Aim: Asthma self-management is widely recommended by guidelines but poorly implemented. We undertook a systematic review of phase IV implementation studies of asthma self-management support interventions to inform implementation in real-life settings.. Methods: We searched 7 electronic databases, and performed snowball and manual searches. We assessed all eligible papers for quality, extracted and synthesised data on process and clinical outcomes (e.g. asthma symptoms, use of health services).. Results: 14 studies were included in the review. Complex whole systems interventions which addressed patient education, professional training and facilitated a supportive organisation were associated with improved asthma control. Individually, however, these core components were insufficient to reliably improve clinical outcomes. Key facilitators were commitment of the healthcare system to supporting self-management, self-management skills training for professionals, patient education programmes supported ...
Results:. Physicians whose patients had a lower mean level of education had significantly better ranks for patient physical and mental health status after adjustment for patients level of education level than they did before adjustment (P , 0.001); this result was not seen for patient satisfaction. After adjustment for patients level of education, each 1-year decrease in mean educational level was associated with a rank that improved by 8.1 (95% CI, 6.6 to 9.6) for patient physical health status and by 4.9 (CI, 3.9 to 5.9) for patient mental health status. Adjustment for education had similar effects for practices with more educated patients and those with less educated patients. ...

Introduction: According to the World Health Organization, patient education mainly deals with chronic diseases and conditions which require self care. We know that this practice is still emerging and it is therefore interesting to observe the amount of international research produced in this field during the last years. Objective: We have analysed the quantitative evolution of scientific papers devoted to patient education (PE) over a 10 years period (1999-2009). Methods: Fourty-one chronic diseases and conditions in 11 disciplines have been selected as being the most concerned with patient education (PE). The identification on Pubmed of the studies dealing with these diseases, published from 1999 to 2009, has been undertaken considering the total number of articles mentioning PE activities, the distribution and the evolution in time of PE studies in those disciplines and chronic diseases, the types of publications (reviews, randomized control trials, meta-analyses) on PE. Results: We have ...

The primary purpose of this study is to compare the effects of a multidisciplinary outpatient clinic, including a brief group-based educational programme, with a traditional individual outpatient clinic for patients with hip, knee, hand or generalized OA. A secondary purpose is to investigate the effects of a telephone follow-up call. This is a pragmatic randomised single-blind controlled study with a total of 400 patients with hip, knee, hand or generalized OA between 40 and 80 years referred to an outpatient rheumatology hospital clinic. The randomisation is stratified according to the diagnostic subgroups. The experimental group is exposed to a multidisciplinary and multifaceted intervention, including a 3.5 hour group-based patient education programme about OA in addition to individual consultations with members of a multidisciplinary team. Background: The mainstay of contemporary management of type 1 diabetes mellitus (T1DM) is physiological insulin replacement. Regimens based on multiple daily injections (MDI) of insulin improve glycemic control and increase the frequency of hypoglycemia. Insulin pump therapy also improves glycemic control, and reduces the within-day and between-day glycemic variability that is seen with insulin injections. Objectives: We aimed to evaluate out first experience with insulin pump therapy by 1) assessing the glycemic control on insulin pump therapy compared to MDI based therapy and 2) to learn the attitude of patients and families towards the use of insulin pump. Patients and Methods: A prospective observational study involving 37 patients who used insulin pump between March & November 2013. Patients were selected according to certain criteria set up by treating physician. Data collected included demographic, clinical and biochemical data before, during and after insulin pump therapy use. Severe ...

The results of this systematic review provide reason for optimism that interventions for socially disadvantaged populations can be effective and have the potential to reduce health disparities in diabetes care and outcomes. They also have practice and policy implications. The results indicate that short-term group-based didactic teaching aimed at improving diabetes knowledge may be of limited value for disadvantaged populations. To be successful, interventions appear to require organizational inputs and resources that go well beyond traditional diabetes education programs. The time and effort required to adapt interventions to the local community is likely to yield important benefits but must be done in each setting to fit local circumstances. The key aspects of tailoring in the reviewed studies were for culture and health literacy, both of which require substantial expertise and knowledge of local communities. Achieving a high degree of responsiveness to individual needs and making extensive ...

BACKGROUND: Radiotherapy (RT) has become an important treatment modality in pediatric oncology, but its delivery to young children with cancer is challenging and general anesthesia is often needed. METHODS: To evaluate whether a psychoeducational intervention might reduce the need for anesthesia, 223 consecutive pediatric cancer patients receiving 4141 RT fractions during 244 RT courses between February 1989 and January 2006 were studied. Whereas in 154 RT courses corresponding with 2580 RT fractions patients received no psychoeducational intervention (group A), 90 RT courses respectively 1561 RT fractions were accomplished by using psychoeducational intervention (group B). This tailored psychoeducational intervention in group B included a play program and interactive support by a trained nurse according to age to get familiar with staff, equipment and procedure of radiotherapy. RESULTS: Group A did not differ significantly from group B in age at RT, gender, diagnosis, localization of RT and ...
Background: Partners of patients with heart failure provide both practical and emotional support. Many partners assume caregiving responsibilities without being aware of the burden related with this role. Objective: Our work has established that a psycho-educational intervention has benefits at 3, but not at 12 months for patients with heart failure. Further we had not described the long-term effects in caregivers. This study aimed to determine the 24-months effects of a dyadic psycho-educational intervention on caregiver burden and morbidity in partners of patients with heart failure and study factors associated with a change in caregiver burden. Design: A randomized controlled study design, with a follow-up assessment after 24 months. Setting and participants: Partners to patients with heart failure were recruited from two hospitals in the southeast of Sweden. Intervention: A three session nurse-led psycho-educational program was tested and included psychosocial support to maintain the ...
OBJECTIVE: Dislocation is a well-known complication after total hip arthroplasty (THA), and is the second-highest cause of revision surgery. Our objective was to assess the effect of preoperative patient education on the occurrence of hip dislocation within 6 months after primary THA. METHODS: Between 1998 and 2007, we conducted a prospective cohort study at the Geneva University Hospital Department of Orthopaedic Surgery, including all primary THAs performed via an anterolateral transgluteal approach with the use of a 28-mm diameter head. The preoperative education session was introduced in June 2002 and included advice on muscle strengthening exercises and postoperative restrictions of range of motion as a means of preventing dislocation. The main outcome was the incidence of dislocation within 6 months of surgery. RESULTS: A total of 597 patients who underwent 656 THAs between June 2002 and June 2007 participated in the education session, whereas 1,641 patients who underwent 1,945 procedures did not.
