Mechanism of exercise-induced ocular hypotension<...
TY - JOUR. T1 - Mechanism of exercise-induced ocular hypotension. AU - Martin, Bruce. AU - Harris, Alon. AU - Hammel, Ted. AU - Malinovsky, Vic. PY - 1999/4/13. Y1 - 1999/4/13. N2 - PURPOSE. Although acute dynamic exercise reduces intraocular pressure (IOP), the factors that provoke this response remain ill-defined. To determine whether changes in colloid osmotic pressure (COP) cause the IOP changes during exercise, standardized exercise was performed after dehydration and hydration with isosmotic fluid. METHODS. Progressive cycle ergometer exercise to volitional exhaustion was performed after 4 hours dehydration, and after hydration with 946 ml isosmotic liquid (345 mOsM). In each experiment, venous blood taken before and immediately after exercise was analyzed for hematocrit, plasma protein concentration, total plasma osmolality, and plasma COP. RESULTS. Exercise in both experiments significantly reduced IOP and elevated COP (each P , 0.01). Dehydration, compared with hydration, also ...

Opioid-induced ocular hypotension: actions at pre- and postjunctional by Duanran Wang
This study examined the ocular actions of an opioid agonist. Experiments were performed to evaluate the effects of DPDPE ([D-pen2, D-pen5] enkephalin), a delta opioid agonist on: 1) intraocular pressure (IOP) in rabbits; 2) cAMP accumulation in rabbit iris ciliary bodies (ICBs); 3) 3H-norepinephrine (NE) overflow from electrically stimulated sympathetic nerves in ICBs. DPDPE Lowerd IOP in normal rabbits but not in sympathectomized (SX) eyes. Naloxone did not inhibit the effect of DPDPE on IOP in normal rabbits. DPDPE inhibited 3H-NE overflow and suppressed cAMP accumulation in ICBs. The presence of naltrindole, a delta receptor antagonist, did not prevent the suppression of cAMP levels by DPDPE. Pertussis toxin (PTX) did not prevent the inhibition of cAMP levels by DPDPE. The data suggest that the lowering of IOP by DPDPE is mediated at both pre- (neuronal) and postjunctional (ciliary body) sites and may involve an atypical opioid receptor. In addition, the actions of DPDPE in the anterior segment may
Hypotony - Glaucoma Service Foundation to Prevent Blindness
Lorraine Miller, Editor, Chat Topic Researcher. On Wednesday, October 19, 2011, Dr. Anand Mantravadi, a glaucoma specialist at Wills, and the glaucoma chat group discussed Hypotony.. Moderator: Tonights topic is Hypotony.. P: What is hypotony?. Dr. Anand Mantravadi: Ocular hypotony is a term that refers generally to a subnormal low intraocular pressure that can potentially impact the eye structurally in several ways. There are loose definitions that define hypotony as an IOP of five mmHg or less. However, one may or may not have any adverse effects at that IOP.. Subnormal low IOP can lead to decompensation of the cornea, scar formation in various parts of the eye, more rapid cataract formation, retinal swelling, and swelling in the choroidal layer of the eye.. P: How long can IOP be too low before permanent problems arise?. Dr. Anand Mantravadi: I am not sure if that is known for certain. Some of the problems resulting from low IOP are reversible and can correct as the IOP spontaneously ...

The typical situation for subchoroidal infusion is ocular hypotony. In the first instance hypotony exists because an infusion in place is removed and another infusion is introduced through the same sclerotomy. The line contains air which instantly escapes through the adjacent sclerotomy for the light pipe. An angulated spatula is used from the opposite site of the infusion to free the tip of the subchoroidal infusion cannula from ciliary body tissue. Eventual remaining subchoroidal air ...
A Clinical Case from the Archives : 09/08/2006 | Veterinary ophthalmology
A swab yielded a profuse growth of Aspergillus. We performed a keratectomy and a conjunctival advancement graft to allow serum to be directed at the cornea, while also giving topical itraconazole. Unfortunately the horse went downhill rather suddenly with what turned out to be a corneal perforation as can be seen in this gross specimen of the enucleated eye. Dennis Brookes in Florida has perfected full thickness corneal grafting in such cases (Equine ulcerative keratomycosis: visual outcome and ocular survival in 39 cases (1987-1996). Equine Veterinary Journal 30:109-16) but here in the UK we dont as yet see enough to be good at such proactive intervention! A bit of global warming will increase our experience Ive no doubt! ...

Long-term outcomes of argon laser photocoagulation in small size cyclodialysis cleft | BMC Ophthalmology | Full Text
In the present study, Argon laser photocoagulation showed a good prognosis similar to surgical direct cyclopexy in small-size cyclodialysis cleft below 1.5 clock-hours. No significant differences in preoperative and postoperative clinical characteristics were shown between Argon laser photocoagulation group and surgical direct cyclopexy group. Given that the Argon laser photocoagulation is simpler and less expensive than surgical direct cyclopexy, Argon laser photocoagulation would be a better treatment of choice in small-size cyclodialysis cleft.. When the extent of the cyclodialysis cleft is not large, it is possible to wait for spontaneous recovery using conservative treatment. Previously, several reports suggested that conservative medical treatment was effective in small-size cyclodialysis cleft. Ormerod et al. reported that 1 % atropine can help to close the cleft by relaxation of the ciliary muscle to sclera [13]. Joondeph et al. suggested that abrupt cessation of steroid can make ...

Diagnosis and Treatment of Hyperfiltering Blebs | IOVS | ARVO Journals
Purpose: : to report the criteria that the authors have established to diagnosis a hyperfiltering bleb and the results of bleb resection for the treatment of this kind of complication following trabeculectomy with mitomycin C. Methods: : thirteen eyes from 13 patients with ocular hypotony caused by hyperfiltering bleb underwent bleb resection. The diagnosis of hyperfiltering bleb was made according to the following criteria: IOP below 6 mmHg measured by Goldmann applanation tonometer; absence of signs of inflammation in the anterior segment at biomicroscopy; elevated and/or diffuse and avascular bleb with or without microcysts inside it; negative Seidel test, even after a slight compression of the ocular globe; absence of ciliochoroidal detachment carried out by ultrasound biomicroscopy in the superior, inferior, nasal and temporal quadrants. The minimum follow-up was of six months. Results: : the ocular hypotony was reversed in all eyes (eight without medication and five with topical ...
Subtype Selective Muscarinic Receptor Agonist With Ocular Hypotensive Properties | IOVS | ARVO Journals
Purpose: : The objectives of this study were: 1) to determine, in vitro, the muscarinic receptor subtype selectivity of 1-[1-(2-Tolyl)-1,4-bipiperidin-4-yl]-1,3-dihydro-2H-benzimidazol-2-one (TBPB): and 2) to examine, in vivo, its effect on ocular hypotension and miosis. Methods: : Chinese hamster ovary (CHO) cells transfected with plasmid expressing human m1, m3, m4, and m5 were used to determine receptor selectivity of TBPB in a fluorometric imaging plate reader (FLPR) assay. Normotensive Dutch Belted rabbits were used to examine the effects of TBPB on intraocular pressure (IOP) and pupillary size upon topical application. In other studies, rabbits were dosed topically with TBPB in the absence and presence of pirenzepine, AF-DX 116 and 4-DAMP, M1, M2, and M3 receptor selective antagonists, respectively, to confirm the predominant muscarinic receptor subtype responsible for the ocular action of TBPB. Results: : TBPB selectively activated m1 receptors with E-max of 91% compared to carbachol ...

Our results show that operated clear corneal and limbal perforations do not cause chronic macular edema, peripapillary RNFL atrophy, choroidal thickness changes, or RVC alterations in the late post-operative period. Of course, this statement is valid only for uncomplicated corneal perforations that do not include posterior segment damage initially. To the best of our knowledge, this is the first report in the literature related to the impact of corneal perforation on posterior pole structures.. Although corneal perforation primarily affects the anterior segment of the eye, it is possible that retinal damage occurs by several mechanisms. In perforating corneal injuries, concomitant choroidal detachment was frequently seen, and this would have an impact on retinal morphology and function.4 Also, hypotony is common in corneal perforations both before and after surgical repair, so it is possible that hypotony maculopathy and its sequelae might occur to some extent. Since OCT is accepted as the best ...

Corneal Endothelial Decompansation and Ocular Hypotony in a... : Journal of Neuro-Ophthalmology
Erythrocyte sedimentation rate was 97 mm per hour, and temporal artery biopsy showed chronic inflammation with giant cells. Carotid doppler revealed moderate stenosis of the right internal carotid artery and mild stenosis of the left internal carotid artery with normal flow. The patient was given a 6-week course of systemic corticosteroids, but at 3-month follow up, there was no recovery of vision in the right eye.. There are several reports of corneal involvement in GCA (3-5). However, hypotony occurs infrequently, and it is thought to be caused by a reduction in aqueous production as a result of inflammation of the ciliary body (2-4,6-8). Hypotony was detected in one third of patients without other signs of anterior segment ischemia in a study by Huna-Baron et al (2). They suggested that low intraocular pressure may help distinguish arteritic from nonarteritic anterior ischemic optic neuropathy.. Mild corneal edema and Descemet folds have been reported in GCA (3-5) in several cases, but severe ...

PEN TORCH by opto shosho on Prezi
The simplest method of assessing ACD is,by shining a pen torch into the patients eye from the temporal canthus such that the pen torch lies in the same plane as the eye. In the case of a deep anterior chamber, the iris lies flat and the whole, iris will be illuminated. In the case of a very shallow anterior chamber the iris lies forward, blocking some of the light and very little of the iris is illuminated.Based on the amount of eye illuminated the ACD can be graded ...

Mean follow-up was 12 months (range 5-26). Main indications were diabetic macular edema in 93 patients (50%), central retinal vein occlusion in 36 (19.5%), and branch retinal vein occlusion in 34 (18.4%). 56 eyes (30%) were treatment naïve. Patients received a mean of 2.3 (range 1-5) injections (5 months median interval between injections). No systemic complications were observed. Cataract progression was documented in 22.3% of 103 phakic patients. Ocular complications were present in 4.3% of cases: 1 sterile endophthalmitis, 1 acute infectious endophthalmitis, 1 retinal tear, 1 vitreous hemorrhage, 1 ocular hypotony and 1 case of anterior chamber migration of the implant. Mean IOP before treatment was 15.8 mmHg (SD 2.6). Highest mean IOP was recorded at the 2-months visit [19 mmHg (SD 6.4)]. IOP levels raised more than 5 and 10 mmHg in 76 (41%) and 28 patients (15%), respectively. 26% of subjects required ocular hypotensive medication after injection (mean of 1.4 drugs, range 1-4). He has also had 3 eye surgeries since the last post as well. He is now only using his right eye in spite of all of the difficulties he has had with it. During his last eye procedures Dr. Trese had to separate the iris and cornea because it had begun to fuse together. The had to cut slits in his pupil to give him a greater chance for it to begin to dilate again. He has his 2nd vitrectomy in this eye as well as had to be injected with a thickening gel. His lens in his right eye also had to be removed. We are still waiting for a false lens to be fitted and placed in this eye. Meanwhile, the left eye has stopped functioning once again. This is the eye that had glaucoma and a cataract for over a year now. During his last exam on this eye the glaucoma has damaged the eye so much that it has now developed hypotony and is beginning to shrink. He is on numerous drops to try to increase the pressures again but if it continues on this track, the eye will need to be removed completely. For now the pupil ...

The Effect of Smokeless Tobacco on Intra-Ocular Pressure in a Nigerian Population - Cogprints
The study was conducted to find out the effect of smokeless tobacco (ST) on the intraocular pressure (IOP) among non-smokers and smokers. Thirty-seven healthy volunteers of between 19-30 years age, classified into two groups, non-smokers (n=22) and- smokers (n=15), were used. The IOPs of the right eyes were measured before (0min) and 1, 3 and 5mins after sniffing 40mg ST with Pulsair 2000 tonometer. Among non-smokers and smokers, the difference in mean IOP at different times (0min, 1min, 3mins and 5mins) was statistically significant by one-way ANOVA (p,0.05). Post hoc analysis using Duncan multiple range (DMR) test showed that the peak IOP was at 1min after sniffing ST. The difference in IOP before sniffing ST between smokers and non-smokers was statistically significant by Mann-Whitney test (p,0.05). Also the difference in the peak time of 1min between smokers and non-smokers was statistically significant (p,0.05). It was concluded that ST stimulates aqueous humour formation thereby tending to ...
