Purpose:: To evaluate the long-term variability of confocal scanning laser ophthalmoscopy (HRT 3, Heidelberg Engineering GmbH, Dossenheim, Germany) glaucoma probability score (GPS) values in glaucoma suspects. Methods:: The study enrolled a cohort of untreated glaucoma suspects from the Diagnostic Innovations in Glaucoma Study (DIGS) who did not develop any evidence of visual field damage or change in the appearance of the optic nerve (as evaluated by stereophotographs) during an average follow-up of 9.4 ± 3.2 years. Subjects underwent ocular imaging using the commercially available HRT II (with HRT 3 software). At each visit, each eye was imaged 3 times and a mean topographic image was obtained. Subjects underwent repeated testing with HRT at approximately 12 month intervals during their follow-up. However, the true nature of retinal disease is apparent only in three dimensions. In diabetic maculopathy, direct ophthalmoscopy can reveal retinal exudates, but the degree of macula oedema, which usually underlies the decision to treat by laser photocoagulation,1 is less clear. In glaucoma, the earliest damage can be seen as thinning of the retinal nerve fibre layer with increased cupping of the optic disc.2 These changes are best viewed stereoscopically. In both cases, disease of the retinal or optic nerve head will change the surface contour of the retina, either elevating or depressing the retinal surface by up to several hundred micrometres. In a retrospective study, we compared the prevalence of retinopathy in two groups of 88 diabetic patients (84 men, 4 women) with either diabetes mellitus secondary to chronic pancreatitis (CP-DM group) or idiopathic diabetes mellitus (I-DM group). The patients of these two groups were pair-matched according to age (48.7 ± 1.1 versus 48.8 ± 1.0 yr in CP-DM and I-DM groups, respectively; mean ± SEM), sex, duration of diabetes (7.96 ± 0.56 versus 8.08 ± 0.8 yr) and therapy (80 on insulin and 8 on oral hypoglycemic agents in each group).
