TY - GEN. T1 - EPIPHYSEAL REPLACEMENT PROSTHESIS.. AU - Wood, R. C.. AU - Carter, D. R.. AU - Fyhrie, David P. AU - Schurman, D. J.. PY - 1987. Y1 - 1987. N2 - Conventional total hip arthroplasty is a highly successful surgical procedure for treatment of severe arthritis of the hip. However, the incidence of mechanical failures in the form of femoral component loosening and stem fracture has become an increasingly significant problem, especially in younger patients, and has renewed interest in conservative alternatives. One such alternative is the Epiphyseal Replacement Prosthesis, a new research-based design which incorporates the interface contours suggested by the geometry of the epiphyseal plate or scar. Finite element stress analyses have shown that interface contours of this geometry transfer stresses from the prosthesis to the bone in a much more natural manner than previous designs. Animal studies are now being planned to verify the theoretical model and to provide an in vivo biological ...

A disc replacement prosthesis is described which is placed within the intradiscal space vacated by a removed deteriorated disc, and which affords mobility rather than fusion between adjacent vertebral bodies. The prosthesis also protects remaining discs from deterioration by providing a shock absorbing prosthesis portion. It adheres initially mechanically to the vertebral bodies and adheres through arthrodesis over a period of time.

In this report, results of the applications of titanium-gold total ossicular replacement prosthesis are presented and they are compared with other types of prosthesis used in clinical practice. Material & Method This study was performed on 24 patients, 14 male and 10 female, aged between 13 to 67. In all patients, primary pathology was chronic cholesteatomatous otitis media, and none of them were revision cases. In all cases, canal wall down technique tympanoplasty type 3 was performed with titanium-gold prosthesis. A thin block of conchal cartilage was put over the plate of the prosthesis and ear drum was reconstructed with autograft temporalis muscle fascia in all cases. The control group consisted of 30 patients operated for same middle ear pathology. In these cases, surgeries were similar except the prostheses that have been used. In this group, in seven of patients pure hydroxyapatite TORP`s were used; in the rest, the choice was a hydroxyapatite polyethilen hybrid prosthesis. An average ...
The average time of surgery in the oto-endoscope group and microscope group was 43.56±18.25 min and 48.25±8.35 min, respectively. The significant difference of operation time did not be detected between the two groups (F=3.26, P,0.05). There were no significant differences in the incidences of postoperative complications between the two groups (tinnitus: c2=0.14, P,0.05; dysgeusia: c2=0.04, P,0.05; vertigo: c2=0.06, P,0.05). The averages of air-bone gap between the two groups were no significant differences before surgery (F=4.31,P,0.05), as well as at the third month after operation (F=3.89,P,0.05). The air-bone gap values of the third month post surgery were significantly lower than that before operation in the two groups (oto-endoscope group: t=2.31, P,0.05; microscope group: t=2.26, P,0.05).The ossiculoplasty surgery did not have significant differences in the operation time, postoperative efficacy, and occurrence of complications between the oto-endoscope and microscope. Biomaterials are further evolving. Biomaterials for hearing implants depend on components, structural polymers, and electrodes. Biomaterials have transformed the medical treatment of hearing loss. The ear dysfunction, hearing loss is able to be addressed and repaired via a device implant that leverages biomaterials. Alloplastic and homograft biomaterials have been used to restore hearing with ossicular replacement prostheses, cochlear implants, and implantable middle ear devices. Biomaterials are used in implantable middle ear devices and inner ear implants. Biomaterials for middle ear applications of implantable devices are significant. The site of implantation and the mass of the implant dictate the choice of biomaterials. Hermetic sealing of an implant is best achieved by the use of metals, not polymers. The Osteocompatibility of an ossicular implant can be enhanced by the incorporation of calcium phosphates such as hydroxyapatite coatings. The market research study provides insight into market

Background It is widely accepted that infection with high-risk human papillomavirus (hrHPV) is associated with development of precancerous changes, namely, cervical intraepithelial neoplasia (CIN) and cervical cancer. It is also accepted that women who develop high-grade CIN and subsequently receive treatment with a procedure called large loop excision of the transformation zone (LLETZ), which uses a wire loop to remove abnormal cells, are at greater risk for developing further CIN than women who have never had CIN. Therefore, these women need regular follow-up assessment to detect additional abnormalities.. Main findings We searched for randomised control trials (RCTs) that compared follow-up management strategies following LLETZ treatment for CIN. We checked 1348 titles and abstracts of potentially relevant references, but we identified no randomised controlled trials (RCTs) that met our inclusion criteria. We identified trials of interest, but they were deemed not relevant because of their ...

Joint replacement surgery in the treatment of arthritic disease is now commonplace and on the whole very successful. Research into the design and development of prostheses has made major advances since the 1940s resulting in complex devices for almost all articulating joints of the body. In this thesis, a programme of work to design and test a surface replacement prosthesis for the metacarpophalangeal joint is presented. The anatomy and kinematics of the MCP joint are discussed for both normal and abnormal joint function and, based on these considerations, the design of a new surface replacement prosthesis is described. Various materials are explored with respect to their biocompatibility, durability and ease of fabrication with special attention being paid to one material - a new cross linked ultra-high molecular weight polyethylene - which is tested for wear and assessed for durability in long-term prototype tests. A finger function simulator is detailed which was designed and developed during ...
The proximal and middle phalanges from 83 proximal interphalangeal joints (PIPJs) were set in clear plastic and sectioned in the transverse plane leaving the heads whole. The sections were cleaned, shadowgraphed and measured. The medullary canals were marked on sagittal and frontal plane shadowgraphs of the intact bones and analysed. The information was then used in the design of a surface replacement prosthesis for the PIPJs. The main dorsal surface of the proximal phalanx (PP) was found to be angled to the longitudinal baseline of the bone by a mean of 5.19 degrees. This angle increased just proximal to the phalangeal head to a mean of 11.84 degrees. The mean ratio between these angles was 2.71. The phalangeal shaft bone was thicker laterally than dorsally and palmarly, and thicker dorsally than palmarly for the proximal and middle phalanges throughout the length of the bone. The shape and size of the transverse cross-section of the medullary canal changed throughout the length of the shaft. The

Currently, explains investigator Dr Philippe Mabo form the University Hospital of Rennes, France, regular in-clinic follow-up must be performed every three months, according to manufacturer guidelines. But there are two drawbacks to the in-clinic follow-up - theyre time-constrained for both the patient and the clinic, and theres no link between the time of the appointment and the clinical event or device malfunction. So theres a clinical need to consider new follow-up strategy.. In response, several manufacturers have developed new technologies which allow the remote transmission of information from the device and on its therapeutic effect. Critical data can be transmitted at any time on system integrity or unexpected events - for example, lead integrity, battery status or ineffectively delivered therapy. Data stored in the device are transmitted by phone from the patients home to the implant centre, with website access to the data.. In this context, said Dr Mabo, remote device ...

On the market since 1996, genetically modified plants expressing an insecticidal toxin (Cry toxin stemmed from Bacillus thuringiensis) target several lepidopteran and coleopteran pests. In this study, we assessed the impact of two varieties of Bt maize producing different toxins (Cry1Ab or Cry1Fa, respectively) on the biology of a storage pest: Plodia interpunctella (Hübner) (Lepidoptera: Pyralidae). The Indianmeal moths were susceptible to both toxins but showed an escape behavior only from Cry1Fa. The weight of females issued from larvae reared on Cry1Ab increased with increasing toxin concentration, but adults of both sexes reared on Cry1Fa had decreased weight. Both toxins increased development time from egg to adult regardless of sex and had no impact on the male adult lifespan. Finally, we recorded a time lag between metamorphosis from the non-Bt and the Bt diets, which increased proportionally to Cry concentration in the Bt diet.

Dr. Austin T. Moore (1899-1963), an American surgeon performed the first metallic hip replacement surgery at Johns Hopkins hospital in 1940. The original design of the prosthesis he developed was a proximal femoral replacement, with a large fixed head, made of the Cobalt-Chrome alloy Vitallium. About a foot in length, it was bolted to the resected end of the femoral shaft (hemiarthroplasty). This was unlike later (and current) hip replacement prostheses which are inserted within the medullary canal of the femur. A later version of Dr. Moores prosthesis (called as Austin Moore prosthesis) was introduced in 1952 and is still in use today ...

Abstract: Photometric follow-up of planets discovered by the radial velocity technique have yielded known transiting extra-solar planets, especially for those with the brightest host stars (e.g., HD 209458b). By using the radial velocity curves of known planets, an optimized photometric follow-up strategy can be constructed to predict transit times. The only limitation to photometric precision is determined by the chosen telescope/instrument and small uncertainties in the timing of the transit. Unfortunately, many of the photometric observations of the southern hemisphere host stars do not have the precision to confirm or exclude the transits of planets. We propose to use the CTIO 0.9m to conduct a photometric survey of transit signatures for 15 exoplanet host stars as part of the Transit Ephemeris Refinement and Monitoring Survey (TERMS). We aim to produce transit ephemerides via optimal observing windows for each of the known radial velocity planets observable from CTIO with a sufficiently ...

Aseptic loosening from polyethylene debris is the leading cause of failure for metal-on-polyethylene hip implants. The accumulation of wear debris can lead to osteolysis, the degradation of bone surrounding the implant components. In the present study, a parametric three-dimensional finite element model of an uncemented total hip replacement prosthesis was constructed and implanted into a femur model constructed from computed tomography (CT) scan data. Design optimization was performed considering volumetric wear as an objective function using a computational model validated in a previous study through in vitro wear assessment. Constraints were used to maintain the physiological range of motion of wear-optimum designs. Loading conditions for both walking and stair climbing were considered in the analysis. In addition, modification of the acetabular liner surface nodes was performed in discrete intervals to reflect the actual wear and creep damage occurring on the liner surface. Stair climbing ...
Polyethylene wear is a significant clinical problem limiting the long-term survival of joint replacement prostheses, particularly in total hip arthroplasty (THA) and total knee arthroplasty (TKA). Although the tribology of joint replacement has consequently become an area of significant research, the effect of joint fluid on lubrication in the replaced joint has been largely overlooked. Several factors that affect the tribology of metal on polyethylene articulation in joint prostheses stem from the fluid lubricating the joint. In particular, the properties and composition of joint fluid likely contribute to fluid film lubrication and boundary lubrication in joint replacements, as they do in natural joints. The primary objective of this thesis is to examine the effect of natural variation in joint fluid composition and properties on friction, lubrication, and wear in joint arthroplasty. To achieve this goal, several parameters relating to the composition and mechanical properties of joint fluid ...
Authors: Silverman CA, Linstrom CJ.. We describe tympanic membrane displacement (TMD) testing for non-invasive estimation of intracranial pressure (ICP). With the TMD test, displacement of the tympanic membrane of the middle ear is recorded during elicitation of the acoustic middle-ear reflex (AR). Increased intracranial/perilymphatic pressure displaces the resting stapes footplate laterally so that TMD during the acoustic reflex is medial. Decreased intracranial/perilymphatic pressure displaces the baseline stapes footplate position medially (inward) so that TMD during the AR is lateral. The TMD typically is bidirectional when intracranial/perilymphatic pressure is normal. Discrepant findings have been reported for the sensitivity of the TMD test to ICP as the regression of TMD on invasive measurement of the ICP reveals substantial intersubject variability and overlap among patient and control groups. Large-sample research on TMD test performance in healthy persons and patients with various ...
