Tiagabine (trade name Gabitril) is an anticonvulsant medication used in the treatment of epilepsy that is produced by Cephalon. The drug is also used off-label in the treatment of anxiety disorders and panic disorder. Tiagabine is approved by U.S. Food and Drug Administration (FDA) as an adjunctive treatment for partial seizures in individuals of age 12 and up. It may also be prescribed off-label by physicians to treat anxiety disorders and panic disorder as well as neuropathic pain (including fibromyalgia). For anxiety and neuropathic pain, tiagabine is used primarily to augment other treatments. Tiagabine may be used alongside selective serotonin reuptake inhibitors, serotonin-norepinephrine reuptake inhibitors, or benzodiazepines for anxiety, or antidepressants, gabapentin, other anticonvulsants, or opioids for neuropathic pain. The most common side effect of tiagabine is dizziness. Other side effects that have been observed with a rate of statistical significance relative to placebo include ...

For over 30 years, methadone has been used to treat opioid addiction. Since methadone is effective in reducing withdrawal symptoms, it is used as a method of detoxification for opiate addicts. However, methadone is not effective in treating other drugs of abuse, such as cocaine. Tiagabine is a drug that enhances levels of gamma aminobutyric acid (GABA), a chemical found in the brain and spinal cord. The objective of this study is to determine the effectiveness of tiagabine in modifying cocaine-using behavior and reducing opiate withdrawal symptoms among newly admitted methadone-treated patients.. This 16-week, double-blind, placebo-controlled clinical trial will involve 120 participants who are both cocaine- and opioid- dependent. Participants will be randomly assigned to receive either tiagabine or placebo, while concurrently receiving methadone treatment. Baseline cocaine use will be determined during the first two weeks of treatment. The study will include three overlapping phases. The first ...

Melchior Hoffman (Hofmann, Hofman), the noted early Anabaptist leader, was born about 1495 at Schwäbisch-Hall. He received a good elementary education and learned the furriers trade. He was early interested in religious literature, especially the writings of the mystics, Tauler, Deutsche Theologie, etc. He also acquired a thorough knowledge of the Bible. The writings of Martin Luther moved him deeply and he became Luthers zealous follower.. In 1523 business took him to Livland; here he was soon drawn into the religious movement of the Reformation. In the shortage of Protestant preachers, Hoffman entered this service and preached at Wolmar until persecution caused his imprisonment and expulsion from the country. Nevertheless his influence here must have been lasting, for even from Sweden he wrote admonitions to his Wolmar friends. In the fall of 1524 he came to Dorpat where he preached against the use of images. This led to a fateful wave of iconoclasm (10 January 1525), in which he, however, ...
GABA transporters belong to a large family of neurotransmitter:sodium symporters. They are widely expressed throughout the brain, with different levels of expression in different brain regions. GABA transporters are present in neurons and in astrocytes and their activity is crucial to regulate the extracellular concentration of GABA under basal conditions and during ongoing synaptic events. Numerous efforts have been devoted to determine the structural and functional properties of GABA transporters. There is also evidence that the expression of GABA transporters on the cell membrane and their lateral mobility can be modulated by different intracellular signaling cascades. The strength of individual synaptic contacts and the activity of entire neuronal networks may be finely tuned by altering the density, distribution and diffusion rate of GABA transporters within the cell membrane. These findings are intriguing because they suggest the existence of complex regulatory systems that control the plasticity

We retrospectively analysed the clinical characteristics, electroencephalogram (EEG) records, brain magnetic resonance imaging (MRI) scans, antiepileptic therapy and prognosis of a case series with ambulatory non-convulsive status epilepticus (NCSE) which evolved into a malignant form (mNCSE). mNCSE was defined as NCSE that continues or recurs five days or more after the onset of general anaesthesia, including those cases where NCSE recurs upon reduction or withdrawal of anaesthetic therapy. Four women and two men were studied. The mean age was 42.8 years (range: 19-63 years). Two of six patients had a previous diagnosis of epilepsy. Four patients died. Two patients had a good outcome with return to consciousness and activities of daily life, and one of these subjects developed temporal lobe epilepsy. In four patients, the cause of mNCSE was encephalitis. Clinical presentation was complex partial status epilepticus (CPSE) in three patients and generalised NCSE (GNCSE) in the other three. Two of ...
TY - JOUR. T1 - Localization and expression of GABA transporters in the suprachiasmatic nucleus. AU - Moldavan, Michael. AU - Cravetchi, Olga. AU - Williams, Melissa. AU - Irwin, Robert P.. AU - Aicher, Sue. AU - Allen, Charles. PY - 2015. Y1 - 2015. N2 - GABA is a principal neurotransmitter in the suprachiasmatic hypothalamic nucleus (SCN), the master circadian clock. Despite the importance of GABA and GABA uptake for functioning of the circadian pacemaker, the localization and expression of GABA transporters (GATs) in the SCN has not been investigated. The present studies used Western blot analysis, immunohistochemistry and electron microscopy to demonstrate the presence of GABA transporter 1 (GAT1) and GAT3 in the SCN. By using light microscopy, GAT1 and GAT3 were co-localized throughout the SCN, but were not expressed in the perikarya of arginine vasopressin- or vasoactive intestinal peptide-immunoreactive (-ir) neurons of adult rats, nor in the neuronal processes labelled with the ...

Were still a month away from the thick of the flurry of deals that always occurs in advance of the July 31 non-waiver trade deadline. Right now, teams are still focused on getting their draft picks signed, a process that will finish two weeks before the deadline, thanks to the new Collective Bargaining Agreement. Because the Phillies are 9.5 games out of first place and four games under .500 and trailing a slew of teams for the two Wild Card spots, the possibility exists that they end up dealing away one or more of their attractive pieces in order to stockpile for the future. The bottom would have to completely fall out for that to happen, but the bottom falling out isnt out of the question. - David Murphy, Philadelphia Daily News

Gamma-aminobutyric acid (GABA) is the major inhibitory neurotransmitter in the mammalian brain. Once released, it is removed from the extracellular space by cellular uptake catalyzed by GABA transporter proteins. Four GABA transporters (GAT1, GAT2, GAT3 and BGT1) have been identified. Inhibition of the GAT1 by the clinically available anti-epileptic drug tiagabine has been an effective strategy for the treatment of some patients with partial seizures. Recently, the investigational drug EF1502, which inhibits both GAT1 and BGT1, was found to exert an anti-convulsant action synergistic to that of tiagabine, supposedly due to inhibition of BGT1. The present study addresses the role of BGT1 in seizure control and the effect of EF1502 by developing and exploring a new mouse line lacking exons 3-5 of the BGT1 (slc6a12) gene. The deletion of this sequence abolishes the expression of BGT1 mRNA. However, homozygous BGT1-deficient mice have normal development and show seizure susceptibility ...
Non-convulsive status epilepticus (NCSE) is present in 10-30% of ICU patients with altered mental status (AMS) and is associated to poor outcomes. To our knowledge, there is no data describing the prevalence and outcomes of critically ill cancer patients with AMS associated to non-convulsive seizures (NCS) or NCSE. We aim to describe the outcomes and risk factors of critically ill cancer patients with encephalopathy associated with non-convulsive seizures (NCS). This is a 3-year prospective observational study in a mixed oncological ICU at MD Anderson Cancer Center. Data of ICU patients with moderate to severe encephalopathy (Glasgow Coma Score | 13) that underwent EEG monitoring to rule out NCS were collected. Multivariate logistic regression was performed to identify risk factors and outcomes. Of the 317 patients with encephalopathy who underwent EEG monitoring, 14.5% had NCS. Known risk factors such as sepsis, CNS infection, antibiotics, and cardiac arrest were not associated with increased risk of

The aim of the proposed research is to compare the diagnostic accuracy of a portable wireless electroencephalography (EEG) device (Biosignal Micro-EEG) to standard EEG in identifying abnormal EEG patterns (mainly non-convulsive seizure and non-convulsive status epilepticus) in emergency department (ED) patients with altered mental status. Comparing the the accuracy of EEG recordings and interpretations of Micro-EEG to those of standard EEG will allow the investigators to assess the utility of this novel device in the ED patients with altered mental status. The unique qualities of Micro-EEG device could potentially facilitate easier access to EEG test in all ED patients.. This study will also provide valid information regarding the prevalence of non-convulsive seizure in ED patients with altered mental status. The gold standard for diagnosing non-convulsive seizure would be standard EEG.. All study participants will undergo electroencephalography using the two devices (standard EEG and micro-EEG) ...
As is the case with other non-convulsive status epilepticus forms, CPSE is dangerously underdiagnosed.[3] This is due to the potentially fatal yet veiled nature of the symptoms. Usually, an electroencephalogram, or EEG, is needed to confirm a neurologists suspicions. The EEG is also needed to differentiate between absence status epilepticus (which affects the entire brain), and CPSE, which only affects one region.[4] ...

Since the observation that malformed brain structure is associated with intractable forms of epilepsy, there has been a great deal of interest in trying to understand the function of dysplastic neurons. Here we performed experiments to investigate the influence of GABAergic inhibition on hippocampal heterotopic neurons in an animal model of malformation-associated epilepsy, e.g., rats exposed to MAM in utero. Our main findings in these animals include the following: (1) an alteration in the decay kinetics of evoked and spontaneous IPSCs recorded on heterotopic neurons, (2) "normal" inhibitory responses for heterotopic neurons after exogenous GABA application, (3) an inability to alter IPSC decay kinetics when heterotopic neurons are exposed to GABA transport inhibitors, and (4) a low level of GAT expression in heterotopic cell regions. Together, these results suggest altered inhibitory synaptic function at heterotopic synapses in the MAM model.. Abnormal electrical discharges, the hallmark of ...
