Hello, my name is Lori Newton and I have been a staff nurse in the operating room at TGH for 25 years.. On Dec. 22, 2010, I found a breast lump and quickly asked for an opinion by the Surgical Oncology department at Princess Margaret. The lump was indeed cancerous and I quickly had surgery, followed by chemotherapy, more surgery and this year, a major plastic reconstruction by the Plastic Surgery Department at Toronto General Hospital.. The whole experience made me realize how much courage our patients need to face a critical illness. I had multiple tests, procedures and visits to TGH, TWH, and PMH. I must say everywhere and every person I dealt with was exceptional.. This wasnt because I was a staff member either, because many times they didnt know this fact.. I always knew I worked at an excellent institution but until I was a patient I didnt realize UHN was SO GOOD!. All of the interfacing between the multiple services and departments was seamless and worked perfectly. I couldnt complain ...

The issue of obesity in patients treated by dialysis presents a difficult dilemma. Although obese dialysis dependent patients face obstacles for transplantation, observational studies suggest that greater BMI is associated with improved survival in this population.[40-44] One of the largest studies, which included more than 400,000 dialysis dependent patients who initiated dialysis therapy from 1995 to 2000, noted substantial and significant differences in overall survival by BMI groups after a median follow-up of 2 years. The unadjusted annual cardiovascular mortality rate was approximately 2-fold greater in patients with BMI less than 22 kg/m2 compared with those with BMI of 37 kg/m2 or greater, and the all-cause mortality rate was more than 2-fold greater in patients with BMI less than 19 kg/m2 versus 37 kg/m2 or greater.[42] The prevalence of reported congestive heart failure was similar across BMI groups, but ischemic heart disease, myocardial infarction, peripheral vascular disease, ...

An important objective of the work item type definition language and the work item query language is for definitions to be portable between Team Foundation Servers. It is also important for third-party integrations to be able to find and refer to specific fields. To this end, the work item type definition language includes the concept of a field reference name. Field reference names are globally unique in the same way that a namespace in the .NET Framework is globally unique.. For this reason, field reference names cannot be renamed. If, for example, you changed the field name "Title" to "Header," the field reference name of that field remains the same.. In keeping with the .NET namespace tradition, two namespaces are predefined: System and Microsoft. The System namespace includes all system fields that are mandatory for Team Foundation system functions. The Microsoft namespace defines all required fields for work item types defined by Microsoft. Customers and partners can create their own field ...

Session presented on Monday, November 9, 2015: Evaluating the Use of C-HOBIC and the C-HOBIC Transition Synoptic Report Purpose/Objective Electronic health records (EHRs) offer the opportunity to improve patient outcomes across the continuum of care. Fundamental to achieving these improvements is the use of standardized clinical data to measure and compare clinical outcomes across care settings. The Canadian Health Outcomes for Better Information and Care (C-HOBIC) encompasses a suite of standardized outcomes including: a) functional status, b) symptom management (dyspnea, nausea, pain, fatigue), c) continence, d) risk for falls and skin breakdown, and e) readiness for discharge. The acute care admission and discharge assessments of these outcomes has been captured numerically and graphically in a Transition Synoptic Report (TSR) designed for sharing with clinicians in post-acute discharge sectors. The TSR provides a summary of the acute care admission and discharge C-HOBIC information. By ...
Objective: To (1) examine the opinions of medical staff working in spinal cord injury (SCI) centres (SCICs); (2) evaluate their knowledge, attitudes and practices towards obesity prevention and management; (3) report the number of beds and dietitians available at each SCIC. Methods: A 37-item questionnaire was sent to 23 SCICs in the UK, the Netherlands, Belgium and the Republic of Ireland between September 2012 and January 2013. Results: Eighteen SCICs returned the questionnaires for analysis. All respondents stated that they had an interest in obesity treatment but only 2.3% of the respondents received training in obesity management. Sixty-one percent of staff did not consider body mass index (BMI) to be appropriate for use in SCI patients and subsequently less than half of the respondents use BMI routinely. The majority of respondents reported that they are confident in dealing with overweight (74.5%) and obese (66.1%) SCI adults, less than half (44.1%) are confident in treating overweight ...
Background: Virtual reality (VR) simulation has been recommended for use as part of a tiered training approach for learning carotid artery stenting (CAS) techniques. However, before such VR simulation technology can be used, the device must first go through a rigorous validation process. This is the first report on the face and content validation efforts on the Vascular Interventional System Trainer (VIST, Mentice) used in CAS training.. Methods: Experienced physicians (n = 19) performing percutaneous endovascular interventions underwent didactic and simulator-based training as part of a structured course for CAS training. Each operator completed 2 VR simulated right internal CAS procedures and then assessed the VIST simulator for both its face and content validity. The assessment was divided into 3 sections based on how the simulation tools looked, felt, and behaved during the procedure. Scores were rated on a 5-point Likert scale.. Results: The guide catheter configuration and placement in the ...

The earliest work in computerized knowledge representation was focused on general problem solvers such as the General Problem Solver (GPS) system developed by Allen Newell and Herbert A. Simon in 1959. These systems featured data structures for planning and decomposition. The system would begin with a goal. It would then decompose that goal into sub-goals and then set out to construct strategies that could accomplish each subgoal. In these early days of AI, general search algorithms such as A* were also developed. However, the amorphous problem definitions for systems such as GPS meant that they worked only for very constrained toy domains (e.g. the "blocks world"). In order to tackle non-toy problems, AI researchers such as Ed Feigenbaum and Frederick Hayes-Roth realized that it was necessary to focus systems on more constrained problems. It was the failure of these efforts that led to the cognitive revolution in psychology and to the phase of AI focused on knowledge representation that ...

Obesity is an increasing epidemic in both the US and veteran populations, yet it remains largely understudied in the Veterans Health Administration (VHA) setting. The purpose of our study was to identify barriers to the effective management of obesity in VHA primary care settings. Three focus groups of clinicians from a Veterans Affairs Medical Center (VAMC) and an affiliated Community Based Outpatient Center (CBOC) were conducted to identify potential barriers to obesity management. The focus groups and previously published studies then informed the creation of a 47-item survey that was then disseminated and completed by 55 primary care clinicians. The focus groups identified provider, system, and patient barriers to obesity care. Lack of obesity training during medical school and residency was associated with lower rates of discussing diet and exercise with obese patients (p | 0.05). Clinicians who watched their own diets vigorously were more likely to calculate BMI for obese patients than other
The individuals for this study included the last 105 consecutive subjects who were part of a cohort of 459 stroke patients in the Kansas City Stroke Study. The participants were recruited from any of 12 participating hospitals in the greater Kansas City area. Details on subject eligibility and recruitment have been described in a previous report.16 To be accepted into this study, the subject had to have a confirmed eligible stroke, defined by World Health Organization criteria17 as of "rapid onset and of vascular origin reflecting a focal disturbance of cerebral function, excluding isolated impairments of higher function and persisting longer than 24 hours." The stroke was confirmed by clinical assessment and/or by a CT/MRI scan. Trained nurses/physical therapists reviewed medical records and interviewed both patients and physicians to determine whether the patient was eligible and had consented to enrollment. The patients were evaluated with a variety of standardized assessments at enrollment ...
Background: The obesity pandemic is causing premature mortality, increasing morbidity and escalating healthcare costs. It is globally acknowledged that current obesity management strategies are failing. It is also accepted that clinicians may hold an Anti-fat bias. To-date it has not been established if there is a relationship between strategy failures and clinicians attitudes. This uncharted territory was the driving impetus to undertaken a study investigating possible links and consequences. Aims: To establish healthcare clinicians attitudes towards patients with obesity and test the extent to which their attitudes influenced obesity management. Methods: Online vignettes were generated via Factorial Survey Design methods to assess obesity management by clinicians. The obesity Implicit Association Test (IAT) was used to assess subconscious Anti-fat bias. Data were collected anonymously from a multi-professional convenience sample in a virtual research site managed by Project Implicit®. ...

To be viable, theories of obesity pathogenesis must account not only for how excess body fat is acquired, but also for how excess body fat comes to be biologically defended. To date, the preponderance of research has focused on the former. However, we must consider the possibility that some (perhaps even most) mechanisms underlying weight gain are distinct from those responsible for the biological defense of excess fat mass. A key question, therefore, is how the energy homeostasis system comes to defend an elevated level of fat mass (analogous to the defense of elevated blood pressure in patients with hypertension). Answering this question requires an improved understanding of the neuro-molecular elements that underlie a "defended" level of body fat. What are the molecular/neuroanatomic predicates that help establish and defend a "set point" for adiposity? How do these elements regulate feeding behavior and/or energy expenditure, so as to achieve long-term energy balance? By what mechanisms is ...
Techniques are described that allow an implantable device to sense gastric data and activity data from a patient, and estimate the patients amount of energy consumed and energy expended based on the sensed data. A system provides feedback to the patient, a family member, or a doctor about the patients energy consumed, energy expended, and net energy. The data may be provided in table or graphical format, and may show daily or weekly energy balance data or may show a trend of the daily or weekly energy data. The patient may receive feedback by an implanted alert module that provides and audio alert or a vibration alert. In addition, data acquired by the system may be used to adjust the patients stimulation therapy parameters.

OBJECTIVE: Adjustment in the doses of opioids has been a focus of interest for achieving better fast-track conditions in cardiac anesthesia, but relatively sparse information exists on the potential effect of psychologic and behavioral factors, such as stress, anxiety, and type of personality, on anesthesia requirements and patient turnover in the cardiac recovery unit (CRU); to the authors knowledge, this particular focus has not been systematically investigated. In this randomized study, the authors tested the hypothesis that low-dose sufentanil, compared with a standard dose, can improve fast-track parameters and the overall quality of recovery ...

The aim of EASO is to build a network of accredited specialised adiposity centres, which can provide the highest standard of quality and efficiency in patient care. Institutes which want to become members in the EASO Collaborating Centres for Obesity Management (COMs) are judged by proven European and academic guidelines. These include an evaluation of the number and new admissions of patients, the expertise of the employed staff and the equipment for treating obese patients. "For the IFB the membership in EASO enhances the centres visibility on an international level and enables a professional dialogue on a high standard", emphasizes Dr. Tatjana Schütz, head of the IFB Core-Unit "Nutrition and clinical phenotyping".. EASO acts as an interface for the collaboration of important, EU-funded projects and supports candidates in establishing and advertising their institute. After the baseline visit, study participants visited at one, three, six, nine, and 12 months. The primary and secondary outcome measures were assessed, as well as vital signs and any adverse effects. At one, six, and 12 months, blood samples were taken to measure the level of topiramate in the blood. These samples were analyzed only after the trial to ensure that the investigators did not know who was in which group.. My veins had become small due to the disease, because there is less need for blood as muscle atrophies. My veins were a particular challenge for the expert nurses of Mass General. Sometimes it took two attempts to coax blood out of my veins, and sometimes my veins refused to yield a drop.. After the 12-month visit, I had the option of taking topiramate. This is called the open-label stage of the trial, in which all participants have the opportunity to take the experimental drug. During the open-label stage, I learned that I had been in the topiramate group. I was happy: finally ...
