Abstract Bancroftian filariasis has been reported in several areas of Papua New Guinea. The epidemiologic features and natural history of Wuchereria bancrofti infection in this geographic region, however, have not been well-defined. The objective of this study was to assess the parasitological and clinical features of bancroftian filariasis in a community in East Sepik Province, Papua New Guinea. In a village of 99 individuals, the overall prevalence of microfilaremia was 68%. The microfilarial carrier rate was high in those ≤ 10 years (62%), remained elevated in the 11-20, 21-30, and 31-40 age groups (42-55%), and peaked in subjects ≥41 years old (90%). The geometric mean level of parasitemia in all subjects with patent infection was 3,198 microfilariae/ml blood. This value was 78 parasites/ml in the ≤10-year-old age group, increased to 1,753 in 21 to 30-year-olds and was markedly elevated in subjects ≥41 years old (6, 792 microfilariae/ml). It has been estimated that there are more than 60 million Hepatitis C virus (HCV) carriers in the World Health Organisations Western Pacific region (WHO-WPR), where liver cancer is among the top three causes of cancer death. WHO and the US Centres for Disease Control and Prevention report the prevalence of HCV in the South Pacific islands (countries within the WHO-WPR) to be high (5-10% and |2% respectively). However, since HCV is not tested for in many of these countries, there is sparse data available to support this assertion. We screened ∼2000 apparently healthy individuals from Papua New Guinea, Fiji and Kiribati and found a sero-prevalence of 2.0%, 0.1% and 0%, respectively. All sero-positive samples tested negative for HCV RNA. During a prospective study of red cell variants and severe malaria in children, a surprising observation was the occurrence of dark urine. Children were grouped according to urine findings: 22 had dark urine that contained a haem protein (Group I), 93 had urine of normal colour that contained a haem protein (Group II) and 236 had normal urine (Group III). To investigate the cause of dark urine, haemolysis and muscle cell injury were assessed. Intravascular haemolysis was greater in Group I than in Groups II and III. However, anaemia was more severe in Group III and is likely to have resulted mainly from extravascular haemolysis. Serum levels of reactive nitrogen intermediates (RNI; nitrate plus nitrite) were measured in 92 patients with cerebral malaria in the Madang Province of Papua New Guinea. RNI levels were compared to disease severity and clinical outcome, and correlated with both the depth of coma on admission and its duration. Median levels were higher among children with deeper coma than among those with lighter coma (35.6 microM vs. 16.7 microM; P = 0.008) and also among children with longer duration of coma (72 h; 59.3 microM vs. 19.3 microM; P = 0.004). RNI levels also correlated with clinical outcome, fatal cases having significantly higher RNI levels than survivors (41.2 microM vs. 18.5 microM; P = 0.014). Thus, high RNI levels are associated with indices of disease severity and may predict outcome in children with cerebral malaria. These data are consistent with the hypothesis that nitric oxide is involved in the pathogenesis of coma in human cerebral malaria.

New Guinea is one of five high biodiversity wilderness areas, and frog diversity is exceptionally large, with more than 400 species described to date. The microhylid frog genus Mantophryne is endemic to New Guinea and consists of four species, three of which have narrow geographic distributions and a fourth, M. lateralis, with a broad range that spans the eastern half of the island. Here, we sequence 104 Mantophryne samples for three mitochondrial and three nuclear loci to reconstruct the first phylogeny of the genus and to examine spatial patterns of diversity within M. lateralis. Results indicate that the wide-ranging M. lateralis is composed of at least nine geographically separated and well-supported lineages that represent putative species. In this study, we use bivariate Poisson lognormal models (BPLM) to estimate covariate-adjusted associations between the incidence of infections (as measured by the force of blood-stage infections, molFOI) and clinical episodes caused by both P. falciparum and P. vivax in a cohort of Papua New Guinean children. The dartfish Ptereleotris brachyptera is described as a new species from eight specimens from Ngargol Island, Palau, and one from Majuro Atoll, Marshall Islands, from lagoons on silty sand and rubble substrata in 20 to 38 m depth. The type specimens, 39.1 to 58.2 mm SL, were first believed to be subadults of P. microlepis or P. hanae until mature fish were found in the series. Ptereleotris brachyptera differs from P. microlepis in having fewer dorsal and anal rays, a more slender body, and a red instead of black bar at the pectoral-fin base. We develop and use DNA-based tools to identify the malaria transmitting Anopheles species in the region in order to study their biology, ecology and potential to adapt to mosquito control strategies.
