The purpose of this large-animal study was to assess the safety and effects of negative pressure therapy (NPT) when used as temporary abdominal closure in the immediate post-decompression period after abdominal compartment syndrome (ACS). Using a hemorrhagic shock/resuscitation and mesenteric venous pressure elevation model, ACS was physiologically induced in 12 female Yorkshire swine. At decompression, animals were allocated to either NPT (n = 6) or Bogota bag (n = 6) as temporary abdominal closure and studied for a period of 48 h or until death. Outcomes measured included morbidity and mortality, as well as hemodynamic parameters, ventilator-related measurements, blood gases, coagulation factors, and organ (liver, kidney, lung, and intestinal) edema and histology at the time of death/sacrifice. In one embodiment, the method provides hollow fiber catheters for the removal of fluid or fluid components from the wound area by ultrafiltration. In another embodiment the hollow fiber catheters are connected to an infusion pump and used to deliver therapeutic fluids to the wound area. In yet another embodiment, a first set of hollow fiber catheters is connected to a vacuum pump and a second set is connected to an infusion pump to simultaneously remove excess tissue fluid and deliver therapeutic fluid to the wound site.

BACKGROUND: We see a high volume of trauma in our department. Massive delays in presentation of patients with acute abdomen are common. Closure at initial laparotomy is not possible in many cases, either because the patient has or will develop abdominal compartment syndrome, or because several re-look laparotomies will be required. A significant proportion of our patients who have undergone laparotomy therefore spend some of their stay in hospital with an open abdomen. The management of these patients is particularly labour intensive for nursing staff. The Opsite sandwich or Bogota bag invariably leaks, and sometimes needs changing daily. Introduction. Management of open abdomen (OA) with enteroatmospheric fistula (EAF) in morbid obese patient with comorbid disease is challenging. We would like to report the management of septic OA in morbid obese patient with EAF which developed after strangulated recurrent giant incisional hernia repair. We would also like to emphasize, in this case, the conversion of EAF to ileostomy by the help of second Negative Pressure Therapy (NPT) on ostomy side, and the chance of new EAF occurrence was reduced with intrarectal NPT. Case Presentation. 62-year-old morbid obese woman became an OA patient with EAF after strangulated recurrent giant hernia. EAF was converted to ostomy with pezzer drain by the help of second NPT on ostomy. Colonic distention was reduced with the third NPT application via rectum. A 72-year-old woman presented with concern for a necrotising soft tissue infection (NSTI) 6 days after a tree branch impaled her left lower extremity while hiking in Hawaii. The wound was irrigated and closed at a local clinic in Hawaii. She completed a 5-day course of clindamycin. She presented to our emergency department 1 day after completion of antibiotics due to worsening erythema and malodorous drainage. Local wound exploration revealed bullae and easy dissection of fascial planes. CT scan revealed complex heterogeneous fluid and inflammatory stranding in the posterior calf. Clinical and radiographic findings raised concern for NSTI prompting initiation of broad spectrum antibiotics and urgent operative debridement. Wound cultures and deep tissues cultures returned positive for pansusceptible Leclercia adecarboxylata. Introduction: Fasciitis of the limb and toxic-shock syndrome due to Streptococcuspyogenes infection is considered a surgical emergency. This condition may occur in previously healthy patients. It is a life-threateningcondition with high morbidity rates when treatment is delayed due to poor diagnosis and non-surgical intervention. Case Report: 17-year-old male patient sustained a minor ankle trauma while skating. Initials findings were a 1cm wound in relation with his left lateral malleolus and moderate swelling. About 6 h later, the patient was admitted at the emergency room with disproportionate limb pain, increased swelling, persistent fever, paleness, tachycardia, and hypotension. The patient was intubated due to his unstable hemodynamic condition and underwent fasciotomy of the limb. A vacuum-assisted closure device was used. After the first surgical procedure, patients condition radically improved. He was extubated with good clinical prognosis. He underwent several surgical procedures in a 3-weeks
Results Ten patients (8 male, median age 59 years) underwent Endo-Sponge therapy. Seven were treated for anastomotic leaks following low anterior resection. Two patients had symptomatic low pelvic cavities following ileal pouch excision and 1 was treated for a pelvic cavity following a perforated low Hartmanns stump. Seven patients were treated within 17 days (0-17 days) of diagnosis of pelvic sepsis. The remaining 3 patients were treated for chronic low pelvic cavities at intervals ranging from 5 months following index surgery. Median duration of treatment was 28.5 days (8-40 days) with a median number of sponge changes of 7 (2-11 changes). One patient developed a transient inflammatory response and pneumoperitoneum which settled with antibiotic therapy but there were no other complications.
