A nationwide retrospective study of exogenous lipid pneumonia (ELP) was carried out to update the data on this disease, with emphasis on thoracic computed tomography (CT) scan and bronchoalveolar lavage (BAL) findings. The inclusion criteria were: 1) presence of abnormal imaging features compatible with the diagnosis of ELP; 2) presence of intrapulmonary lipids; and 3) exogenous origin of the lipid pneumonia. Forty four cases were included (20 males and 24 females; mean age 62 +/- 11 yrs), of which four were occupational (chronic inhalation of cutting mist or oily vapour in an industrial environment). Thirty of the 40 nonoccupational cases were related to aspiration of liquid paraffin used for the treatment of constipation. Quantitation was achieved by LC-MS using a 3-µm C-18 column coupled to an electrospray ionization source on a linear ion-trap mass spectrometer. α-Cobratoxin (CTX) is a large peptide (71 amino acids) with strong analgesic effect and may be misused in sports such as horse racing. To prevent such misuse, a sensitive method is required for detection and confirmation of the toxin in equine samples. CTX was extracted from equine plasma using an optimized mixed-mode solid-phase extraction (SPE) procedure. Extracted CTX was reduced with dithiothreitol and alkylated with iodoacetamide, and then was digested by trypsin at 56 C for 30min. The digest was analysed by liquid chromatography coupled to tandem mass spectrometry (LC-MS/MS), and tryptic peptides T2 ( CFITPDITSK ) and T4 ( TWCDAFCSIR ) were monitored for detection and confirmation of CTX. The limit of detection (LOD) was 0.05ng/mL for CTX in plasma, and the limit of confirmation (LOC) 0.2ng/mL. Non-freezing cold injury develops after sustained exposure to cold temperatures, resulting in tissue cooling but not freezing. This can result in persistent sensory disturbance of the hands and feet including numbness, paraesthesia and chronic pain. Both vascular and neurological aetiologies of this pain have been suggested but remain unproven. We prospectively approached patients referred for clinical assessment of chronic pain following non-freezing cold injury between 12 February 2014 and 30 November 2016. Of 47 patients approached, 42 consented to undergo detailed neurological evaluations including: questionnaires to detail pain location and characteristics, structured neurological examination, quantitative sensory testing, nerve conduction studies and skin biopsy for intraepidermal nerve fibre assessment. Determination of fatty acid acquisition routes in an isogenic model with either Akt or Ras pathway activation demonstrated that Akt induces de novo fatty acid synthesis, whereas Ras decreases it. This was most evident for the mono-unsaturated fatty acid oleate (C18:1), of which 96% was produced de novo in Akt-driven cells but only 57% in Ras-driven cells. We confirmed that Akt pathway activation leads to elevated levels of stearoyl-CoA desaturase 1 (SCD1), a key enzyme in the synthesis of oleate, and that Ras pathway activation leads to increased uptake of exogenous lipids. We hypothesized that SCD1 inhibition would therefore be particularly toxic to the Akt-driven cells. We confirmed that Akt activated cells were significantly more sensitive to SCD1 inhibition than those with Ras activation.
