The Azzopardi phenomenon, or Azzopardi effect, is the presence of DNA in necrotic venules. It can occur in small cell carcinomas and in some high-grade malignant neoplasms. Artifacts may result from improper handling and staining of smears. Avoid contact of smears and stains with water! Exposure of smears to formalin vapors causes a bluish background, making it more difficult to differentiate cells. Dont store formalin in the same area where staining is done. Also, dont package surgical pathology specimens with blood smears. Never store smears in a refrigerator or freezer, since water condensation, RBC lysis, and hemoglobin crystallization may occur. First, the tumor is more pleomorphic without prominent nucleoli, whereas typical high grade prostate cancer has a more uniform appearance where each nucleus is enlarged with prominent nucleoli yet not significant variation in size and shape. Second, there is extensive necrosis, which also differs from usual high grade prostate cancer.
