Large plaque parapsoriasis (also known as parapsoriasis en plaques) are skin lesions that may be included in the modern scheme of cutaneous conditions described as parapsoriasis. These lesions, called plaques, may be irregularly round-shaped to oval and are 10 cm (4 in) or larger in diameter. They can be very thin plaques that are asymptomatic or mildly pruritic. Large-plaque parapsoriasis is a common associate of retiform parapsoriasis, can be accompanied by poikiloderma vasculare atrophicans, and can in rare occasions be a precursor to cutaneous T-cell lymphoma. Parapsoriasis treatment consists primarily of light therapy (more specifically PUVA therapy or UVB therapy) possibly in combination with topical steroids. Large plaque parapsoriasis is usually a chronic condition that needs long-term treatment. Parapsoriasis refers to one of a group of skin disorders that are characterized primarily by their resemblance to psoriasis (red, scaly lesions), rather than by their underlying cause. Neoplasms can develop from parapsoriasis. For example, it can develop into cutaneous T-cell lymphoma. The word parapsoriasis was formed in 1902. The parapsoriasis groups, described and debated for nearly a century, has spawned a confusing nomenclature. There are some authors who prefer to limit the term parapsoriasis to large- and small-plaque variants only. However, the following classification scheme is now generally accepted: Large-plaque parapsoriasis Small-plaque parapsoriasis Pityriasis lichenoides Pityriasis lichenoides chronica Pityriasis lichenoides et varioliformis acuta Lymphomatoid papulosis Poikiloderma vasculare atrophicans List of cutaneous conditions Sehgal VN, Srivastava G, Aggarwal AK (2007). Poikiloderma vasculare atrophicans, or PVA, indicates that extra or altered skin pigmentation (poikiloderma)[10] is occurring, associated with heightened visibity of capillaries (vasculare, referring to telangiectasia) under the skin, related to thinning and wasting away (atrophicans) of the skin and its tissue. Telangiectasia is an enlargement of capillaries underneath the skin.[10] PVA also has common names that include parapsoriasis-related terminology (i.e. parapsoriasis variagata, or variegated parapsoriasis).[5] Parapsoriasis is a term first used by Brocq in 1902,[11] intended to represent a group comprising a number of uncommon skin disorders, under a once used, now antiquated classification scheme for all inflammatory dermatoses (skin diseases known to be associated with or cause inflammation).[5] Brocq chose the term parapsoriasis to illustrate that the dermatoses placed in this group had or would have commonalities with psoriasiasis, including appearance and chronicity ...
Cytokines such as IL-2 and IL-4 due to their increased presence in patients with mycosis fungoides and Sezary syndrome.7. There are three stages of mycosis fungoides and therefore clinical presentation will vary depending on the stage of disease:. Patch stage: Erythematous, or brownish scaly patch, which may show some atrophy. It is possible to have one or multiple lesions develop in areas such as the gluteal region or on the proximal thighs. The likeness of this stage has been compared to small-plaque or large plaque parapsoriasis; however, the plaques are actually not plaques but patches instead.10. Plaque stage: This is the second stage - lesions will be larger, more numerous and will show infiltration. The lesions appear annular, are raised and have well-defined edges as well as asymmetry in terms of their distribution. Face and scalp involvement can also be seen starting at this stage.11. Tumor stage: The final stage - erythematous-purplish papules or nodules of larger ...

The aim of this study was to improve the level of diagnosis and differential diagnosis of lymphomatoid papulosis (LyP). Two cases of type B LyP were identified and the literature was reviewed to summarize the clinical outcomes and pathology of LyP and its treatment. The two patients exhibited symptoms with papulonodular lesions, the centers of which gradually underwent ulceration and necrosis. CD30, a helper T-cell marker specifically expressed in tumor cells was analyzed by immunohistochemical staining and the result showed that CD30-negative or only scattered CD30-positive cells were present. Therefore, a diagnosis of type B LyP was made. A fairly good curative effect was achieved following treatment with retinoic acid, glucocorticoids and immunomodulatory drugs. LyP is a type of low-level malignant lymphoma and is easily misdiagnosed as pityriasis lichenoides et varioliformis acuta and other diseases. In order to avoid under diagnosis and misdiagnosis, doctors should evaluate suspected ...
A 38-year old Japanese woman visited our dermatological department on June 24, 2013, with a three-day history of erythematous lesions on the trunk and extremities. She had been treated orally with 50 mg of diaminodiphenyl sulfone (dapson) daily for pityriasis lichenoides et varioliformis acuta (PLEVA) for 38 days. Physical examination on admission revealed fever (38.4℃), bilateral cervical lymphadenopathy, facial edema with erythema, and round erythematous and purpuric plaques on the trunk and extremities. Erythematous targetoid lesions were also found. Blood examination revealed an increased white blood cell count (13.4 ~ 109/l: normal 3.5-9 ~ 109/l), and mild liver dysfunction (alanine aminotransferase; 40 IU/l :normal 5-25 IU/l). A skin biopsy obtained from the patients abdomen revealed acanthosis, hydropic and vacuolar degeneration of epidermal basal cells and upper dermal infiltration consisting of lymphocytes and neutrophils. No extensive epidermal necrosis was found. A lymphocyte ...

NIH Rare Diseases : 50 pityriasis lichenoides (pl) is a skin condition characterized by small, raised pink spots that tend to come together in groups. it is not contagious. there are two main types of pl: an acute form called pityriasis lichenoides et varioliformis acuta (pleva), and a milder, longer-lasting form called pityriasis lichenoides chronica (plc). there is also a rare, severe variant of pleva called febrile ulceronecrotic pleva, associated with high fever and complications that may affect other body systems. in both types of pl, spots usually occur on the trunk, buttox, arms and legs. pleva begins abruptly and may cause itching or burning. plc may develop over days, is less irritating, and lasts longer than pleva. spots associated with plc typically fade within 3 to 4 weeks, but new spots may then appear. there is no clear consensus regarding how long either form of pl lasts, but most cases resolve on their own within one to several months. some cases may wax and wane, or relapse over ...
It took three months and three missed diagnoses from the moment my symptoms first appeared to the point when my mysterious red spots were biopsied and conclusively identified as pityriasis lichenoides et varioliformis acuta (PLEVA). Take a read below: Does any of this sound familiar?. I remember the exact day that my journey with pityriasis lichenoides began. In mid-December of 2012, I woke up, went to the bathroom to brush my teeth and noticed that I had broken out in a strange rash, apparently overnight - a few dozen red bumps dotted my lower torso, clustering around my navel and extending around my torso to my flanks. This is how youd describe the event in more precise medical parlance: PLC and PLEVA usually present with the sudden appearance of multiple red papules on the trunk, buttocks, and proximal extremities. It can, however, occur anywhere on the body.. I didnt feel itchy or otherwise unwell, so as a relatively healthy guy in my mid-twenties, I chalked it up to some kind of ...

Hi, I am a 23 yr old female that over the past month developed odd bumps on the tops of my legs. I went to see my doctor who said, its probably just viral but, let me know if its still there in a week. Of course, it got worse just over that short period of time. My doctor sent me to see a dermatologist who decided to do a biopsy. The biopsy revealed that I had pityriasis lichenoides. My doctor has tried me on every cream, and oral medication you could think of but, these bumps have now spread to the bottoms of my legs, stomache, and all over my arms. I hear that this is a rare disease and I am having problems finding out much information about it. Can you give me any? What caused this to happen to me ...

First, lets define what mycosis fungoides is. It is a type of blood cancer. What it does is it affects the skin of the person primarily, and then over time, it will slowly progress internally. Common symptoms include but not limited to itchy skin, skin lesions, tumors, and rash.. The cause of this type of cancer until now is still unclear. Most of the case, it is not hereditary also it usually affects people who are at the age of 20 and more. Men are the ones who get this more compared to women. Various treatments include ultraviolet light, exposure to the sunlight, chemotherapy, topical corticosteroids, as well as radiotherapy.. Lets discuss in detail about its symptoms that are visible. One thing is for sure; it does affect the skin. So, there will be lesions, rashes, tumors, and even itching is also common. The symptoms do not appear right away. They do pop up progressively. Thus, expect during the early stages of mycosis fungoides, there will be the appearance of lesions of which will be ...

Sézary syndrome (SS) and Mycosis Fungoides (MF) are T-cell lymphomas whose primary manifestation is in the skin. Mycosis Fungoides, is the most common form of cutaneous T-cell lymphoma. It generally affects the skin, but may progress internally over time. The name mycosis fungoides is somewhat misleading--it loosely means mushroom-like fungal disease. The disease, however, is not fungal infection but rather a type of non-Hodgkins lymphoma. It was so named because the skin tumors of a severe case as having a mushroom-like appearance.. None! Mycosis fungoides is the common name for the most common type of cutaneous T-cell lymphoma. A lymphoma is a malignancy of the white blood cells that are known as lymphocytes. Mycosis fungoides primary involves the skin and produces patches, plaques and tumors. The patches may be scaly or flaky and can thus look a lot like an ordinary rash such as the rash of proriasis or of a non-specific dermatitis. Many times, the plaque-like rash will look like a fungal dermatitis, and thus the name for the syndrome. However, there really is not a fungus involved.. ...

17 NCCN Guidelines for Patients ® Mycosis Fungoides, Version 1.2016 2 Treatment planning Physical exam Physical exam Doctors should perform a physical exam along with taking a medical history. A physical exam is a study of your body for signs of disease. During this exam, your doctor will listen to your lungs, heart, and gut. Parts of your body will likely be felt to see if organs are of normal size, are soft or hard, or cause pain when touched. For mycosis fungoides, a skin exam of your total body is needed. While mycosis fungoides is often confined to the torso, this exam includes areas like your scalp, between your legs, and toe webs. Your doctor will note the type of skin lesions and assess how much of your skin has lesions. He or she will also assess if any lymph nodes or other organs are enlarged. A piece of cholesterol, or plaque, may be seen in the arteries of the retina after dilating your eyes. If there is a small plaque in the retinal vessels of the back of the eye, it is likely that there are larger plaques in other places of the body. Large plaques increase your risk for heart attack and stroke. This is one more reason why we encourage patients to have their eyes dilated at their annual eye exams. With both eyes dilated, Dr. Nate and I can look closely at those vessels to rule-out these kinds of concerns.. If Dr. Nate or I find any of these signs, especially if you are under 60 years of age, we may ask you more questions about your lipid levels or even recommend that you get blood tests done with your primary care doctor.. Dont delay your annual eye exam! Getting your eyes checked should be included in your routine appointment days.. -Dr. Beth. ...
