N2 - As a result of the common usage of albumin in total parenteral nutrition (TPN) solutions at The Ohio State University Hospitals, a review of the use of this product was performed. Albumin therapy in 18 patients was evaluated using prospective criteria. Therapy was considered appropriate if the serum albumin level was less than or equal to 3.0 g%. Using these criteria, 12 of 18 patients (67%) received albumin in the TPN solution. Of these 12 patients, 7 received albumin unnecessarily. During the 3-week review, a total of 168 vials of 50 ml, 25% albumin (2100 g) were administered to patients with serum albumin concentrations greater than 3.0 g/100 ml at a cost of 6,014. Because several studies have shown a significant inverse correlation between depressed serum concentrations of albumin and hospital morbidity, a study with central total parenteral nutrition (TPN) with normal serum albumin (NSA) in hypoalbuminemic patients was conducted. Sixty-one patients who required central TPN were randomized into one of two groups: group 1 (n = 31) received TPN plus NSA (25 to 37.5 g/day) until their measured serum albumin was greater than 3 g/dl, and group 2 (n = 30), who received TPN alone. Jamie was 1st diagnosed with Stage 4 Neuroblastoma at the age of 3, in August 2003. This was after a spell of 5 weeks of fevers, sickness, tummy ache, weightloss, general not being right, on and off. We visited doctors several times during these 5 weeks, to be told that Jamie had the tail end of a virus. Little did we know then, that Jamie had a large tumour growing from his left adrenal gland, squashing his kidney, wrapped around his aorta and peeking into his chest cavity!!!. Jamie received 12 rounds of Rapid COJEC Chemo, every 10 days, which lasted 2 or 3 days each, followed by surgery to remove his tumour on Nov 5th 2003. It took Jamie only 7 days to recover from surgery; the first night was a scary one in ICU for precautions. Noah has been on TPN (total paranteral nutrition) for the past 2 1/2 weeks (he is being fed directly into his veins through a central line). It has done absolute wonders for Noah. He has put on an amazing amount of weight (8lbs 10oz) and his energy is unbelievable. He really is a different baby! Unfortunately, we cant do TPN much longer as his liver functions are rapidly climbing (a warning sign that we could end up damaging his liver). So it seems that we are back to where we started. We decided to give jejunum feeding one more try, so yesterday the gj tube was reinserted, along with another tube to try to keep the bile drained off his stomach. The hope is that Noah will have nothing in his stomach to reflux. If this doesnt work, we will probabl have to go back to g-tube feeding and just take Noah home. This will mean taking our chances with aspirations, which is certainly not a great situation, but it may be the only option we have left ...
Today is our 10th day. Im at the point of the stay where Im just done and I want out. Im so tired of being here and so depressed by the lack of progress. Total parenteral nutrition with parenteral lipid emulsion is being used with increased frequency in malnourished pregnant women. Placentas examined in 20 reported cases of maternal hyperalimentation with lipid emulsions have been described as normal. We report a case of a 31-year-old pregnant woman who received total parenteral nutrition with daily lipid emulsions for 8 weeks. Intrauterine fetal death was diagnosed at 22 weeks gestation. The tan-yellow, pale placenta showed vacuolated syncytial cells and Hofbauer cells, which stained for fat by the oil red O stain, in the chorionic villi. To our knowledge, this is the first reported case of fat deposits in the placenta associated with intravenous lipid emulsions.

Millions of intravascular catheters are purchased each year by hospitals and clinics, placing many patients at risk for serious catheter-related infections. This chapter summarizes the rapidly expanding body of literature concerning prevention of intravascular catheter-related infections in the hope of reducing the risk posed to present and future patients. Prophylaxis with vancomycin or teicoplanin during central venous catheter insertion does not reduce the incidence of catheter-related bloodstream infection. The addition of prophylactic vancomycin to flush solutions or total parenteral nutrition solutions significantly reduces the incidence of catheter-related bloodstream infection; however, use of systemic antimicrobial agents to prevent intravascular catheter-related infections is not recommended. Administering very low doses of warfarin reduces thrombosis due to long-term central venous catheters. Candidemia is an important cause of morbidity and mortality in the healthcare setting. However, there is limited information about risk factors for such infection among elderly patients. A case-control study was conducted during the period 2008-2011. For each case, two controls were selected among patients admitted to the same hospital, and individually matched by sex, age, time of admission, hospital ward and hospitalisation duration. The adjusted odds ratio (OR) was calculated using multiple conditional logistic regression. We identified 145 episodes of candidemia occurring in 140 patients with a median age of 80 years. Candida albicans caused 55% of all candidemia episodes. In the past decade intestinal transplantation has evolved to a viable option in the treatment of short bowel syndrome, especially for patients who have developed life-threatening complications attributable to their intestinal failure and/or long-term total parental nutrition therapy.1 Technical improvements, novel immunosuppressive agents and increased clinical experience have contributed to an improvement in intestineal transplant graft and patient survival. One-year graft and patient survival are nowadays both estimated at 80%.2 3.
