Healthcare acquired infections are no longer confined to the hospital environment. Recently, many reported outbreaks have been linked to outpatient settings and attributed to non-adherence to recommended infection-prevention procedures. This study was divided into two parts: The first is a descriptive cross-sectional part, to assess the healthcare personnels knowledge and compliance with Standard Precautions (SP). The second is an intervention part to assess the role of health education on reducing the level of environmental and reusable medical equipment bacterial contamination. Assessment of the doctors and nurses knowledge and compliance with SP was performed using a self-administered questionnaire. Assessment of environmental cleaning (EC) and reusable medical equipment disinfection has been performed using aseptic swabbing method. The extent of any growth was recorded according to the suggested standards: (A) Presence of indicator organisms, with the proposed standard being ,1cfu/cm(2). ...

Malaria and HIV/AIDS constitute major public health problems in Ethiopia, but the burden associated with malaria-HIV co-infection has not been well documented. In this study, the burden of malaria among HIV positive and HIV negative adult outpatients attending health facilities in Oromia National Regional State, Ethiopia was investigated. A comparative cross-sectional study among HIV-positive patients having routine follow-up visits at HIV care and treatment clinics and HIV-seronegative patients attending the general medical outpatient departments in 12 health facilities during the peak malaria transmission season was conducted from September to November, 2011. A total of 3638 patients (1819 from each group) were enrolled in the study. Provider initiated testing and counseling of HIV was performed for 1831 medical outpatients out of whom 1819 were negative and enrolled into the study. Malaria blood microscopy and hemoglobin testing were performed for all 3638 patients. Data was analyzed using

Mr. Herbig, While I understand your concern and I think the practice you speak of occurs, I dont think it is widespread (to that extent). I dont have proof of this, but I think that is besides the point. We are very careful to expouse the benefits of returning to our hospital for OP therapy, but CLEARLY reinforce that it is up to the patient. The issue at hand is that CMS appears to be intending one thing (one, referral by qualified clinicians, and two, improved continuity of care by assuring close contact of the referral source and the treating therapist). There is merit in these intentions but the unintended consequences are the problem. My program provides multidisciplinary therapy services. Only 2-3 other programs exist in my medium size city. We also provide specialty programs such as Lymphedema, Wound Care, Vital Stim therapy etc. While we get many in house referrals, there are many docs that refer to us from private practice that dont have privelages at our hospital. Where will those ...

Attitude is a hypothetical construct representing ones degree of like or dislike for something. It is a positive or negative subjective view of persons, places, things, or events, referred to as attitude objects. People can also be in conflict or ambivalent towards an object, meaning that they posses simultaneously, both the positive and negative attitudes towards the attitude objects, Mama Lucy Kibaki Hospital, counselling Department handles clients with counselling needs. These formed the sample population for my study. The study focused on the attitudes that motivate or inhibit clients to seek counselling services. My main objective was to evaluate clients attitude towards counselling. The study made use of a descriptive case study method to achieve the set objectives. The study used random sampling, ending up with 1OO clients as the sample. The study used questionnaires in data collection. Descriptive statistics including mean scores, frequencies and percentages were used to analyze ...
Even low-dose glucocorticoid therapy is now known to cause bone loss, especially trabecular bone loss. This loss is greatest during the first six months of glucocorticoid use but continues at a rate of up to 10 percent per year. This bone loss and its associated increased fracture risk occur in patients taking as little as 7.5 mg of prednisone daily. Calcium and vitamin D supplementation, hormone replacement therapy (HRT), calcitonin and bisphosphonates are used to maintain bone mass.. The American College of Rheumatology recommends that all patients taking glucocorticoids also take at least 1,500 mg of calcium and 800 IU of vitamin D every day. HRT is recommended in postmenopausal women. If HRT is contraindicated, bisphosphonates and calcitonin are the suggested alternates. Aagaard and colleagues conducted this retrospective review to determine whether these recommendations were being followed in a hospital outpatient clinic.. Adult outpatients who were receiving prednisone in a dosage of at ...
Despite compelling evidence of the benefits of treatment and well-accepted guidelines for treatment, hypertension is controlled in less than one-half of United States citizens. This randomized controlled trial tests whether explicit financial incentives promote the translation of guideline-recommended care for hypertension into clinical practice and improve blood pressure (BP) control in the primary care setting. Using constrained randomization, we assigned 12 Veterans Affairs hospital outpatient clinics to four study arms: physician-level incentive; group-level incentive; combination of physician and group incentives; and no incentives (control). All participants at the hospital (cluster) were assigned to the same study arm. We enrolled 83 full-time primary care physicians and 42 non-physician personnel. The intervention consisted of an educational session about guideline-recommended care for hypertension, five audit and feedback reports, and five disbursements of incentive payments. Incentive payments

Study site and study participants. The study was conducted at the Chulaimbo Sub-District Hospital outpatient clinic in Kisumu County, Kenya. The clinic is located approximately 10 kilometers from the research laboratory at the Kenya Medical Research Institute research station in Kisian. Malaria transmission in this area is perennially high, with peaks coinciding with seasonal rains in May-July and October-November (31, 71). The study population was almost exclusively of the Luo ethnicity. P. falciparum is the primary malaria parasite species (72).. Participants were enrolled in this observational cohort between June 2013 and April 2016; they included children aged 1-10 years with acute uncomplicated malaria diagnosed by light microscopy, healthy asymptomatic children aged 1-10 years, and healthy asymptomatic adults aged 18 years or older residing in location-matched areas. All cases of uncomplicated malaria were febrile (temperature ≥37.5°C) and without signs or symptoms of severe malaria. ...

Heres whats happened: The transfer of cancer care to the outpatient setting has produced substantial savings. Its cheaper to deliver care outside the hospital, in doctor offices. This is certainly true for inpatient care. It also applies to outpatient care delivered in hospital-based clinics. One study by Avalere Health that examined three years of health claims data found (after adjusting for other variables) that the average cost of an office-based episode of cancer care was about $28,000 compared with $35,000 for a hospital outpatient department - a difference of 24%. ...

Note: Counties were sorted into quintiles based on their level of vertical consolidation in 2013. Specifically, the counties in the lowest quintile were considered to have low levels of vertical consolidation, and the next four quintiles were considered to have medium-low, medium, medium-high, and high levels of vertical consolidation, respectively. The association GAO found between higher levels of vertical consolidation and higher utilization of evaluation and management office visits in hospital outpatient departments remained after using regression analyses to control for other factors that could affect the setting in which evaluation and management office visits were performed. Specifically, GAOs regression analyses found that the level of vertical consolidation in a county was significantly and positively associated with a higher number and percentage of evaluation and management office visits performed in hospital outpatient departments-that is, as vertical consolidation increased in a ...

Enabling patients to walk in for certain types of diagnostics or treatment, without an appointment, has considerable potential. Access time can be eliminated, the time until treatment can be shortened, and patients can save one hospital visit. Moreover, patients are enabled to autonomously choose their preferred date and time for service. In terms of system efficiency, compared to an appointment system, walk-in eliminates both scheduling efforts and no-shows. Because there are various organizational and medical reasons to give a patient an appointment, most hospital outpatient clinics that serve patients on a walk-in basis also offer a limited number of appointment slots, and thus employ a combined walk-in and appointment system.. For such clinics, CHOIR researchers have developed a methodology that generates a blueprint appointment schedule, prescribing the number of appointments to schedule on each day and the timing of these appointments. The developed schedules provide good accessibility ...

After initiation, the child regularly visits the hospital outpatient clinic where they are seen by the dietitian and neurologist, and various tests and examinations are performed. These are held every three months for the first year and then every six months thereafter. Infants under one year old are seen more frequently, with the initial visit held after just two to four weeks.[9] A period of minor adjustments is necessary to ensure consistent ketosis is maintained and to better adapt the meal plans to the patient. This fine-tuning is typically done over the telephone with the hospital dietitian[19] and includes changing the number of calories, altering the ketogenic ratio, or adding some MCT or coconut oils to a classic diet.[18] Urinary ketone levels are checked daily to detect whether ketosis has been achieved and to confirm that the patient is following the diet, though the level of ketones does not correlate with an anticonvulsant effect.[19] This is performed using ketone test strips ...

BACKGROUND: It has been suggested that polymorphisms in Toll-like Receptors (TLRs) are associated with Rheumatoid Arthritis (RA), but the implicated alleles have differed between studies. The aim of this investigation was to explore whether polymorphisms of TLR genes are associated with RA in a predominantly Caucasian population from Denmark using a case-control approach. FINDINGS: DNA samples (3 university hospital outpatient clinics) were obtained from patients with RA (n = 704) and healthy controls (n = 639) in a Danish population. TLR single nucleotide polymorphisms (SNPs) were selected based on the previously reported associations with chronic autoimmune diseases. Genotyping for the TLR SNPs was performed using Sequenom Multiplex technology. We identified one SNP in TLR3, [(rs3775291, P = 0.02, OR (95% CI) 1.31 (1.1087-1.5493)] significantly associated with the whole RA cohort. Subgroup analysis according to IgM rheumatoid factor (RF) and anti-cyclic citrinullated peptide (CCP) status ...
The variables which best discriminated between the dizzy subjects and the controls were computerised dynamic posturography, computerised psychiatric assessment, and the diagnosis. Very few patients had purely psychogenic symptoms32; most had deficient postural control or signs of some balance system dysfunction or medical disorder which could account for their dizziness. However, gross vestibular abnormalities were rare in this sample, whereas neurotic disturbance was extremely common. Moreover, patients could not be readily classified as having dizziness due either to physical or psychiatric dysfunction, as signs of balance system dysfunction were as common among those with psychiatric disturbance as among those without.. The clinical profile these results suggest is of prevalent, mild combined physical and psychiatric disorder. This mixed aetiology is remarkably similar to that found in samples of dizzy patients drawn from hospital outpatient clinics9 12 and primary care.16 26 Indeed, there is ...

Shiu et al12 pointed out four boundaries and six hindering factors for expanding advanced nursing practice in nurse-led clinics. The former included community-hospital, wellness-illness, public-private, and professional-practice boundaries. The latter included stakeholder and public awareness of advanced nursing practice role in nurse-led clinics, provision of advanced specialty education programmes, organisational support, multidisciplinary collaboration, and changing health care context and provision. When ACAC was commenced, professional-practice boundary was the first and the most important hurdle. The APN was directly coached by an AC about various aspects in TJR. The TKR and THR questionnaires and workflow protocol were devised to facilitate the patient care process. She was authorised to order standard radiographs according to the region of interest, and make referral to physiotherapy, occupational therapy, prosthetics and orthotics, and the general orthopaedic clinic.
