Orbital fractures are common injuries found in facial trauma. Typical etiologies of orbital fractures include motor vehicle collisions and assault. We report the case of a 32-year-old male who suffered an orbital fracture from a water balloon. Additionally, we describe the aeromedical complications that may result from this injury. Finally, we attempt to answer the question of when a patient may return to flying after sustaining such an injury through review of the literature. A 32-year-old male pilot with the United States Air Force was at an outdoor event with his unit when he was struck with a water balloon launched from a sling shot into his left orbit. Shortly afterwards, he had an onset of subcutaneous emphysema and was escorted to a nearby Emergency Department. Computed tomography identified an orbital fracture with associated orbital and subcutaneous emphysema. Blowout fractures in the medial orbital wall may lead to enophthalmos, ocular dysmotility, and diplopia. Ten consecutive patients with unilateral, isolated fractures of the medial orbital wall were retrospectively studied. The radiologic accuracy of the medial orbital wall reconstructions and the long-term clinical outcomes were assessed. All cases were treated through a bicoronal approach and by use of porous polyethylene-titanium implants. The total fracture area and the orbital volume increase from the blowout were measured on computed tomographic scans. Next, we evaluated the reconstruction in the posterior part of the medial wall. This was done by calculating the ratio between the defect area and the implant area located behind the anterior ethmoidal canal. A retrospective study of patients with orbital fractures had 48% patients in the age group of 20 - 40 years with male : female ratio of 10:1. Road traffic accidents (71.43%) were the most common cause followed by injury due to fall (20%). Eighty five percent of patients had normal visual acuity at presentation and 65.57% patients had no ocular complaints. Diplopia was present in 14.2% of patients. Of the orbital fractures infraorbital rim was involved in 43.13%, floor in 19.6%, lateral wall in 13.7%, pure blow out in 14.28% and the roof in 2.9%. Important ocular findings were extraocular movements restriction in 9 (10.3%), infraorbital dysaesthesia in 3 (3.4%), enophthalmos in 2, RAPD and globe rupture in 1 patient each. 32 patients underwent surgical management. I was playing hockey one week ago and got hit in the face with a puck. I ended losing vision in that eye for a few days (but is basically completely recovered now 1 week later) and a fracture close to my nose on section where the maxillary meets the zygomatic. The Korean doctor says I need surgery but when I look at the Xray & touch it the fracture feels about a cm or 2 wide and a cm or 2 deep at most. I have read multiple times on internet that surgery is only necissary if fracture greater than 50 percent of orbital floor by the look of the xray its looks to be 10 to 20 percent. The white-eyed blowout fracture is an orbital injury in children that is commonly initially misdiagnosed as a head injury because of predominant autonomic features and lack of soft-tissue signs. We present five patients who presented with nausea and vomiting following an apparent mild head or facial injury. None of the five had any external evidence of injury. Despite each case describing diplopia, there was a delayed diagnosis of at least 24 h. CT examination demonstrated an inferior orbital wall fracture in all cases with entrapment of the inferior rectus muscle. A 4-month-old female infant presented with a growing extracranial mass at the left frontotemporal region. Covered with intact skin, the lesion measured 3.5 x 3 x 3 cm; it was well-defined, bony hard in consistency, and fixed to the underlying skull. There were no other abnormal symptoms, and routine laboratory tests were within normal limits. The mass appeared sclerotic on skull film. Computerized tomography scan of the brain (Fig. 1) showed a homogeneously enhancing expansile bone tumor. The lesion caused widening of the lower part of the left coronal suture, just above the orbital roof. To describe and evaluate a novel surgical approach to orbital wall reconstruction that uses three-dimensionally (3D) printed templates to mold a customized orbital implant. A review was conducted of 11 consecutive patients who underwent orbital wall reconstruction using 3D-printed customized orbital implant templates. In these procedures, the orbital implant was 3D pressed during surgery and inserted into the fracture site. The outcomes of this approach were analyzed quantitatively by measuring the orbital tissue volumes within the bony orbit using computed tomography. All 11 orbital wall reconstructions (6 orbital floor and 5 medial wall fractures) were successful with no post operative ophthalmic complications. Statistically significant differences were found between the preoperative and post operative orbital tissue volumes for the affected orbit (24.00 ± 1.74 vs 22.31 ± 1.90 cm3; P = 0.003). Transient traumatic isolated neurogenic ptosis (TTINP) is a sporadically reported rare entity. However, to the best of our knowledge, nearly all the reported cases are either secondary to direct periorbital trauma or surgery. We would like to report on a case of TTINP with countre-coup injury of the periorbital region. A 49-year-old female slipped and fell down while walking. She was hospitalized with a moderate headache and undisturbed mental state. The patient recalled that the force bearing point was her occipital region. Physical examination and computed tomography (CT) on admission showed right isolated ptosis and mild contusion and laceration in the bilateral frontal cortex. Further radiological investigation revealed nothing remarkable except for a fracture of the superior portion of the right medial orbital wall. Ok, so a very peculiar and interesting thing happened. This could be a feature, but I will report as bug just in case.. I had a 20x22 fenced area (most of it exposed to the sky, but with a 3x3 covered area in the center). The area was fully carpeted, with trapdoors hanging on the fences (to prevent spider climbing). In the 3x3 covered area I had a Pillager banner hanging on obsidian blocks (nether portal).. So, I was walking around the fenced area collecting sand and 2 Pillagers spawned during the day (1 patrol leader, 1 regular). The patrol leader spawned either on the trapdoor, on the fence or on the carpet, because we was trapped inside the fenced area.. The other pillager spawned either on the trapdoor, on the fence or outside the fenced area (sand), because he managed to chase me outside the fence, while the leader got himself trapped inside the fenced perimeter.. Today was the first day of no Vigabitran - currently no seizures - yet we usually dont experience any seizure changes until day 4 or 5 into the med change. He is still taking 3ml of generic Keppra in the morning and at night. The only big thing we have experienced are the blow out diapers - he has massive blow outs - we are uncertain if it is due to the meds or if it is because he has a terrible cold. Whenever he gets a cold - he gets them bad - each time he coughs - it is as if he is going to throw up - at times he does throw up and other times he doesnt- his eyes only water. That is the biggest thing that I hate - is when his left eye begins to shed a tear. It seems to be quite often - yet I have not idea why the tear comes down only one eye.
