As the most common NK/T-cell lymphoma in Asian countries, extranodal NK/T-cell lymphoma, nasal type (ENKTL), has unique clinical features and a strong association with Epstein-Barr virus (EBV). In order to gain a preliminary understanding of the relationship between ENKTL and EBV, we performed genotypic analysis of EBV and investigated LMP1 expression in extranodal NK/T-cell lymphoma. Our study shows that ENKTL is an EBV-associated malignancy and that A, C and F are the predominant EBV genotypes in northern China. LMP1 expression is stronger in extranasal sites than nasal sites, and the expression level is strongly correlated to ENKTL and may play an important role in the development of ENKTL.
We evaluated the outcome of 65 French patients with Extranodal NK/T-cell lymphoma, nasal type (ENKTL) undergoing haematopoietic stem cell transplantation (HSCT) (19 allogeneic and 46 autologous). Fifty-four patients (83%), most of which receiving L asparaginase (L-aspa) containing regimens (81%), achieved complete or partial response at time of HCST. After a median follow-up of 79.9 months, 4-years progression-free survival (PFS) and overall survival (OS) were similar in both autologous and allogeneic groups (PFS: 34% vs 26%, p=0.12 and OS: 52% vs 53%, p=0.74). Response status at HSCT was the major independent prognostic factor on survival (OS: HR: 4.013 [1.137; 14.16], p=0.031 and PFS: HR: 5.231 [1.625; 16.838], p=0.006). Extranodal NK/T-cell lymphoma (ENKTCL) reports from sub-Saharan Africa (SSA) are remarkably rare, despite early childhood acquisition and high prevalence of the causative infectious agent, Epstein-Barr virus (EBV), and frequent occurrence of other lymphoproliferative disorders causally associated with EBV. At a national teaching hospital in Malawi, three patients of African descent were seen with ENKTCL between 2013 and 2014. Patients were aged between 29 and 60 years, two with craniofacial involvement and one with a primary abdominal tumor, and all were HIV-negative. All had systemic B symptoms, and two severely impaired performance status. On histologic review, morphology and immunophenotyping demonstrated classical ENKTCL features in all cases, including diffuse proliferations of intermediate-to-large atypical lymphocytes with high mitotic activity and extensive background necrosis, positivity for CD3 and CD56, and negativity for CD20. We describe a case of radiation-induced olfactory neuroblastoma in a patient with a history of radiation for Graves ophthalmopathy. We also reviewed the literature and found four other cases of radiation-induced olfactory neuroblastoma reported since 2000, suggesting prior radiotherapy as one risk factor for this disease.

Esthesioneuroblastoma (ENB) is a rare, aggressive tumor with no established treatment in children. We analyzed a series of pediatric ENB patients with the aim of improving our knowledge of this disease. 9 patients (6 males; age 0.9-18 years, median 9.9) were identified by searching the AIEOP (Italian Association of Pediatric Hematology and Oncology) registry and the national databases of rare tumors, soft tissue sarcomas (STS) and neuroblastomas. The data on the cases included in STS treatment protocols were collected prospectively and histology was centrally reviewed; the data and histology concerning the other children were reviewed for the purpose of this analysis. All tumors occurred in the sinonasal region with bone erosion (7 patients) and intracranial (4) or intraorbital (4) extension. Three patients were in Kadish stage B, and 6 in stage C. My husband is 76 years old and was operated on February, 2014, for an esthesioneuroblastoma. He had both the cranial and nasal approach. Margins were all clear, thankfully. Doctors recommended no chemo or radiation, but follow-up scans every 6 months for 2 years. He irrigates twice a day. While it is a minor problem in view of all that can happen he constantly complains of a wierd smell, which cant really be a smell because his olfactory nerve was totally removed. He also complains of stuffiness in his sinus area that causes inability to sleep through the night. I notice that his short term memory is not very good. CASE DESCRIPTION: A 12-year-old 500-kg (1,100-lb) American Quarter Horse mare was evaluated because of chronic mucopurulent, bloody discharge from the left nostril, inspiratory dyspnea, and respiratory noise. CLINICAL FINDINGS: The horse had severe inspiratory dyspnea and stertorous respiration with no airflow from the left nostril. A temporary tracheostomy was performed. Endoscopy revealed a tan mass protruding from the left middle nasal meatus into the left common nasal meatus; it extended caudally into the nasopharynx and around the caudal edge of the nasal septum into the right nasal cavity. Radiographically, a soft tissue opacity was evident in most of the left nasal cavity and left paranasal sinuses. Cytologic examination of mass tissue revealed evidence of pyogranulomatous rhinitis; thickly encapsulated, budding yeast typical of Cryptococcus neoformans were detected. To the editor: The discussion in the masterly paper by Fauci, Johnson, and Wolff on the radiotherapy of the idiopathic midline granuloma in the July issue (2) raises several important points.. The entity of the lethal or midline granuloma has been doubted. I agree with Fauci and colleagues that this is an entity characterized by destructive lesions localised to the upper respiratory tract that is unrelated to detectable tumours and probably caused by a fulminant inflammatory response to an unknown antigen. 8 Prognostic factors were analysed in tumours from 98 patients diagnosed with IP in Stockholm between 2000-2010. In total, 12.2% of the IPs were HPV positive and p16 overexpression was found in the only high-risk HPV positive tumour. Patients with HPV positive lesions were younger and tended to present with more dysplasia and to relapse less frequently. The tumours also had a higher proportion of EGFR expression compared to HPV negative tumours (91.7% and 52.3%, respectively). Stathmin was expressed by the tumours cells and not at all or weakly in the normal mucosa and more specimens with dysplasia were stathmin-positive than specimens without dysplasia (40.0% as compared to 12.6%).
