Pulse oximetry is an accurate screening test for critical congenital heart defects in newborns. Pulse oximetry is simple to use, widely available, and has moderate sensitivity (76.3%) and good specificity (99.9%). However, the prevalence of critical congenital heart defects is low, and most newborns who screen positive do not have a critical congenital heart defect.

Background: Information from pulse oximeter waves confirms the presence of a pulse and helps obtain waves from tissue when the supplying artery is not readily accessible. Objectives: This study determined the predictive value of pulse oximeters for detecting improved arterial pulses after angiography. Patients and Methods: This cross-sectional, multi-center study included 467 4-day-old to 12-year-old patients and was conducted from January 2012 to January 2016. Angiographies were performed on 12-year-old or younger children for various medical reasons using venous, arterial, or both types of paths. The posterior malleolar or dorsalis pedis were palpated in punctured lower extremities. In the absence of a pulse, pulse oximetry was performed to identify pulse curves at 1 hour, 6 hours, and 12 hours after each angiography. Results: Pulse oximetry displayed the pulses of 319 patients immediately following each angiography. Of these, 262 patients had palpable pulses at 6 hours after angiography (P , ...
Pulse oximetry probes placed peripherally may fail to give accurate values of blood oxygen saturation when the peripheral circulation is poor. Because central blood flow may be preferentially preserved, we investigated the oesophagus as an alternative monitoring site. A reflectance blood oxygen saturation probe was developed and evaluated in 49 patients undergoing cardiothoracic surgery. The oesophageal pulse oximeter results were in good agreement with oxygen saturation measurements obtained by a blood gas analyser, a CO-oximeter and a commercial finger pulse oximeter. The median (IQR [range]) difference between the oesophageal oxygen saturation results and those from blood gas analysis were 0.00 (-0.30 to 0.30 [-4.47 to 2.60]), and between the oesophageal oxygen saturation results and those from CO-oximetry were 0.75 (0.30 to 1.20 [-1.80 to 1.80]). Bland-Altman analysis showed that the bias and the limits of agreement between the oesophageal and finger pulse oximeters were -0.3% and -3.3 to ...
The Innovo Premium CPE Fingertip pulse oximeter shows your blood oxygen saturation levels (SpO2) and Pulse Rate on a OLED screen (with six multidirectional display choices). The product comes with a Plethysmograph and Perfusion Index (PI). Each wave corresponds to a heartbeat and the amplitude to the amount of blood detected in your blood vessels. When the height of the wave amplitude no longer fluctuates, this means that the pulse oximeter is now optimized and you may now take the readings. Meanwhile, the Perfusion Index informs you of the strength of your pulse. If the PI value is less than 0.2%, this means that your hands are too cold or improperly positioned in the pulse oximeter. Simply warm your finger and reposition it so that it is right under the LED lights. Comes with a lanyard and 2 AAA batteries so that you can use the pulse oximeter right out of the box. Please note that this is not a medical device. Intended for home use. The oximeter might not work on kids or adults with small ...

Retinal oximetry is a non-invasive technique to investigate the hemodynamics, vasculature and health of the eye. Current techniques for retinal oximetry have been plagued by quantitatively inconsistent measurements and this has greatly limited their adoption in clinical environments. To become clinically relevant oximetry measurements must become reliable and reproducible across studies and locations. To this end, we have developed a convolutional neural network algorithm for multi-wavelength oximetry, showing a greatly improved calculation performance in comparison to previously reported techniques. The algorithm is calibration free, performs sensing of the four main hemoglobin conformations with no prior knowledge of their characteristic absorption spectra and, due to the convolution-based calculation, is invariable to spectral shifting. We show, herein, the dramatic performance improvements in using this algorithm to deduce effective oxygenation (SO2), as well as the added functionality to
BACKGROUND--The British Thoracic Society report on the diagnosis and treatment of the sleep apnoea/hypopnoea syndrome (SAHS) suggests that, if the pulse oximetry baseline oxygen saturation is above 90%, then 15 4% oxygen desaturation/hour in bed will diagnose SAHS requiring treatment. The diagnostic outcome of applying these guidelines has been studied. METHODS--One hundred patients referred to a district general hospital sleep clinic were recruited. After initial clinical assessment, overnight pulse oximetry measurements were performed, followed by full polysomnography at the regional laboratory. RESULTS--Sixty nine patients underwent both pulse oximetry and polysomnography. All 10 patients with more than 15 4% desaturations/hour on pulse oximetry had SAHS confirmed on polysomnography (specificity = 100%). Twenty two patients with SAHS were misdiagnosed using pulse oximetry alone (sensitivity = 31%). These patients had low apnoea scores but high hypopnoea scores. CONCLUSIONS--The BTS pulse ...

Congenital heart disease is a relatively common disease seen in 8/1,000 live births. Many of these diseases are associated with hypoxemia. Hypoxemia results from the mixing of pulmonary and systemic blood or from the mixing of parallel circulations as seen in transposition of the great vessels. Pulse oximetry can be used to measure hypoxemia. Pulse oximetry estimates oxygen saturation by measuring absorption of light in human tissue beds. The amount of light absorption varies depending on the amount of deoxygenated blood in the tissue measured. Measuring the changes in light absorption allows for an estimation of arterial oxygen saturation and therefore the amount of hypoxemia. Most manufacturers claim a confidence interval of 4% for their oximeters. There have been a few studies recently describing the presenting pulse oximetry of known congenital heart disease. However, these were small studies with a small sample size of patients. Moreover, there is no literature describing the hourly ...

There Are Four Main Types Of Pulse Oximeter. Pulse oximeter is a device that are used to measure amount of oxygen in the blood. It consists of a monitor in which oxygen levels is seen in percentage values. Red blood cells absorb the light which is measured by the other side of the probe. An average amount of several pulses must be detected in order for the oximeter to work. Low blood and a weak pulse leave the pulse oximeter to detect inaccurate results.. There are four main types of pulse oximeter: bedside, wrist, hand held and finger type. Each type has several of models to choose from. A bedside oximeter is typically used in stationary medical facility and suited for non ambulatory patients such as those who are chronically ill. Wrist oximeter is those usually used by individuals involved in sports activities and is used for sleep monitoring.. The handheld type can be seen in hospital. It is commonly used in doctors offices and emergency medical services because of its portability and ...

The present study reconfirms the known risk factors for postoperative delirium which are old age, altered cognitive function, and neurological or psychiatric disease [4, 20, 21]. Beyond this, the study identifies preoperative cerebral oxygen saturation (ScO2) measured with near-infrared spectroscopy as an important predictor of postoperative delirium. This finding is in line with observations of Morimoto and colleagues [17], who showed a relationship between preoperative ScO2 and the incidence of postoperative delirium in a small cohort of non-cardiac surgical patients.. Regarding the technical basis of the method, cerebral oximetry is based on the characteristic of hemoglobin to change its absorbtion spectrum with the degree of oxygenation [22]. Two wavelengths of visible and near-infrared light are used to determine the fraction of oxyhemoglobin in a small region of the frontal lobe. According to the blood distribution in cortical brain tissue, regional cerebral oxygen saturation is a ...
The SafeBoosC trial showed that cerebral oximetry combined with a treatment guideline can reduce the the burden of hypoxia in neonates by 50% [Brit. Med. J.350, g7635 (2015)]. However, guidelines based on oximetry by one oximeter are not directly usable by other oximeters. We made a blood-lipid phantom simulating the neonatal head to determine the relation between oxygenation values obtained by different oximeters. We calculated coefficients for easy conversion from one oximeter to the other. We additionally determined the corresponding SafeBoosC intervention thresholds at which we measured an uncertainty of up to 9.2% when varying hemoglobin content from 25μM to 70μM. In conclusion, this paper makes the comparison of absolute values obtained by different oximeters possible. ...

The purpose of this his post-market, observational study titled Verification of Pulse Oximetry in the Neonatal Population is to gather data to further support the accuracy of Nonins pulse oximeters when used in the neonate population. Neonates who are currently undergoing arterial blood gas (ABG) sampling, as part of their medical care will be recruited to participate in this study for collecting opportunity samples. Nonins PureLight® noninvasive sensor will be applied to a foot of the neonate; an adhesive wrap secures the sensor to the neonate. The sensor immediately begins obtaining SpO2 and pulse rate measurements which are sent to the oximeter module for display on the Electronic Data Collection System (EDCS). At the time the prescribed ABG is drawn, the EDCS must have a marker captured to identify the end of the ABG for SpO2 comparison. The SpO2 accuracy of pulse oximeter will be measured by comparing SpO2 readings of the pulse oximeter to values of SaO2 (opportunity samples) determined ...
In this paper the measurement technique pulse oximetry is examined in 25 halothane-anaesthetized horses. Furthermore measures are presented which lead to a successful sensor placement at the tongue of the horse. The hemoglobin saturation determined by the pulse oximeter (SaO2) correlated very well with the hemoglobin saturation calculated by blood gas analysis (sO2). Nevertheless in the range of low saturation the pulse oximeter increasingly overestimates sO2. Pulse oximetry is an important progress in equine patient monitoring. A decline of oxygen saturation in the blood is detected immediately and the registration of the pulse amplitude renders a rough estimation of the quality of peripheral perfusion. ...

February 14, 2008 - Masimo yesterday said the University of California at San Francisco Medical Center completed its system-wide implementation of Masimo SET pulse oximetry and the Masimo Rainbow SET technology platform, making it the first hospital to implement Rainbow SET capabilities system-wide.. UCSF Medical Center said it performed a pulse oximetry comparison and evaluation spanning virtually every department and found that Masimo SETs obtained accurate and reliable oxygen saturation measurements under difficult conditions, including patient motion and low perfusion, as the chief reason for conversion to Masimo SET. As part of the evaluation, UCSF clinicians utilized new Masimo Rainbow SET technology to noninvasively and continuously measure physiologic and hemodynamic components that were previously only available by invasive tests. As a result, UCSF decided to expand the adoption of Masimo technologies beyond pulse oximetry to include the noninvasive patient monitoring capabilities. ...
A pulse oximeter is a non-invasive electro-optical device for measuring both the heart rate and the degree to which the hemoglobin in ones blood has been saturated with oxygen. This quantity is inferred by measuring the optical transmittance of perfused skin in different wavelengths of light. Red, green and infrared LEDs are convenient and common light sources. Blood oxygen saturation can be determined by comparing the values of Oxy-hemoglobin to Deoxy-hemoglobin. One method for doing this is by shining both a red LED and an infrared LED into a body part (such as a finger or wrist), and then measuring and comparing the relative intensities of the light from each LED in terms of a ratio. This ratio is then typically expressed as the percentage of oxygen saturation.. Pulse oximeter designs call for low power consumption and extensive analog and digital filtering. They are often implemented both as stand-alone medical and fitness devices as well as components of more comprehensive vital sign ...

Pulse Oximetry What is pulse oximetry? Pulse oximetry is a test used to measure the oxygen level (oxygen saturation) of the blood. It is an easy, painless measure of how well oxygen is being sent to parts of your body furthest from your heart, such as the arms and legs. A clip-like device called a probe is placed on a body part, such as a finger or ear lobe. The probe uses light to measure how much oxygen is in the blood. This information helps the healthcare provider decide if a person needs extra oxyg...
Heart surgery patients are more at risk of poor peripheral perfusion, and peripheral capillary oxygen saturation (SpO2) measurement is regular care for continuous analysis of blood oxygen saturation in these patients. With regard to controversial studies on accuracy of the current pulse oximetry probes and lack of data related to patients undergoing heart surgery, the present study was conducted to determine accuracy of pulse oximetry probes of finger, toe, forehead and earlobe in detection of oxygen saturation in patients admitted to intensive care units for coronary artery bypass surgery. In this clinical trial, 67 patients were recruited based on convenience sampling method among those admitted to intensive care units for coronary artery bypass surgery. The SpO2 value was measured using finger, toe, forehead and earlobe probes and then compared with the standard value of arterial oxygen saturation (SaO2). Data were entered into STATA-11 software and analyzed using descriptive, inferential and Bland

Pulse oximetry screening of the well newborn to assist in the diagnosis of critical congenital heart disease (CCHD) is increasingly being adopted. There are advantages to diagnosing CCHD prior to collapse, particularly if this occurs outside of the hospital setting. The current recommended approach links pulse oximetry screening with the assessment for CCHD. An alternative approach is to document the oxygen saturation as part of a routine set of vital signs in each newborn infant prior to discharge, delinking the measurement of oxygen saturation from assessment for CCHD. This approach, the way that many hospitals which contribute to the Australian New Zealand Neonatal Network (ANZNN) have introduced screening, has the potential benefits of decreasing parental anxiety and expectation, not requiring specific consent, changing the interpretation of false positives and therefore the timing of the test, and removing the pressure to perform an immediate echocardiogram if the test is positive. There are
The Importance Of Pulse Oximeter As A Medical Device. Ambulatory care is one of the most important and critical aspects of the health care field. These men and women are truly a life saver when it comes to emergency health situations that require a quick response. Many lives are saved everyday by these individuals and their role continues to grown within the health care field. One of the most important devices that has been used by ambulatory care is a pulse oximeter. A pulse oximeter is a medical device that is used to measure the pulse rate, pulse strength, as well as the blood oxygen saturation or SpO2 of individuals. The reason why this device is so needed and used by ambulatory care individuals is that it provides portability. Portability is key to emergency response because they never know exactly where and how they need to response to emergency situations. So for example say that there is an emergency fire in a downtown building. Well, in order for these individuals to be able to reach ...

The objective of this trial is to investigate the effects of protective lung ventilation on regional cerebral oxygen saturation (rSO2) during dura opening, that is from Ta (after dura opening) to Tb (before dura closing), in patients undergoing intracranial tumor surgery. This is a randomized controlled trial which will be carried out at the Second Affiliated Hospital of Soochow University. Fifty-four patients undergoing intracranial tumor surgery will be randomly allocated to the control group (C group) or the protective lung ventilation group (P group). In the C group, the tidal volume (VT) will be set at 8 ml/kg of predicted body weight, but positive end-expiratory pressure (PEEP) and recruitment maneuvers will not be used. In the P group, VT will be set at 6 ml/kg of predicted body weight combined with individualized PEEP during dura opening, while in other periods of general anesthesia, VT will be set at 8 ml/kg of predicted body weight. The level of rSO2, partial pressures of oxygen and carbon
In 1935, Karl Matthes (German physician 1905-1962) developed the first 2-wavelength ear O2 saturation meter with red and green filters (later switched to red and infrared filters). His meter was the first device to measure O2 saturation.[3]. The original oximeter was made by Glenn Allan Millikan in the 1940s.[4] In 1949 Wood added a pressure capsule to squeeze blood out of the ear so as to obtain an absolute O2 saturation value when blood was readmitted. The concept is similar to todays conventional pulse oximetry, but was difficult to implement because of unstable photocells and light sources; the method is not now used clinically. In 1964 Shaw assembled the first absolute reading ear oximeter by using eight wavelengths of light. Pulse oximetry was developed in 1972, by Takuo Aoyagi and Michio Kishi, bioengineers, at Nihon Kohden using the ratio of red to infrared light absorption of pulsating components at the measuring site. Susumu Nakajima, a surgeon, and his associates first tested the ...

A self-piercing pulse oximeter sensor is provided for attachment to subject, e.g., an individual or an animal. The device includes a flexible pulse oximeter sensor, an earring post, and a grommet. The earring post may be used as a piercing device if there is not a pierced body part suitable for attaching the pulse oximeter sensor to the body. Otherwise the earring post may be slid into the pierced hole. In either case, the tip of the earring post engages a grommet once passing through the body part. Thus, the pulse oximeter sensor functions in a transilluminance mode by transmitting light through the pierced body part.
with the aim of determining, in children with cyanotic congenital heart disease, which of three different pulse oximeter sensors is the most accurate at lower blood oxygen saturation levels, when compared to the gold standard of arterial blood gas analysis with CO-oximetry. The three sensors were Masimo SET® with LNCS® sensor (Masimo Standard), Masimo SET® with Blue sensor (Masimo Blue), and Nellcor™ N-600 with MAX-I sensor (Nellcor). In the study, published in the journal Pediatric Critical Care Medicine, Dr. Harris and colleagues collected data from 50 infants and children weighing 3-20 kg with baseline saturations under 90% (measured by pulse oximetry at the time of clinical assessment) undergoing surgical or catheterization procedures. Following standard care monitoring, which included placement of the Masimo Standard sensor, the Masimo Blue and Nellcor MAX-I sensors were placed on a thumb or great toe, or alternatively on a finger. Up to four arterial blood samples were taken from ...
Retinal vein occlusions (RVOs) are the second most common cause of vision loss due to retinal vascular disease.94 They can be divided into central or branch RVOs depending on the location of the occlusion. As described below, retinal oximetry has demonstrated some potential to help in differentiating between ischemic branch RVO (BRVO) and non-ischemic branch RVO. Yang et al.95 utilized the Oxymap retinal oximeter to specifically compare vessels immediately affected by a retinal vein occlusion, normal vessels in the same eye, normal vessels in the opposite eye, and healthy patients. Mean arterial and venous oxygen saturation was significantly higher in healthy individuals. BRVO was further classified into ischemic or non-ischemic using FA. They observed that, in patients with ischemic BRVO, the venous oxygen saturation in the affected vessels was decreased and the arterial saturation was increased. In non-ischemic BRVO, the mean venous oxygen saturation was lower in the affected vein. Notably, ...

In this study, a significant rise of rSO2 was observed after ACZ administration, and the results (Figs 2⇑ and 3⇑) indicated a relationship whereby the greater the Δ%rCBF, the larger the ΔrSO2. ACZ, a selective inhibitor of carbonic anhydrase, has been shown to increase CBF markedly without any change of CMRO2.10 We did not analyze blood gases during the ACZ challenge; however, it has also been reported that ACZ administration significantly increases jugular venous oxygen saturation but does not alter arterial oxygen saturation.10 Furthermore, with an advanced MRI technique, it was recently demonstrated that ACZ administration induces cerebral venous hyperoxygenation in the cortical and subcortical gray matter.12 These findings suggest that a rise in CBF induced by ACZ directly elevates tissue oxygenation and venous oxygen saturation, which results in a rise of rSO2 level as monitored with an oximeter. From the theoretical relationship between rSO2 and rCBF depicted in Fig 1⇑, ...

An oximeter sensor adapter which allows a sensor without a resistor in parallel with its LEDs to operate with an oximeter expecting such a resistor in parallel. The adapter includes LED drive electronics and appropriate oximeter drive current sensing circuitry for converting the drive signals from the oximeter into appropriate LED drive signals for the sensor. Instead of a resistor being on the sensor in parallel with one or more of the LEDs, the resistor is placed across the leads in front of the LED drive electronics and oximeter drive current sensing circuitry, on the oximeter side of the adapter. By providing LED drive electronics and oximeter drive current sensing circuitry which do not draw significant current at a low voltage, the oximeter is able to measure the resistor independently just as if it were in parallel with the LEDs.
A method and apparatus for the application of Blind Source Separation (BSS), specifically independent Component Analysis (ICA) to mixture signals obtained by a pulse oximeter sensor. In pulse oximetry, the signals measured at different wavelengths represent the mixture signals, while the plethysmographic signal, motion artifact, respiratory artifact and instrumental noise represent the source components. The BSS is carried out by a two-step method including an ICA. In the first step, the method uses Principal Component Analysis (PCA) as a preprocessing step, and the Principal Components are then used to derive sat and the Independent Components, where the Independent Components are determined in a second step. In one embodiment, the independent components are obtained by high-order decorrelation of the principal components, achieved by maximizing the sum of the squares of the higher-order cumulants of the plurality of mixture signals.

Arterial oxygen saturation (SaO(2)) was measured at 3,600-3,850 m by pulse oximetry at rest and during submaximal exercise in three study groups: 1) highland Aymara natives of the Bolivian altiplano (n = 25); 2) lowland European/North American sojour
Simply put, oximetry is the vital sign of oxygenation. It is the direct measurement of the oxygen in your bloodstream. It does not quite measure the oxygen that is actually available to your cells, but it gets close.. First, remember that actual oxygen delivery requires not just adequate hemoglobin saturation, but also enough total hemoglobin, moving around at an adequate rate. In hypovolemia, such as the shocky trauma patient, or in anemia, you might see a high SpO2 - which may be entirely accurate - but this doesnt necessarily mean that the organs are not hypoxic. After all, you could have nothing but a single lonely hemoglobin floating around, and if it had four oxygen bound to it, you would technically have a sat of 100%. But that wont keep anyone alive. Evaluating perfusion is a separate matter from evaluating oxygenation.. Second, remember our discussion of the oxyhemoglobin dissociation curve. The fact that you have oxygen bound to your hemoglobin doesnt mean that its actually being ...
What should my oxygen saturation level be?. A range of 94-99% is normal for healthy adults breathing room air which contains 21% oxygen. Anyone who is not achieving the critical blood oxygen saturation level of 90% (SpO2) or of 55-60mmHg (SaO2), may require additional oxygen. A qualified medical practitioner will assess your situation and prescribe the appropriate dose of supplemental oxygen.. Oxygen saturation levels - what do they mean?. Oxygen saturation levels measure the degree to which the haemoglobin contained in the red blood cells (erythrocytes) has bonded with oxygen molecules. Oxygen is taken in by the lungs when we breathe in.. The two most common ways of assessing the saturation of oxygen in the blood are arterial blood gases (ABG) and pulsatile oxygen.. What does an ABG measure?. An ABG test measures how efficiently your lungs are bringing oxygen into the blood stream and removing carbon dioxide. Arterial blood gases are taken from an artery usually on the wrist. This procedure can ...

SulfHb was identified in the blood of a patient who presented with persistently low oxygen saturation on pulse oximetry, normal oxygen partial pressure on arterial blood gas sampling, and repeated error messages returned by arterial blood gas machines from two manufacturers for haemoglobin species present. It illustrates the diagnostic difficulties encountered where the rarely encountered SulfHb is present, and the potential for confusion with MetHb (reviewed by Lu et al., 1998). MALDI-ToF MS successfully identified the presence of sulphur, sulphur monoxide and sulphur dioxide bound to haem moieties.. SulfHb is a rare complication of exposure of haem groups to sulphur. It causes irreversible binding of sulphur to the porphyrin ring of the haem moiety, with resultant cyanosis as SulfHb does not bind oxygen. The clinical presentation is similar to that of MetHb, but it does not respond to treatment with methylene blue or ascorbic acid. However, the presence of SulfHb decreases oxygen affinity of ...

Hypertrophic cardiomyopathy (HCM) is the most common heritable cardiovascular disease with a prevalence that approaches 1 in 500 (1).One in 5 individuals in the general population (9% to 12% of women and 27% to 35% of men) suffer from obstructive sleep apnea (OSA), an acquired clinical condition that is currently recognized as an important reversible cause of left ventricular (LV) hypertrophy (2,3). Furthermore, both HCM and OSA are independently associated with an increased risk of sudden cardiac death (1,2). Whether the 2 diseases commonly coexist in patients has not been examined.. Current guidelines acknowledge a fundamental treatment goal of HCM is alleviation of symptoms (1). Symptoms such as dyspnea, angina, and syncope can be difficult to control with pharmacologic therapy and are associated with higher mortality in HCM patients (4-6). Nonpharmacologic measures including surgical myectomy and alcohol septal ablation are often necessary for drug refractory symptomatic patients with LV ...

0194] FIG. 8A illustrates an example of a typical pulse oximeter signal output from the detectors. The red and IR LEDs are alternately powered, resulting in a substantially square wave output signal from the oximeter detector. In this case, the maximum (max) may correspond to the red LED intensity and the minimum (min) of the square wave may correspond to the IR LED intensity as seen by the detector, which may convert light energy into an electrical potential. FIG. 8B illustrates that the composite red and IR signals can be built or extracted from the square wave. The ratio of the red and IR signals may be proportional to SpO2. For example, according to certain aspects, SpO2 may be equal to k1+k2*Ared/AIR+k3*(red/IR) 2, where Ared and AIR are respective absorbances of the red and IR signals, and k1, k2, and k3 are calibration constants. In some aspects, Ared and AIR may be proportional to the red and IR signals, respectively. In some aspects, SpO2 may be proportional to a function of the ratio ...
Under the diagnostic-related group (DRG) reimbursement system, hospitals are looking to decrease costs related to unnecessary laboratory measurements. To assess the efficacy of continuous SvO2 as the only means to monitor the balance of the oxygen transport of the stable postoperative cardiac patient in the ICU, we studied 26 adult patients undergoing cardiac surgery with an uneventful postoperative course. All subjects had an Opticath fiberoptic PA catheter inserted for 29.6. +/- 11.0 hours (range 16-66) and spent an average of 42.4 +/- 17.5 hours in the Intensive Care Unit (range 20-87). Cardiac output, and Hemoglobin/Hematocrit were determined serially every 2 hours during the first 6 postoperative hours and 4 hours respectively according to our ICU practice. Arterial blood gases were determined freely in relation to changes in the hemodynamic and respiratory status. No clinical decisions were undertaken on the basis of SvO2. Retrospectively it was determined whether basing decisions on the SvO2
The reduced pressure in the aircraft cabin may cause significant hypoxaemia and respiratory distress in patients with COPD. Simple and reliable methods for predicting need for supplemental oxygen during air travel have been requested.. Objective To construct a pre-flight evaluation algorithm for COPD patients.. Methods In this prospective, cross-sectional study of 100 COPD patients, sea level pulse oximetry at rest (SpO2 SL) and exercise desaturation (SpO2 6MWT) were used to evaluate whether the patient a) is fit to fly without further assessment, b) needs further evaluation with hypoxia-altitude simulation test (HAST) or c) should receive in-flight supplemental oxygen without further evaluation. HAST was used as reference method.. Results An algorithm was constructed using a combination of SpO2 SL and SpO2 6MWT. Categories for SpO2 SL: ,95%, 92-95%, and ,92%, the cut-off value for SpO2 6MWT was calculated to 84%. Arterial oxygen pressure ,6.6 kPa was the criterion for recommending supplemental ...
If we do not make deep cuts in emissions now, the changing climate will bring heat stress, sea level rise and more extreme drought and floods. Scientific observations tell us that the world is already warming, and it appears that many of the most vulnerable people are starting to experience the impacts of climate change, Liverman said. Without a serious effort to reduce warming, and in the absence of international funds for adaptation, the food, water, health and livelihoods of millions of people will be at risk ...

Bedside catheterization of the right heart with a percutaneously introduced flowdirected catheter was carried out in 24 critically ill patients; 71 determinations of mixed venous oxygen saturation (MVo2) were obtained. A second catheter was inserted either into a central intrathoracic vein or the right atrium for simultaneous sampling of central venous (CVo2) or right atrial (RAo2) oxygen saturation.. For the group as a whole, mean CVo2 (57.9% ± 15.25) was significantly greater than mean MVo2 (53.3% ± 14.84) (P,0.001), but there was no significant difference and correlation was good between changes in central venous compared to changes in mixed venous oxygen saturation. On the other hand, there was no significant mean difference as well as an excellent correlation between individual values of RAo2 and MVo2 (r=+0.95).. Patients with heart failure or shock showed poor correlation between CVo2 and simultaneously determined MVo2. Furthermore, subjects with shock showed a mean CVo2 (58.0%±13.05) ...

To the Editor:. We read with great interest the article by Aries et al,1 in which they studied near infrared spectroscopy (NIRS) to monitor 9 patients with ischemic stroke who had more desaturations in the affected hemisphere than the nonaffected hemisphere. Their study raises questions of why this would pathophysiologically occur, such as different collateral flow or differential cerebral O2 consumption after stroke. We believe NIRS is an emerging method of measuring mixed arterial-venous brain tissue oxygenation with its primary advantage being a noninvasive procedure (versus invasive brain tissue PbO2, Licox probes, etc) as well as having a simple application and continuous trending capability for cerebral desaturations. The disadvantages of NIRS technology is that it provides only regional saturation of oxygen probe information, similar to other, typically invasive, neurointensive care unit probes such as Licox PbO2, intracranial pressure probes, or Hemedex cerebral blood flow sensors. ...
In 2018, an estimated 5.3 million children globally died before their fifth birthday, most from diseases that can be prevented and treated, including pneumonia. To improve detection of critically ill children and reduce childhood mortality, one of the most effective steps that governments can take is to ensure that tools to identify severe illness are available and accessible at the primary health care (PHC) level.. Devices like pulse oximeters (POX), which measure oxygen saturation, or clinical decision support tools (CDSTs) that help process patient information and symptoms through digital applications, are essential for alerting PHC workers to signs of severe disease and need for urgent treatment or hospital referral. While routinely used in high-income countries, in low-resource settings these critical tools are often not available, not functioning properly, not suited for infants who need them most, or providers lack appropriate training.. Thats why in July 2019, Unitaid, together with the ...

The invention is directed to a disposable oximetry sensor having an integrally formed connector. In one aspect, the sensor includes a clear flexible substrate on which a light emitter and/or light detector (collectively active component(s)) are mounted to allow those components to emit/detect light though the clear substrate and a second patient-side surface of that substrate. The clear substrate will generally contain one or more electrically conductive traces, preferably formed through a printing process, for interconnection to the active components. In this regard, the clear substrate acts as the electrical connector and lens structure for the active components reducing the overall part count for the sensors assembly. Additional materials/layers may be added to the sensor including an adhesive layer, a compressible material layer, a light blocking layer, and/or a heat sink for thermal management. Generally, any additional layers/materials applied the patient-side of the clear substrate will be
