TY - JOUR. T1 - The eyes have it: An unusual case of Escherichia coli ophthalmia neonatorum or a shifting landscape?. AU - Maalouli, Walid Mounir. AU - Pitt, Michael Barclay. PY - 2017. Y1 - 2017. N2 - Despite dramatic declines in the incidence of ophthalmia neonatorum with universal prophylaxis, it remains a clinically important cause of eye disease in newborns. While clear guidelines exist for the treatment of the historically primary agents of ophthalmia neonatorum (Chlamydia trachomatis and Neisseria gonorrhoeae), it is less clear how to manage newborns with conjunctivitis secondary to other bacterial organisms, particularly those also frequently implicated in neonatal sepsis. We present the case of a 3-day-old well-appearing term infant with unilateral purulent conjunctivitis. The eye culture grew Escherichia coli, an unusual cause of ophthalmia neonatorum. After a limited sepsis evaluation proved negative, the infant was switched to moxifloxacin ophthalmic drops and made a full recovery. ...

Ophthalmia neonatorum is conjunctivitis occurring in infants during the first month of life. Gonococcal ophthalmia neonatorum occurs when gonococcal infection is transmitted to newborns during delivery by women infected with N gonorrhoeae.10 The rates of gonococcal conjunctivitis in infants are directly related to the rates of gonorrhea among women of reproductive age.3 In the United States, adolescents and young adult women have the highest rates of gonorrhea, with rates peaking at age 19 years (872.2 cases per 100,000 women); among women aged 20 to 24 years, there were 648.8 cases per 100,000 women in 2017.1 Estimated rates of gonorrhea among pregnant women in the US primary care setting are not available. Although gonococcal infection rates have declined since national screening programs were implemented in the 1970s, reported gonorrhea cases have increased recently, from 105.3 cases to 171.9 cases per 100,000 population from 2013 to 2017, respectively.1 An estimated 6.2% of births in the ...
Many different bacteria and viruses can cause conjunctivitis in the neonate. The two most common causes are N. gonorrheae and Chlamydia acquired from the birth canal during delivery. Ophthalmia neonatorum due to gonococci (N. gonorrhoeae) typically manifests in the first 5 days after birth and is associated with marked bilateral purulent discharge and local inflammation. In contrast, conjunctivitis secondary to infection with C. trachomatis produces conjunctivitis 3 days to 2 weeks after delivery. The discharge is usually more watery in nature (mucopurulent) and less inflamed. Babies infected with chlamydia may develop pneumonitis (chest infection) at a later stage (range 2-19 weeks after delivery). Infants with chlamydia pneumonitis should be treated with oral erythromycin for 10-14 days.[8]. Other agents causing ophthalmia neonatorum include herpes simplex virus (HSV 2), Staphylococcus aureus, Streptococcus haemolyticus, and Streptococcus pneumoniae. Diagnosis is performed after taking swab ...

Erythema toxicum appears as small (1-3 mm), firm, yellow or white raised bumps filled with pus on top of a red area of skin. There may be a few to many lesions, and they may be found on any area of the body, with the exception of palms and soles. Erythema toxicum often begins on the face and spreads, and it may be clustered in areas where there is pressure on the skin. Although it most frequently appears during the first 3-4 days of life, erythema toxicum can be seen at birth and may not be present until 10 days of life. New lesions may appear as older ones resolve. ...

Purpose: Human conjunctival cytology has been extensively studied. Lack of data about the characteristics of conjunctival smears in patients treated for retinoblastoma who underwent enucleation and radiotherapy motivated this study. Methods: A previously studied group of 114 orbits of 58 patients was examined. The authors present the results of conjunctival cytology in 4 different situations, with and without enucleation, with and without radiotherapy. Results: In the leukocytic infiltrate in orbits with ocular bulbi predominated the lymphocyte, while in the others predominated the polymorphonuclear neutrophil. In general, the majority of epithelial cell alterations seen in the conjunctival smears of irradiated orbits were: cell with large nuclei, prominent nucleoli, and cytoplasmatic hyalinization. Conclusion: The cytological findings of the conjunctival smears do not indicate a characteristic and distinct pattern for each group ...

Sclerema neonatorum Unlike the condition just described, sclerema neonatorum presents itself as symmetrical areas of induration on cheeks, shoulders, buttocks, and calves. The skin over involved subcutaneous fat is uniformly board-like, cold, and livid in color, as though frozen. Infants so affected appear rigid because mobility is interfered with by the sclerema and they are severely ill. Mortality is high. The condition is more common in premature infants and in those with severe underlying disease, such as sepsis or dehydration. ...

In this study, among the 27 significantly downregulated microRNAs, hsa-miR-1, hsa-let-7e, hsa-miR-9, and hsa-miR-182 were found to be interrelated with the inflammatory signaling pathway and to be an important signature of SO pathogenesis. In contrast, there was no significant upregulated microRNA. Hsa-miR-9, in particular, has the potential to target a significant number of genes involved in the TNF-α and NFκB signaling pathways. A previous study has shown that overexpression of hsa-miR-9 by transfection with pre-microRNA mimics significantly attenuates interleukin-1β-induced TNF-α protein production by 40% in human primary osteoarthritis chondrocytes. Moreover, locked nucleic acid inhibitor of hsa-miR-9 marginally elevates basal TNF-α protein production compared with a control. 25 With the luciferase reporter assay, NFκB1 mRNA is also found to be directly regulated by hsa-miR-9 in human peripheral monocytes and polymorphonuclear neutrophils. 26 These results indicate that TNF-α and ...

Management of patients with loss of light perception after open-globe injury has historically been to observe or enucleate the eye with the goal of reducing the risk of sympathetic ophthalmia. Existing data, however, support rethinking that paradigm and instead considering surgery for carefully selected patients who have a chance for recovering vision. ...

On re-presentation in the UK, slit-lamp examination revealed setae embedded within his conjunctiva and eyelids causing multiple corneal abrasions. There was no evidence of intraocular inflammation. Optic disc and macula were normal. Visual acuity was 6/6 in both eyes.. The patient was taken to theatre to remove as many setae as possible. Single-use suture-tying forceps were used, including for those setae in the conjunctiva, cornea and subtarsal surfaces. Subsequent surgery (with anaesthesia) allowed more deeply embedded setae to be removed, particularly within the conjunctiva. Interestingly, very few setae initially affected the cornea. Removal was always attempted in the direction of barbs, but this was complicated because some barbs were friable and liable to break if pulled too hard.. Postoperatively, he was prescribed both ofloxacin and prednisolone (0.5%) eye-drops four times a day. To alleviate the pain from his abrasions, he was given Viscotears and cyclopentolate 1% two times per day. A ...

Neonatal sepsis is a type of neonatal infection and specifically refers to the presence in a newborn baby of a bacterial blood stream infection (BSI) (such as meningitis, pneumonia, pyelonephritis, or gastroenteritis) in the setting of fever. Older textbooks may refer to neonatal sepsis as sepsis neonatorum. Criteria with regards to hemodynamic compromise or respiratory failure are not useful clinically because these symptoms often do not arise in neonates until death is imminent and unpreventable. Neonatal sepsis is divided into two categories: early-onset sepsis (EOS) and late-onset sepsis (LOS). EOS refers to sepsis presenting in the first 7 days of life (although some refer to EOS as within the first 72 hours of life), with LOS referring to presentation of sepsis after 7 days (or 72 hours, depending on the system used). neonatal sepsis is the single most important cause of neonatal death in hospital as well as community in developing country. It is difficult to clinically exclude sepsis in ...

Authors: Lee, Seahyoung , Ganesan, Ramakrishnan , Krüger-Genge, Anne , Kratz, Karl , Franke, Ralf-Peter , Lendlein, Andreas , Jung, Friedrich Article Type: Research Article Abstract: Establishing an endothelial cell (EC) monolayer on top of the blood contacting surface of grafts is considered to be a promising approach for creating a hemocompatible surface. Here we utilized the high affinity interactions between the EC plasma membrane expressed enzyme called endothelin converting enzyme-1 (ECE-1) and its corresponding substrate big Endothelin-1 (bigET-1) to engineer an EC-specific binding surface. Since enzymatic cleavage of substrates require physical interaction between the enzyme and its corresponding substrate, it was hypothesized that a surface with chemically immobilized synthetic bigET-1 will preferentially attract ECs over other types of cells found in vascular system such as …vascular smooth muscle cells (VSMCs). First, the expression of ECE-1 was significantly higher in ECs, and ECs ...

Combined Hf isotope-age data reveal two distinct groups of zircon. Zircons of group I have Archean ages (greater than 2.65 Ga), and define a crustal array that points to protracted crust reworking in the hinterland between 3.10 and 2.65 Ga. Likely sources for these zircons are granitoids of the Pietersburg Block, which is located at the northern edge of the Transvaal basin. Zircons of group II have Paleoproterozoic ages between 2.45 and 2.08 Ga. They are mostly subchondritic, and show highly variable εHft between +4 and −19. These large variations hint to their formation in a magmatic arc and/or collisional setting. As granitoids with ages between 2.65 and 2.06 Ga are absent on the Kaapvaal Craton, detrital zircons of such ages must have been derived from sources outside of the present-day craton. One potential source could be the Ophthalmia orogenic belt, which became amalgamated to the joined Kaapvaal-Pilbara and perhaps Zimbabwe cratons (Zim)vaalbara at 2.22-2.14 Ga, whereas coeval ...
