The major findings of this study were as follows: (1) significant vasospasm occurred in cerebral arteries on day 7, and vasospasm was attenuated after nitroglycerin administration; (2) rCBF on the right side was significantly decreased after SAH, and nitroglycerin significantly improved rCBF on the same side; (3) cGMP levels were significantly decreased in the right MCA and cerebral cortices after SAH, and nitroglycerin did not increase those cGMP levels; (4) nitroglycerin significantly increased cGMP levels in the basilar artery; and (5) cAMP levels were not affected by either SAH or nitroglycerin.. In this study, MABP was significantly decreased by nitroglycerin, but rCBF was increased. In the same primate model, the central conduction time on the clot side has been observed to remain fairly constant in MABPs above 80 mm Hg.13 The change in rCBF was found to be very small in MABPs of 80 to 100 mm Hg.13 Thus, the dosage of nitroglycerin in our study would be safe for the maintenance of rCBF and ...

The effects of the nitric oxide donor nitroglycerin on gastric emptying and antropyloroduodenal motility were evaluated in nine healthy male subjects (ages 19-36 yr). Antropyloroduodenal pressures were recorded with a manometric assembly that had nine side holes spanning the antrum and proximal duodenum and a pyloric sleeve sensor; gastric emptying was quantified scintigraphically. In each subject, the emptying of 300 ml of 25% glucose labeled with 99mTc was assessed on two separate days during intravenous infusion of either nitroglycerin (5 micrograms/min in 5% dextrose) or 5% dextrose (control). Studies were performed with the subject in the supine position; blood pressure and heart rate were monitored. Nitroglycerin had no significant effect on blood pressure or heart rate. Nitroglycerin slowed gastric emptying (P , 0.02), and this was associated with greater retention of the drink in the proximal stomach (P , 0.05). In both nitroglycerin and control studies, ingestion of the drink was ...
The aim of this study was to evaluate the impact of concurrent nitroglycerin administration on the thrombolytic efficacy of recombinant tissue-type plasminogen activator (rTPA) in patients with acute anterior myocardial infarction (AMI). Sixty patients (53 men, 7 women; mean age 54 +/- 7 years) with AMI entered the study. Thirty-three patients were randomized to receive rTPA alone (100 mg in 3 hours) (group A) and 27 to receive rTPA plus nitroglycerin (100 micrograms/min) (group B). Time from the onset of chest pain and delivery of rTPA was similar in the two groups of patients. Patients in group A had signs of reperfusion more often than the patients in group B (25 of 33 or 75.7% vs 15 of 27 or 55.5%, p less than 0.05). Time to reperfusion was also shorter in group A than in group B (19.6 +/- 9.4 minutes vs 37.8 +/- 5.9 minutes, p less than 0.05). Group B had a greater incidence of in-hospital adverse events (9 of 27 vs 5 of 33, p less than 0.05) and a higher incidence of coronary artery ...
Nitroglycerin (TNG) decreases ST-segment elevation accompanying myocardial ischemia, but its effect on morphometrically and enzymatically estimated infarct size (IS) has not been defined. Accordingly, coronary occlusion was produced in 92 conscious dogs; 65 survived for 24 hours. Thirty-three received TNG (200-300 microgram/min i.v. for 8 hours) and the results were compared with those in 32 untreated dogs. Coronary blood flow (CBF) was measured with tracer microspheres (141Ce, 85Sr and 95Nb) 5 minutes after occlusion before TNG, 20 minutes after TNG and again at 8 hours. Mean blood pressure decreased from 103 to 84 mm Hg with TNG, vs 99 to 94 mm Hg in controls (p , 0.02). Nitroglycerin increased CBF in the subendocardium of ischemic areas by 45% (0.09 to 0.13 ml/min/g). The dogs were sacrificed after 24 hours and IS was estimated morphometrically (25 +/- 1% vs 27 +/- 1% of left ventricular weight) and from myocardial CK depletion (23 +/- 1% vs 24 +/- 1%) were similar for the two groups. Thus, ...

Nitroglycerin was shown to induce significant positive inotropic effect in adequate oxygen supply in isolated papillary muscles of the rat heart left ventricle. The effect depended upon inotropic reserve of the myocardium. The decrease in oxygen supply of papillary muscles inhibited their contractile activity. In these conditions, nitroglycerin exerted no positive inotropic influence on papillary muscles. Positive inotropic action of nitroglycerin became weaker after preliminary perfusion of papillary muscles with propranolol. Calcium antagonist verapamil completely abolished the positive inotropic action of nitroglycerin. The data suggest that positive inotropic action of nitroglycerin depends upon transsarcolemmal influx of calcium ions and is partially associated with the stimulation of myocardiac beta-receptors.
Absorption: In six healthy subjects, the average absolute bioavailability of nitroglycerin applied to the anal canal as a 0.2% w/w ointment was approximately 50% of the 0.75 mg nitroglycerin dose. Distribution: The volume of distribution of nitroglycerin following intravenous administration is about 3 L/kg. At plasma concentrations between 50 and 500 ng/mL, the binding of nitroglycerin to plasma proteins is approximately 60%, while that of 1,2- and 1,3-dinitroglycerin is 60% and 30%, respectively. Metabolism: Nitroglycerin is metabolized by a liver reductase enzyme to glycerol di- and mononitrate metabolites and ultimately to glycerol and organic nitrate. Known sites of extrahepatic metabolism include red blood cells and vascular walls. In addition to nitroglycerin, the two major metabolites, 1,2- and 1,3- dinitroglycerols are found in plasma. The contribution of metabolites to the relaxation of the internal anal sphincter is unknown. The dinitrates are further metabolized to nonvasoactive ...

PY - 1987/8. Y1 - 1987/8. N2 - A model comprising six compartments, a systemic and presystemic compartment for nitroglycerin and each of its two dinitrate metabolites is presented to describe the interrelationships between plasma concentrations of the two metabolites and metabolism in skin after intravenous and transdermal ointment administration of nitroglycerin. Using a perfusion-limited pharmacokinetic model, the equation for the calculation of the fraction (F)of the dose of nitroglycerin systemically available from skin was derived independent of nitroglycerin plasma concentrations. Estimated F values (0.68-0.76) are comparable to values reported in Rhesus monkeys (0.80-0.84). Simulated plasma concentration-time profiles were reasonably fitted to the observed concentrations of nitroglycerin and its ...
We have demonstrated for the first time that not only the brachial artery but also retinal and intrarenal hemodynamic responses to an intravenous infusion of l-arginine are impaired in type 2 diabetic patients. Despite no significant difference in the vascular responses of the retinal and brachial arteries after the nitroglycerin administration between the patient and control groups, there were significant differences in the vascular responses of these arteries in both the normoalbuminuric and microalbuminuric patients compared with the control subjects after the l-arginine infusion, suggesting that the endothelium-dependent vascular responses in the retinal and brachial arteries are impaired but that the responsiveness of the smooth muscle to exogenous NO is not impaired in diabetic patients. On the other hand, the reduction in vascular resistance of the interlobar artery after the nitroglycerin administration was significantly lower in the microalbuminuric patients than in the control ...
Treatment of angina with nitroglycerin is more than 100 years old, but recent advances promise to revolutionize its use (1). With intravenous, long-acting transdermal and long-acting transmucosal nitroglycerin recently available, we stand on the threshold of an era of rational, effective nitroglycerin prophylaxis. To provide real benefits to patients, clinicians will have to use nitroglycerin more critically and systematically than in the past.. The basic action of nitroglycerin is relaxation of vascular smooth muscle (2). The molecular mechanism of this action remains unclear. Although calcium flux may be altered, nitroglycerin is not similar in mechanism to nifedipine, verapamil, and diltiazem ...

For more than 130 years, doctors have prescribed nitroglycerin for relief of chest pain without a clear knowledge of how it actually worked. Now, not only have researchers from Howard Hughes Medical Institute (HHMI) and Duke University Medical Center solved this age-old riddle, they have also shed light on the second major mystery surrounding nitroglycerin, why patients eventually develop a tolerance to the drugs effects.. Shortly after heart patients take nitroglycerin, the blood vessels supplying the heart muscle relax, allowing oxygen-rich blood to nourish the heart and relieve the pain. While it is known that nitric oxide, a breakdown product of nitroglycerin, plays a critical role regulating blood vessel relaxation, scientists still did not know the mechanism by which nitric oxide is generated from the nitroglycerin molecule, which in fact shows little resemblance to nitric oxide.. The research team led by Jonathan Stamler, M.D., HHMI investigator at Duke, found an enzyme that not only ...

Introduction: While nitroglycerin (NTG) is often used in the treatment of CHF it is not formally approved for that purpose. Whether addition of low dose NTG is of value in patients already on guideline-based therapy (ACE inhibitors, beta-blockers, diuretics) is not known, nor is it known whether response to NTG added to these medicines varies with severity of left ventricular dysfunction.. Methods: Exercise capacity by six-minute walk test (6MWT), quality of life by the Minnesota Living with Heart Failure Questionnaire (HFQ), and ejection duration (ED) by Sphygmocor© were measured at baseline and after a 4 week course of transdermal NTG, given at 0.2 mg/hr for 12 hours daily. We studied 45 NYHA Class II and III patients (84% male, 53.4 ± 9.1 yrs, 42% with CAD, CHF duration 3 ± 2.6 yrs, LVEF ≤40%) on stable, standard, guideline-based CHF therapy. ED was defined in msec from the foot to the incisura of the applanation tonometry derived radial artery pressure waveform.. Results: Subjects were ...

Pain prior to and at two minutes and five minutes after ...
Nitroglycerin is rapidly absorbed following lingual spray administration. In a pharmacokinetic study when a single 1200 mcg dose (three activations of a 400 mcg dose) of NitroMist was administered to healthy volunteers (n=12), all subjects had detectable trinitroglycerin plasma levels (mean Cmax 0.8 ± 0.7 ng/mL and tmax of 8 minutes, range 4 to 15 minutes) beginning at 2 minutes post-dose and higher levels of the 1,2- (mean Cmax 3.7 ± 1 ng/mL and tmax 34 ± 21 minutes, range 15 to 90 minutes) and 1,3-dinitroglycerin metabolites (mean Cmax 1 ± 0.3 ng/mL and mean tmax 41 ± 20 minutes, range 20 to 90 minutes). The volume of distribution of nitroglycerin following intravenous administration is 3.3 L/kg. A liver reductase enzyme is of primary importance in the metabolism of nitroglycerin to glycerol di- and mononitrate metabolites and ultimately to glycerol and organic nitrate. Known sites of extrahepatic metabolism include red blood cells and vascular walls. In addition to nitroglycerin, 2 major ...
The ill effects of nitroglycerin overdose are generally the results of nitroglycerins capacity to induce vasodilatation, venous pooling, reduced cardiac output, and hypotension. These hemodynamic changes may have protean manifestations, including increased intracranial pressure, with any or all of persistent throbbing headache, confusion, and moderate fever; vertigo; palpitations; visual disturbances; nausea and vomiting (possibly with colic and even bloody diarrhea); syncope (especially in the upright posture); air hunger and dyspnea, later followed by reduced ventilatory effort; diaphoresis, with the skin either flushed or cold and clammy; heart block and bradycardia; paralysis; coma; seizures; and death.. Laboratory determinations of serum levels of nitroglycerin and its metabolites are not widely available, and such determinations have, in any event, no established role in the management of nitroglycerin overdose.. No data are available to suggest physiological maneuvers (e.g., maneuvers to ...

To study the effects of nitroglycerin on the HIF-1 pathway and P-gp regulation in cancer cells in tumor tissues, we studied the rates of immunoreactive cells regarding HIF-1α, P-gp, and VEGF in the tissues by immunohistochemical assessment in 17 patients with operable lung adenocarcinoma and angina pectoris in study A. First, we compared the rates of immunoreactive cells for HIF-1α, P-gp, and VEGF proteins in tumor tissues between the nitroglycerin and control groups before operation. Furthermore, we studied the relationship between the rate of immunoreactive cells for VEGF protein and that for HIF-1α protein and the relationship between the rate of immunoreactive cells for VEGF protein and that for P-gp protein in cancer tissues. Second, a prospective trial was designed to evaluate the effects of nitroglycerin on the plasma VEGF levels which are regulated by HIF-1 (40), and the relationship between changes in plasma VEGF levels by nitroglycerin treatment for 3 days before chemotherapy and ...
Twenty-six women with SLE and a history of CVD (defined as a history of objectively verified angina pectoris, myocardial infarction, cerebral infarction or intermittent claudication; age 52 ± 8.2 years) were compared with age-matched SLE women without CVD (SLE controls) and population-based age-matched control women (population controls). Flow-mediated dilatation (FMD) of the Brachial artery after reactive hyperemia and after sublingual nitroglycerine administration was performed on women who were not already on nitro-related medication. Levels of thrombomodulin, tumour necrosis factor (TNF) alpha, sTNF receptors, homocysteine and fibrinogen have been associated with endothelial activation/inflammation and were measured in the circulation by use of the ELISA technique. ...
Cardiac surgery requires periods of therapeutic hypotension. Anesthesia providers often administer intravenous (IV) nitroglycerin to accomplish these meticulous hemodynamic goals. However, patients occasionally manifest adverse responses to nitroglycerin such as decreased stroke volume (SV), decreased cardiac output (CO), decreased renal, splanchnic, cerebral perfusion, and tolerance. In these situations, an alternative medication is needed that is just as effective and safe for the patient. Clevidipine is currently the best choice for this because it has rapid onset and termination of action, and it has arterial specific effects. Multiple studies show that clevidipine is as effective and safe as nitroglycerin.. ...

The pharmacokinetics and pharmacodynamics of nitroglycerin and aspirin have been evaluated with emphasis being placed on the in vitro and in vivo metabolism of these drugs in various tissues of the body.. Nitroglycerin (GTN) was metabolized by blood components to the dinitro- (GDN) and in turn mononitroglycerin (GMN) metabolites. The rate of GTN and GDN metabolism was concentration dependent. The rate of GTN metabolism was reduced in the presence of GDNs and prevented by iodoacetamide which was used to stabilize GTN in blood samples obtained for pharmacokinetic studies.. Intravenous (IV) bolus doses of GTN administered to sheep evoked rapid and substantial changes in haemodynamic parameters in a dose-dependent manner. The magnitude of the responses was related to the peak GTN femoral arterial A or V plasma concentration. Large differences in peak GTN concentrations and area under the plasma-time curves (AUC) for A and V were observed. The extent of this femoral A-V gradient, and of gradients ...

Several cardiovascular risk factors are associated with an increased incidence of Alzheimers disease (AD); however, the mechanistic link is unknown. A common feature of cardiovascular risk factors is endothelial dysfunction, specifically, loss of bioavailability of endothelial nitric oxide (NO). Recently, we demonstrated that loss of endothelial NO led to increased protein levels of amyloid precursor protein (APP), β-site APP cleaving enzyme 1 (BACE1) and beta amyloid (Aβ) peptide. APP, the parent protein, when sequentially cleaved by BACE1 and γ-secretase generates amyloidogenic, Aβ, the primary component of AD plaques. Our data was the first to demonstrate a possible mechanism linking endothelial dysfunction, via loss of endothelial NO, with the upregulation of important AD related proteins in the cerebral microvasculature. Therefore, we sought to determine if supplementation of cultured human brain microvascular endothelial cells with NO, by utilizing the NO donor, DETA NONOate, would ...
The sustained coronary vasodilator effect of intrapericardial delivery of nitroglycerin suggests that the drug is contained within the pericardial space, prolonging the time of action and exposure to perivascular tissue, and is not diffused rapidly into the systemic circulation. In contrast, intracoronary or intravenous administration of nitroglycerin is limited by a relatively short duration of action, with minimal or no organ selectivity, and is associated with systemic effects such as hypotension. These limitations may be particularly important when considering administration of vascular or myocardial growth factors, viral vectors or other proteins to induce angiogenesis or improve contractility, whose possible side effects include intimal hyperplasia, mitogenesis and systemic toxicity (12-15). For such agents, exposure to myocardial tissue needs to be maximized while minimizing availability to other organs. Lazarous and coworkers (16)demonstrated that intrapericardial administration of basic ...
Nitroglycerin is, at typical doses, a VENOUS dilator. By decreasing venous return to the heart (preload) nitroglycerin decreases the filling pressure of the heart, decreasing myocardial wall tension, force of contraction, and by extension, myocardial workload and oxygen demand of the stressed tissue. Nitroglycerin only minor effect on arteries or arterioles at usual doses, although a high dose drip (greater than 40mcg/min) can have artery dilating effects. So, nitro does not dilate atherosclerotic arteries to allow blood to flow around any clots or occlusions in the arterial system of the heart. When nitroglycerin has been injected directly into a blocked coronary artery, it has not relieved pain! In other words, nitroglycerin eases coronary chest pain by reducing the hearts oxygen needs, NOT by providing more oxygen supply. Although blood pressure can drop with nitroglycerin, it can be minimized with the patient lying down ...
Nitroglycerin - Nitroglycerin is used in the treatment of heart conditions such as angina (chest pain) and chronic heart failure.
Nitroglycerin relieves chest pain caused by a lack of blood to the heart (angina) by getting more oxygen-rich blood to your heart. Long-acting nitroglycerin helps prevent angina.
