Background: The incidence of post-operative nausea and vomitting (PONV) is increased after middle ear surgery and it may complicate and interact with reconstruction after surgery, so prevention and treatment of these complications are necessary. The aim of this study was to evaluate the efficacy of anti-emetic combinations in decreasing the PONV after middle ear surgery. Materials and methods: This double blind clinical trial was carried out during 2007-2008 on 111 patients of 15-45 years old with ASA I-II who were candidates for elective middle ear surgery under general anesthesia. The patients were divided into three groups. Patients in the ON group received Ondansetrone 0.1mg/kg + Nacl 0.9% 2cc, OD group received Ondansetrone 0.1mg/kg + Dexamethasone 0.15mg/kg and MD group received Dexamethasone 0.15mg/kg+ Metoclopramide 0.15mg/kg intravenously just before the end of surgery. The patients were evaluated for nausea, vomiting, need of anti-emetic drugs and drug dosage in recovery, 1-6, 6-12 and 12-24
PURPOSE: To study the association between petrous carotid canal (PCC) and internal carotid artery (ICA) stenoses in patients with malignant osteopetrosis. MATERIALS AND METHODS: Mean and minimum PCC diameters obtained from cranial computed tomographic (CT) scans in 20 patients were compared with similar measurements in 52 control subjects. ICA caliber, evaluated with magnetic resonance (MR) arteriography, was correlated with age and PCC dimensions. RESULTS: There was a statistically significant difference between patient and control PCC diameters. There was a strong positive correlation between age and PCC diameter in the control subjects, but only a weakly positive correlation in the patients. One or both ICAs were stenotic on all patient MR arteriograms. MR angiographic stenosis grade correlated positively with age but not with PCC diameters. Patients and Methods: A retrospective review of 1500 consecutive patients with CPA tumers revealed eight patients who were below age 18 at the time of treatment. Patients with neurofibromatosis type 2 were excluded. Medical records were evaluated with respect to preoperative symptoms, treatment, and outcome. Results: The patients ages ranged from 13 to 17 (average 15.2) years. Unilateral sensorineural hearing lass, tinnitus, and headache were the most frequent preoperative symptoms. Tumor size was between 1.5 and 4.5 cm (average 3.6 cm). Patients underwent surgery using translabyrinthine approach (n=7) and transcochlear route (n=1) with an acceptable morbidity. Im super bummed to have to do this, but I know that I cant be ready to run that race the way I want to run it, and I just dont want to dig myself any deeper into a hole.
