Orthognathic surgery refers to the surgical correction needed to fix substantial abnormalities of the maxilla (upper jaw), the mandible (lower jaw), or both. The abnormality may be a birth defect, a growth defect, or the result of traumatic injuries to the jaw area.. Orthognathic surgery is generally performed by an Oral and Maxillofacial Surgeon to correct malocclusion (bad bite) in cases where routine orthodontic treatment has not or will not be effective. Orthognathic surgeries include the reconstruction of the mandible or maxilla, mandibular ramus, maxilla osteotomy, and mandibular osteotomy.. There are several classifications of malocclusion (the improper coming together of teeth) which may require orthognathic surgery:. Class I Occlusion -This malocclusion means that the lower anterior incisors sit directly behind the upper anterior incisors when the patient bites down. This is considered less destructive than Class II and Class III malocclusions.. Class II Malocclusion - This is ...

Previous studies have reported significant discrepancies between the surgical treatment objectives and final surgical results in orthognathic surgery patients. When dealing with complex asymmetrical maxillary movements in bimaxillary osteotomy patients, it can often be difficult to reposition the maxilla with a desirable degree of accuracy at the planning stage. Inaccurate placement of the maxilla can result in an unwanted repositioning of the mandible, which can lead to less than ideal functional and esthetic outcomes. The use of a model respositioning frame for orthognathic surgical planning is demonstrated. This can be utilized to position the maxilla at the model surgery stage with increased accuracy in both the vertical and horizontal plane. This frame can be used on semiadjustable articulators. DISCUSSION. Anteroposterior skeletal disharmony in Class III skeletal pattern patients may be surgically treated with maxillary advancement, mandibular setback or a combination of both. The type of surgery to be performed will depend on the location and the amount of discrepancy, also considering facial aesthetics.1 Many times, maxillary advancement is chosen based upon potential impairment of upper airways. However, in cases of larger skeletal discrepancies or those in which there is greater aesthetic impairment by mandibular influence, it becomes unavoidable to perform mandibular setback. In the present study, mean total discrepancy was 7.27 mm, which justified the need for bimaxillary surgery. It is important to highlight that one patient required turbinectomy and septoplasty, since there was a 5-mm upper repositioning of the maxilla, and interference of nasal turbines would hinder final bone position, which could lead to septum deviation and functional alteration.. For proper airway ...

People who can benefit from orthognathic surgery include those with an improper bite or jaws that are positioned incorrectly. Many patients with Cleft Lip and Palate need orthognathic surgery. Jaw growth is a gradual process and in some instances, the upper and lower jaws may grow at different rates. The result can be a host of problems that can affect chewing function, speech, long-term oral health and appearance. Injury to the jaw and birth defects can also affect jaw alignment. Orthodontics alone can correct bite problems when only the teeth are involved. Orthognathic surgery may be required for the jaws when repositioning in necessary.. Difficulty in the following areas should be evaluated:. ...

Orthognathic surgery is prescribed when the top and bottom teeth do not meet properly and the difference between them is greater than can be corrected by orthodontic treatment alone (braces).. When there is a significant malocclusion that cannot be corrected by braces alone then there is often a significant functional difficulty experienced by the patient. This may include pain or restricted movement at the jaw joints (TMJs), difficulty chewing certain foods, uneven wear and damage to the teeth and gums.. There is also often a significant cosmetic imbalance that will also be addressed by the surgery. This may include a very prominent or very retrusive chin, a very "gummy" smile and an "open bite" where the front teeth dont meet. Less commonly significant facial asymmetry will also require orthognathic surgery to correct the facial harmony. ...

CMF surgery involves the correction of congenital and acquired deformities of the skull and face. Due to the complex nature of the CMF skeleton, it requires extensive presurgical planning. Unfortunately, the traditional planning methods, e.g. prediction tracings and simulating surgery on stone models have remained unchanged over the last 50 years. Many unwanted surgical outcomes are the result of these deficient methods. To solve these problems, we have developed a Computer-Aided Surgical Simulation (CASS) system. Although it still needs significant improvements, the use of CASS has eliminated most of the limitations of the traditional methods. Unfortunately, it also creates a new problem that the digital establishment of dental occlusion becomes significantly more difficult. The dental articulation is an important step during the planning process to correct preexisting malocclusions or to surgically reestablish a new occlusion. The current gold standard is to utilize stone dental models and ...

Advancing the maxilla can have affects on the nose as you have described. Swelling still may be why you have the sensation of your uvula resting on your tongue. The maxillary advancement would have to be pretty significant to usually have this problem. Without examining you pre & post-op, it is tough to comment on that. Revisions can be done at anytime but since you are only 5 weeks post-op, I would recommend that you have alittle more patience before making that type of decision because soft tissue and bone healing after surgery can take considerably longer (6-9 months). If you still are not satisfied, get another opinion.. ...
