two reviewers independently extracted data (study purpose, setting, population, method of sampling, sample size, intervention characteristics, outcome, and implementation activities) and assessed the quality of the included studies. BACKGROUND: Ambulance paramedics play a critical role expediting patient access to emergency treatments. Standardised handover communication frameworks have led to improvements in accuracy and speed of information transfer but their impact upon time-critical scenarios is unclear. Patient outcomes might be improved by paramedics staying for a limited time after handover to assist with shared patient care. We aimed to categorize and synthesise data from studies describing development/extension of the ambulance-based paramedic role during and after handover for time-critical conditions (trauma, stroke and myocardial infarction). METHODS: We conducted an electronic search of published literature (Jan 1990 to Sep 2016) by applying a structured strategy to eight bibliographic databases. Two reviewers independently assessed eligible studies of paramedics, emergency medical (or ambulance) technicians that reported on the development, evaluation or implementation of (i) generic or specific structured handovers
The National Clinical Handover Initiative Pilot Program involved 56 hospitals in six jurisdictions, including primary and aged care services. The Pilot Program was externally evaluated in 2010. The evaluation found that the Pilot had significantly increased the evidence base surrounding clinical handover.
