While many studies have reported that providing parenteral nutrition (PN) can change nutritional outcomes, there are limited data that demonstrate that PN influences clinically-important end points in critically-ill patients. The purpose of the present paper is to systematically review and critically appraise the literature to examine the relationship between PN and morbidity and mortality in the critically-ill patient. Studies comparing enteral nutrition (EN) with PN and studies comparing PN with no PN were reviewed. The results suggest that EN is associated with reduced infectious complications in some critically-ill subgroups. PN, on the other hand, is associated with increased morbidity and mortality in critically-ill patients. When nutritional support is indicated, EN should be used preferentially over PN. Further studies are needed to define the optimal timing and composition of PN in patients not tolerating sufficient EN. Strategies to optimize EN delivery and minimize PN utilization in ...
Dysphagia in cancer patients are mainly caused by brain tumor, head and neck cancer and esophageal cancer. Also both paralytic and mechanical dysphagia may occur with a high rate of malnutrition that has been reported. In cases when oral ingestion is impossible for a long time due to cancer treatments including surgery, decision should be made on if it may be an indication of nutrition support or not. In the case of nutrition support having to be initiated, the route and method of nutrition support must be determined. During enteral nutrition supply, we should predict and monitor probable complications according to the feeding rate depending on the change of patients conditions, and if required, medication assistances may be needed. Through VFSE (Video fluoroscopic swallowing exam), when the transition to oral intake is carried out, nutritional supply should be monitored to plan the tapering and discontinuation of nutrition support. Since the effort of international standardization regarding ...

Introduction:. Nutritional support of the intensive care (ICU) patients is recommended as a standard of care. It is also strongly recommended to start enteral feeding as soon as possible whenever the gastrointestinal tract is functioning (1). However, studies have repeatedly shown that with enteral support alone, insufficient energy and protein intakes often occur (2). The resulting energy and protein deficit is associated with an increased complication rate in ICU patients (3-5). Recently we proposed to decrease this deficit by promoting a combined nutrition support by enteral nutrition (EN) and supplemental parenteral nutrition (SPN) whenever EN is insufficient (6).. Contrary to former beliefs, recent meta-analyses show that parenteral nutrition (PN) does not carry excess mortality (7, 8). These reports convey a concept that is a major break-through in current routine of nutritional support in ICU patients by promoting a much wider use of PN. SPN could be the optimal modality to provide the ...

Nutrition support is considered an integral component of standard supportive care in the critically ill patient. In humans, during stress associated with trauma, sepsis, or other critical illness, there is high consumption of various nutrients by the gastrointestinal tract, immune cells, kidneys, and other organs. Requirements for and losses of these nutrients may outstrip synthetic capacity, leading to an erosion of body stores and depletion of proteins and other key nutrients. Historically, in an attempt to mitigate such deficiencies and preserve lean body mass, traditional nutrition (protein, calories, vitamins, etc) has been provided to critically ill patients. The relative merits of nutrition were evaluated in the context of protein-calorie economy (weight gain, nitrogen balance, muscle mass and function, etc). In this chapter we take a broader view of the benefits and risks of nutrition support. The benefits of nutrition support in general include improved wound healing, a decreased ...

In our study, the NST and control groups had similar patient demographics and basic data, allowing for the easy comparison of the effects of the nutritional feeding activities. In both groups, PN could be initiated within 24 hours after admission. However, on average, EN was initiated faster in the NST group. Furthermore, the EN dose was initially increased later in the control group than the NST group, suggesting that the final NCI satisfaction rate on discharge may have been lower in the control group. Such differences can be attributed to the effects of the NST activities. The EN calorie increase in the control group was decided on by the neurosurgeon in charge. However, different neurosurgeons may have had different nutritional feeding strategies and may also have reacted to FI differently. In the NST group, owing to the presence of standardized strategies and goals, better clinical outcomes may have been possible.. In the NST group, the percentage of patients in whom EN was started was ...

Pressure ulcers are frequently associated with malnutrition. Previous studies have reported that nutritional support can improve the healing rate of pressure ulcers but no attention has been clearly given to the nutritional status of the patient treated. Moreover, other investigators have suggested the additional healing power of some nutrients. Unfortunately, previous results have been unpowered by small sample size, poor monitoring of compliance to treatments and the confounding effect of other nutrients. The investigators aimed to test the effect and additional benefits of several micronutrients delivered through oral nutritional support on the healing rate of pressure ulcers in malnourished patients ...
Background: Stress cardiomyopathy (SC) is a transient ventricular dysfunction rarely described in the critical care setting. Objective: To evaluate the mechanisms, incidence, treatment and prognosis of SC. Method: This is a retrospective observational study of every critically-ill patient admitted to the ICU over a period of two years. Results: Among 1314 patients admitted in the ICU, 20 patients (1.5%) were diagnosed with SC. A total of 249 patients experienced cardiogenic shock, whereas 8% were suffering from SC. SC was suspected because of hemodynamic impairment (80% of cases), ECG modifications (15%) and/or dyspnea (15%). SC was apical (typical Tako-tsubo) in 90% and atypical in 10% of cases. Several mechanisms or conditions may explain the occurrence of SC and are may be combined: catecholamine toxicity (45%), psychological stress, seizures or neurological impairment (35%), non-epicardial coronary ischemia (20%) and left ventricular outflow track (LVOT) obstruction (10%). SC could have indirectly
Introduction NICE Nutrition Support In Adults (2006) recommends that all Trusts should have a multidisciplinary Nutrition Support Team (NST). Austin & Stroud (2007) suggest its implementation can improve parenteral nutrition (PN) services and potentially reduce PN usage by up to 25%. At Croydon University Hospital (CUH), a multidisciplinary NST was set up in October 2010, consisting of a Consultant Chemical Pathologist, Consultant Gastroenterologist, dietians and pharmacists. It combined the previous PN team with the enteral nutrition (EN) team with the aim to optimise the EN of patients thereby reducing inappropriate PN usage as recommended by NCEPOD 2010. We aimed to investigate the use of PN 1 year prior to the implementation of a multidisciplinary NST and compare this to the use of PN 1 year post-implementation at CUH. ...
Anorexia, malabsorption and metabolic derangements contribute to the malnutrition that occurs in most children with chronic liver disease. Nutritional support should be started early in the management of these children with the co-operation of a paediatric dietitian to improve quality of life and decrease post-transplant mortality. Nutritional assessment entails a detailed dietary history, physical examination and anthropometry. Weight-based anthropometric measures are unreliable while mid-upper-arm circumference and skinfold thickness provide more reliable estimates of nutritional status. Special investigations such as serum vitamin levels and skeletal X-rays further guide management. High energy (130 - 150% of recommended daily intake (RDI)) and protein (3 - 4 g/kg/day) intakes are recommended. Diets are usually enriched with medium-chain fatty acids because of their better absorption in cholestatic liver disease. High-dose fat-soluble vitamin supplements are given while care is taken to avoid

By Lita Lee, Ph.D.Psoriasis, causes and nutritional support Discusses the many causes and triggers of psoriasis plus what to do about it nutritionally. Psoriasis is described and a nutritional support program provided to help relieve this condition. The cause of psoriasis has not been determined but there are many factors associated with it, such as food allergies, metabolic problems, liver and colon problems, hypothyroidism, stress, emotional triggers and structural factors. A nutritional support program with diet, enzymes, herbs and hormonal balancing is presented.
Critical illness following head injury is associated with a hypermetabolic state but there are insufficient epidemiological data describing acute nutrition delivery to this group of patients. Furthermore, there is little information describing relationships between nutrition and clinical outcomes in this population. We undertook an analysis of observational data, collected prospectively as part of International Nutrition Surveys 2007-2013, and extracted data obtained from critically ill patients with head trauma. Our objective was to describe global nutrition support practices in the first 12 days of hospital admission after head trauma, and to explore relationships between energy and protein intake and clinical outcomes. Data are presented as mean (SD), median (IQR), or percentages. Data for 1045 patients from 341 ICUs were analyzed. The age of patients was 44.5 (19.7) years, 78 % were male, and median ICU length of stay was 13.1 (IQR 7.9-21.6) days. Most patients (94 %) were enterally fed but received
Critical illness refers to a wide spectrum of life-threatening medical or surgical conditions usually requiring intensive care, including special nutrition support like amino acids and calories and, if indicated, immune-modulatory agents. Critical illness may be prolonged if nutritional requirements are not met. Additionally, patients suffering from critical illness typically suffer from impaired immunological functions, possibly impacting on outcome. Consequently, patients should receive special nutrition support when indicated. Fresenius Kabi offers parenteral and enteral products particularly suited to nutritional support in critical illness.. In addition Fresenius Kabi has a wide range of intravenous fluids and medicines that are used in the critical care and theatre settings.. ...

in Food, Health, Nutrition, Vitamins How important is proper nutrition? Lets start with the cardiovascular syste. It needs the B vitamins in balance to protect the heart by reducing homocysteine levels in the blood (homocysteine is a common amino acid found in the blood, high levels are related to early development of heart and blood vessel disease). Arteries and nerve fibers are benefitted from the necessary B vitamins. The B vitamins B3, B6, B12, and Biotin are also beneficial to metabolic health along with vitamin E, CoQ10, chromium, magnesium and zinc, to help with processing daily sugar/carbohydrate keeping systems responsive to insulin and restoring lost insulin reception. Some sources of B Vitamins include, Beef, Brewers yeast, Chicken, Chick-peas, Eggs, Kidney bean, Navy beans, Brown rice, Whole wheat, Rye, Salmon, Sunflower seeds, Swiss Cheese, and Wheat germ….this blog to be continued.. ...

Make sure the body is working right in other areas that affect adrenals. The endocrine system as a whole, and especially the thyroid, kidneys, and hypothalamus, must be in decent working order to heal the adrenals all the way. If necessary, kill Candida and balance the gut. Start grounding regularly, at least 15 minutes a day (more is better). Do some sun gazing while youre at it (but do not look directly at the sun). Get out in nature a little every day or as often as possible (again, more is better). Avoid or compensate for Wi-Fi, cellular, and other EMFs (salt lamps help, grounding probably does, too). A five months old, Flat-Coated Retriever puppy with poor body condition diagnosed with pyelonephritis and chronic kidney disease (CKD) was referred for a nutritional consult. The conflicting protein and phosphorus requirements for growth and poor body condition versus CKD were scrutinised. To maximise the quality of life (QoL) despite the poor prognosis, several diet plans were advised taking into account the different growth periods of this medium breed puppy. At 15 months of age, body condition was still below target and the creatinine level had increased consistently since the first consult. Despite the progressive decline in kidney function, serum urea concentration decreased compared with seven months before, whereas serum concentrations of total protein, albumin and phosphate remained normal at all times. With the different adjusted diet plans, the QoL of the puppy was improved, allowing for a normal and exceptionally energetic life until the age of nearly three years ...

Scoring systems for critically-ill patients provide a measure of the severity of illness of patients admitted to intensive care units (ICUs). They are primarily based on patient characteristics, physiological derangement, and/or clinical assessments. Severity scores themselves allow for risk-adjusting outcomes, but they can also be used to provide a prediction of the overall risk of death, length of stay, or other outcome for critically ill patients. This allows for comparison of outcomes between different cohorts of patients or between observed and predicted ICU performance. There are a number of general ICU scoring systems that are in use. All scoring systems have limitations. Future scoring systems may include prediction of longer-term outcomes, and assimilation of granular data temporally and at the molecular level that could result in more personalized severity scores to help guide individual care decisions.
The COVID-19 disease has been subject to numerous publications since its emergence. Almost 20% of people suffering from COVID-19 develop severe to critical symptoms and require hospitalization, often in Intensive Care Unit (ICU). Respiratory failure is the main reason for admission in ICU of these patients. Therapeutic strategies implemented for the management of critically-ill patients may often lead to short-term muscular and functional alterations resulting in ICU-Acquired Weakness (ICUAW). These lead to long-term disabilities expressing trough dependence and quality of life impairment of survivors. The purpose of this study is to assess the quality of life, dependence and survival at one year in patients who survived from COVID-19 in ICU and are admitted in post-ICU setting for difficult weaning purpose. Ancillary studies aim to assess the course of muscle function (atrophy, structural modifications), lung function (loss of aeration) and safety of early mobilization. ...

Acute lung injury (ALI) and acute respiratory distress syndrome (ARDS) are important causes of morbidity and mortality in critically-ill patients. These life-threatening diseases are characterized by an accumulation of fluid in the alveolar sacs that ultimately prevents the diffusion of oxygen into the lung microvasculature. As an in-vivo model, lipopolysaccharide (LPS)-induced ALI is frequently used to study the molecular mechanisms that underlie, and potential therapies for inflammation-associated lung injury. However, since the experimental protocols are not standardized, inter-study comparisons are difficult. In addition, septic mice often suffer a high degree of distress during the experimental procedures. The development of an in-vitro system to accurately simulate the pathophysiological environment would, in serving as an alternative to the LPS-induced ALI-model in-vivo, help to cut down on the number of experiments with living animals.. ...

CHO - 10 to 25% of the metabolizable energy of the diet. 3. Calculate Approximate Caloric Needs. Nutritional support provides substrates for gluconeogenesis and protein synthesis and provides the energy needed to meet the additional demands of host defense, wound repair, and cell division and growth. An estimate of the animals nutrient requirements is needed to determine the minimum amount of food necessary to sustain critical physiologic processes.. Accurate, direct measurements of energy expenditure in sick dogs and cats are not available. Despite the paucity of data on energy requirements of sick animals, opinion exists that the requirements of critically ill animals are less than normal maintenance amounts, but slightly greater than RER. The RER is the patients energy requirement at rest in a thermoneutral environment and in a post-absorptive state. The veterinary literature is rife with reports documenting various values or factors for altering initial calculations to determine the ...
