Knowledge management strategy can assist a firm to induce individuals to offer their knowledge voluntarily for a firms use, achieve distinctive advantage through full exploitation of a firms knowledge base, build institutional memory and protect it from prying competitors. Objective: To assess current heart failure (HF) care processes and organizational context in long-term care (LTC) homes as a prelude to adapting the Canadian Cardiovascular Society (CCS) HF guidelines for use in these settings. Methods: This research reports on the results of thirteen focus groups (N = 83 participants; average of 60 minutes duration) conducted in three Ontario LTC homes to better understand how HF was managed and how organizational context impacted care. Participants included physicians, nurse practitioners, registered nurses, registered practical nurses, and personal support workers. Results: Focus group findings revealed that the complexity of the LTC environment presents challenges for managing HF. Most residents have multiple advanced chronic conditions that must be managed simultaneously. When we followed up with our survey participants, we found that one in five women and one in seven men had taken early retirement benefits with early retirement being common in all occupational groups except for doctors and dentists.. 65.1 per cent of the 2206 employees for whom we had information about all types of organisational change had experienced one or more changes. Change was most frequent among social and healthcare workers (74.9 per cent) and least frequent among laboratory technicians (46 per cent).. Employees whose workplace had undergone a change of management or a merger were much more likely to have taken early retirement than those who had not. After taking background factors including age, marital status, gender etc. into account where a change in management had occurred, the likelihood of early retirement increased even more. Information Technology (IT) is widely used in organizations and managers continue to struggle with how to govern IT. IT governance concerns the decision rights and division of responsibilities to achieve value from IT investments. Any IT governance approach is incorporated into a given organizational structure. However in the particular context of public organizations, there is little research on IT organizational structure relationship with IT governance performance. In this research, a case study is done in a public organization to find out how suitable is the organizational structure of the IT department is in relation with the IT governance performance. The results reveal that the IT department organizational structure needs to suit the IT governance performance desired outcomes. RESULTS: The final model resulted in a good fit to the data (χ (1215) = 1906.86, P , .00; comparative fit index = 0.94; root mean square of error of approximation = 0.04). A significant relationship was noted between adherence to the central line bundle interventions and organizational context (β = 0.23, P , .01), whereas the relationship between context and CLABSIs was not significant (β = -0.20, P = .78).. The Patient-Centered Medical Home (PCMH) is a primary care model that provides coordinated and comprehensive care to patients to improve health outcomes. This paper addresses practical issues that arise when transitioning a traditional primary care practice into a PCMH recognized by the National Committee for Quality Assurance (NCQA). Individual organizations experiences with this transition were gathered at a PCMH workshop in Alexandria, Virginia in June 2010. An analysis of their experiences has been used along with a literature review to reveal common challenges that must be addressed in ways that are responsive to the practice and patients’ needs. These are: NCQA guidance, promoting provider buy-in, leveraging electronic medical records, changing office culture, and realigning workspace in the practice to accommodate services needed to carry out the intent of PCMH.
