We show that perceptual sensitivity to visual stimuli can be modulated by matches between the contents of working memory (WM) and stimuli in the visual field. Observers were presented with an object cue (to hold in WM or to merely attend) and subsequently had to identify a brief target presented within a colored shape. The cue could be re-presented in the display, where it surrounded either the target (on valid trials) or a distractor (on invalid trials). Perceptual identification of the target, as indexed by A, was enhanced on valid relative to invalid trials but only when the cue was kept in WM. There was minimal effect of the cue when it was merely attended and not kept in WM. Verbal cues were as effective as visual cues at modulating perceptual identification, and the effects were independent of the effects of target saliency. A major strength of our study is the large sample size and the fact that all cases of severe acute maternal morbidity that occurred in all hospitals in the Netherlands were collected meticulously over two years. As far as we are aware, this is the largest study to date into the association between planned place of birth and severe adverse maternal outcomes.. Our study has some limitations as well. Firstly, because we used registration data, some were missing or may have been misclassified. For example, information on the variable "start of labour in primary or secondary care" was not always consistent between midwifery and obstetric registration. The participants in this study are parents residing in the central Canterbury province who had an infant in the Christchurch Womens Hospital labour ward or the NICU. The Christchurch Womens Hospital NICU is the only level III centre in a geographical region that has 7000 births annually, and consequently all sick newborns are admitted to this unit. Criteria for NICU admission are birth weight ,1800 g, gestation ,34 weeks, or any illness in the infant such as respiratory distress. Families were not eligible if it was known that the infant would go straight into foster care or be adopted (12 families). The only exclusion criterion was lack of written informed consent. In a perfect world, we would never be short-staffed. There would always be enough nurses so that you could do one-on-one labor support for each mom, and you had plenty of time for teaching. Gone are the days of juggling 3 labor patients. Charting would be minimal, as each nurse is equipped with a device that electronically (magically) charts every intervention that is done/heart tones/etc as it is being done, automatically. No more staying after your shift for an hour, looking at strips and charting - its already done as you go! (Man, wouldnt that be wonderful!). Decisions about how patient care would be carried out would be made by people who actually are at the bedside...not people who havent done patient care for decades, or ever. There would always be working equipment that was easy to use. Patient scanning devices for medications/labs/etc., would be user friendly.
