The Le Fort I osteotomy is one of the most widely used and useful procedure to correct the dentofacial deformities of the midface. The changes of the maxilla position affect to overlying soft tissue including the nasal structure. Postoperative nasal septum deviation is a rare and unpredicted outcome after the surgery. There are only a few reports reporting the management of this complication. In our department, three cases of the postoperative nasal septum deviation after the Le Fort I osteotomy had been experienced. Via limited intraoral circumvestibular incision, anterior maxilla, the nasal floor, and the anterior aspect of the septum were exposed. The cartilaginous part of the nasal septum was resected and repositioned to the midline and the anterior nasal spine was recontoured. Alar cinch suture performed again to prevent the sides of nostrils from flaring outwards. After the procedure, nasal septum deviation was corrected and the esthetic outcomes were favorable. Careful extubation, intraoperative

... What is it? The Le Fort fractures are named after the French surgeon Rene Le Fort. He discovered the unique fracture patterns by examining crush injuries in cadavers. A Le Fort fracture of the skull is a panfacial fracture that occurs in the mid facial region. This includes the
Severe impairment of the blood supply occurred in a patient who underwent a Le Fort I type osteotomy in which the fragment was displaced in a cranio-dorsal direction. Necrosis with sequestration of the torus palatinus present and some permanent parodontal damage ensued. Alternative procedures and management of developing circulatory impairment during surgery are discussed ...
A Le Fort fracture of the skull is a classic transfacial fracture of the midface, involving the maxillary bone and surrounding structures in either a horizontal, pyramidal or transverse direction. The hallmark of Lefort fractures is traumatic pterygomaxillary separation, which signifies fractures between the pterygoid plates, horseshoe shaped bony protuberances which extend from the inferior margin of the maxilla, and the maxillary sinuses. Continuity of this structure is a keystone for stability of the midface, involvement of which impacts surgical management of trauma victims, as it requires fixation to a horizontal bar of the frontal bone. The pterygoid plates lie posterior to the upper dental row, or alveolar ridge, when viewing the face from an anterior view. The fractures are named after French surgeon René Le Fort (1869-1951), who discovered the fracture patterns by examining crush injuries in cadavers. Lefort I - Slight swelling of the upper lip, ecchymosis is present in the buccal ...
Fort Crawford was an outpost of the United States Army located in Prairie du Chien, Wisconsin, during the 19th century. The armys occupation of Prairie du Chien actually spanned the lifetime of two fortifications, both of them named Fort Crawford. The first of these was occupied from 1816-1832, the second from 1832 - 1856. Both of the forts formed part of a string of fortifications along the upper Mississippi River that also included Fort Snelling near Saint Anthony Falls in Minnesota, and Fort Armstrong in Rock Island, Illinois. Fort Crawford was also part of a string of forts built along the Fox-Wisconsin Waterway that included Fort Winnebago in Portage, Wisconsin and Fort Howard in Green Bay, Wisconsin. The site of the second fort now includes the Fort Crawford Museum, which is located in the Second Fort Crawford Military Hospital, a 1930s reconstruction of the second forts hospital. This building, which encompasses the only surviving building fragments of either fort, was designated a U.S. ...
FORT LEATON. Fort Leaton (Old Fortin, El Fortín, Fortin) is on Farm Road 170 five miles southeast of Presidio in southern Presidio County. The fort sits on a bluff overlooking the Rio Grande and what has been called the Chihuahua Trail. Fort Leaton was listed as the first seat of Presidio County in 1850. It was the private citadel of a Chihuahua Trail freighter and the first Anglo-American farmer in Presidio County, Ben Leaton. Leaton built on the ruins of a Spanish fort founded in 1773 and known as El Fortín de San José at La Junta. After El Fortín was abandoned in 1810, the structure stood unoccupied until Juan Bustillos took it as his home in 1830. By August 1848 Leaton had bought El Fortín from Bustillos and established Fort Leaton as his home, trading post, and private fort. During that month an expedition led by John C. (Jack) Hays to find a practicable road from San Antonio to Chihuahua via El Paso spent ten days at Fort Leaton.. The fort was built in an L-shape with the long side ...
Common facial osteotomy procedures, such as the Lefort I, fall under the generic category of orthognathic surgery. As implied in its name, this involves the jaw bones and how the teeth meet or interdigitate. One of the essential aspects of this type of surgery is the need for before and after surgery orthodontics. Getting the teeth prepared so that they will meet perfectly once the facial bones are moved into their new position is not only of functional significance but often the definitive measure of the success of the procedure.. But not every patient who is in need of a Lefort I osteotomy, or any form of orthognathic surgery, is always a good candidate for orthodontic therapy. Due to compliance issues or the inability to tolerate the intraoral manipulations of orthodontic appliances, consideration can be given to doing the surgery braces free. This is done with the understanding that the goal of an ideal occlusal interdigitation is not achievable.. Case Study: This 15 year-old teenage ...
The purpose of this study was to test whether associated subcranial Le Fort III (sLF III) and Le Fort I (LF I) osteotomies are stable after large advancements of the middle third of the face and maxilla. The authors designed a retrospective study and enrolled a sample of consecutive patients with midface hypoplasia treated with associated sLF III and LF I osteotomies in this IRB-approved study between September 2013 and February 2015. To test whether the long-term stability was satisfactory, the authors compared cephalometric changes from immediately after surgery to 18 months after surgery taken from multi-slice computed tomography using two different third-party imaging software programs ...
The classification of maxilla fractures again follows the concept of areas of relative strength within the facial skeleton. There are three principal fracture lines which correspond to relative areas of weakness, and these are referred to as Le Fort fractures. By definition, these fractures must transect the pterygoid process of the sphenoid bone.. Le Fort I. This is a transverse fracture through the inferior maxillary antra, which separates off the alveolar process of the maxilla. The Le Fort I is demonstrated on the OM view with fractures through the medial and lateral walls of the maxillary antra, and the nasal septum.. Le Fort II. This is a pyramidal fracture, which separates off the central portion of the face. The OM radiograph identifies the Le Fort II with fractures through the lacrimal bones, medial orbital walls, infra-orbital rim and lateral walls of the maxillary antra.. Le Fort III. This fracture is characterised by separation of the entire facial skeleton from the skull. The ...
TY - JOUR. T1 - 091. Role of submandibular intubation according to altemir in osteotomies to access the cranial base. AU - Biglioii, Federico. AU - Mortini, Pietro. AU - Brusati, Roberta. PY - 2001. Y1 - 2001. N2 - Aim: Transfacial access to the cranial base often implies displacement of the upper teeth arcade or part of it as in Le Fort I osteotomy or zygomatic-maxillo-cheek flap. The correct and exact reposition of the bone segment is simplified by modeling of the titanium plates prior to executing the osteotomy and by controlling the restoration of individual occlusion at the end of surgery. This is possible if the endotracheal tube does not exit through the mouth. Also, downward displacement of the maxilla in Le Fort I osteotomy is limited by the presence of the orotracheal tube. As the tube may not be rhinotracheal because it would be into the surgical field, a good solution is represented by converting the passage of the orotracheal tube from the mouth through the oral pelvis to the skin ...

Alar base cinching for the correction of flat and flaring noses was first described about 10 years ago. Its use to correct, without the use of skin incisions, nasal base flaring that results from Le Fort I osteotomy has also been reported. The use of the nasotracheal tube in orthognathic surgery hampers or otherwise affects the use of alar base cinch sutures. A technique is described to effect alar base cinching in a more predictable manner ...
The Friends of Fort Fremont Historical Park have created an original exhibit about the historic fort titled Fort Fremont: Beaufort in the Spanish American War and Coast Defense. The exhibit will be on display at the Verdier House in downtown Beaufort from March 23 - June 30, 2012. A variety of collateral events are planned during the exhibits tenure, including lectures and guided tours of the fort. SC Humanitiessupported this project through a Mini Grant in February 2012.. Fort Fremont was built in 1899 on Saint Helena Island and is one of only two extant Spanish-American War fortifications that retains its character from that period. The history and role of Fort Fremont are not well-known by South Carolinians, and the exhibit is designed to help introduce this interesting historical property to a wider public. The exhibit will answer the questions: 1) Why was Ft. Fremont built in Beaufort County? 2) What was at the fort, and what did it do? And 3) What was life at the fort like for a ...

The aim of this cadaveric study was to evaluate the accuracy of computer-assisted design and manufacturing (CAD/CAM) generated splints used for maxillary repositioning during a Le Fort I osteotomy. Our results provide the following considerations. First, the accuracy related to the imaging process evaluated by the differences between pre- and post-operative translational and rotational movements as measured on the only structure that was not repositioned during the surgery such as the skull was found to be excellent. The calculation of the accuracy was directly influenced by: a) the imaging acquisition error related to the multi-slice CT scan used (CT-Sensation 64 = within 0.3 mm); b) the intrinsic software error related to the procedure of 3D segmentation and fusion between planned pre-operative and post-operative CT scan images. This step was made by powerful algorithms that allowed for a very rapid automated calculation. The precision rate of the surgical planning software used in the present ...
Introduction. For most of my first 24 years, I had only been able to eat on one side of my mouth. While this didnt cause too many problems initially, within the last few years it led to my jaw rather painfully popping and locking up. Over the winter of 2002, I was clinically diagnosed with an asymmetrical jaw. To correct this, I had 8 months of orthodontics to position my teeth for after my surgery, followed by a Sagittal Split Osteotomy to raise the left side of my jaw even with my right side and to bring my lower jaw forward, and a Le Fort I Maxillary Osteotomy to close up my overbite. After the surgery on May 19, 2004 (and a subsequent unplanned surgery on June 2, 2004), I had another 8 months of orthodontics to make sure my teeth were lined up properly.. When I was doing my research, I was astounded at the lack of web resources for my condition, and I decided to create this webpage to help towards filling that apparent void, at least in terms of personal accounts. One of the most ...

Victory for the Slave. Not only a strategic military victory, the capture of Forts Henry and Donelson provided a welcomed victory for thousands of enslaved African-Americans. Slaves from surrounding areas in Tennessee and Kentucky came to the forts seeking protection and work. Other slaves were impressed into the Union Army to work as laborers.. Photograph of several hikers on the Fort Ancient trail, ca. 1930-1960. The Fort Ancient Earthworks are a series of earthen embankments that extend for more than three and one half miles around a high bluff along the Little Miami River in southwestern Ohio. Although it is called a "fort" it probably never served as a defensive work. Ditches are located inside the walls rather than outside as might be expected in a fortification. There are more than 60 gateways in the walls, making it difficult to defend the site against enemies. The Hopewell culture (100 B.C. to 500 A. D.) of prehistoric American Indian people constructed the earthworks. Later native residents built a village and a cemetery within the walls of the already ancient South Fort. Archaeologists mistakenly assumed that these villagers had built the earthen walls. It was called the Fort Ancient culture (1000 A.D. to 1650 A.D.) after the name of the site. This mistake has caused confusion for later students of Ohio archaeology. Recent ...

Lieutenant Mullan having learned from the Indians and half-breeds of the mountains the existence of a pass leading directly to Fort Benton, through which wagons could be carried with little of no difficulty, determined upon its examination, and if practicable, to test it by bringing wagons from Fort Benton to Bitter Root valley. With this view he started from Cantonment Stevens on March 2, 1853, for Fort Benton, following the Hell Gate valley to its junction with that of the Little Blackfoot; thence along the left bank of the Missouri to the Gate of the Mountains, when he crossed the river on the ice, and following along its right bank, reached Fort Benton on the morning of the twelfth. He found from 12 to 15 inches of snow on the main divide of the Rocky mountains, little or no snow in the valleys or on either slope. He found the route until reaching the Gate practicable and easy, but here the road passed over a succession of difficult pine-clad hills that precluded the possibility of a wagon ...

BARTOW - Regional planners heard more than three hours of testimony Monday before they unanimously approved construction of a new phosphate gypsum stack near Fort Meade.. The crowd of more than 200 people was split between stack opponents, who fear water and radiation pollution from the stack, and supporters many of whom rely on the U.S. Agri-Chemicals mine for a living.. The issue now moves to the Polk County Commission for final approval.. "This is the kiss of death for Fort Meade," said Fort Meade resident John Barnett, who led an anti-stack group. "Its kind of obvious that this whole thing was cut and dried before it started.". Barnett, a retired state air pollution investigator, claimed the new stack could pollute the underground aquifer and release large concentrations of wind-blown radon and fluoride gas into the city, only 1 1/2 miles away.. US Agri-Chemicals wants to build a new stack on the south side of County Road 630 at Peeples Road. The 350-acre stack would succeed a nearby ...
