OBJECTIVE To evaluate the possible role of tight junction protein Occludin in nasal polyps. METHODS The expression of Claudin-1, Occludin and ZO-1 in nasal polyps (n = 20) and healthy uncinate mucosa (n = 15) were examined using immunohistochemical staining, real-time quantitative polymerase chain reaction (PCR) and Western blot analysis. The regulatory effects of proinflammatory cytokines (IFN-γ, IL-13, IL-17, TGF-β, TGF-α) on the expression of Occludin in cultured human nasal epithelial cells were investigated. RESULTS The immunohistochemical results showed that Claudin-1, Occludin and ZO-1 were detected both in the nasal polyp group and the control group. The expression sites were the cell membrane and cytoplasm of nasal mucosa epithelial cells. The mean optical density of Claudin-1, Occludin and ZO-1 were 0.187 ± 0.076,0.172 ± 0.109 and 0.098 ± 0.035 respectively in the nasal polyp group and were significantly lower than those in the control group (0.312 ± 0.101, 0.220 ± 0.069 and 0.233

article{1188623, abstract = {Chronic rhinosinusitis with nasal polyps (CRSwNP) is characterized by biased Th2 inflammation and CRS without nasal polyps (CRSsNP) by a Th1 immune response. Colonization by Staphylococcus aureus is increased in CRSwNP. We aimed to determine macrophage phenotypes in nasal mucosa of CRSwNP and CRSsNP and to examine phagocytosis of S. aureus in these pathologies. Methods: Macrophage phenotyping was performed by immunohistochemical staining on nasal mucosa sections from 28 patients; in addition flow cytometry analysis was performed. Tissue homogenate protein levels of IFN-gamma, IL-5, IL-6, IL-1 beta, TGF-beta, eosinophil cationic protein (ECP) and total IgE were analyzed and correlated with macrophage subtypes. Phagocytosis of S. aureus was analyzed by flow cytometry. Survival of S. aureus in Thp1 cells in the presence of polarizing cytokines was studied in vitro. Results: By immunohistochemical analysis more M2 macrophages were present in CRSwNP than in CRSsNP. This ...

Are you experiencing nasal blockage and difficulty breathing? You could be suffering from nasal polyps! While most are familiar with polyps that occur in the colon, polyps can appear in the stomach, uterus, and nasal cavities. Although they are concerning, nasal polyps are almost always benign. Despite this, individuals with nasal polyps should seek treatment from an otolaryngologist immediately, as they affect ones lifestyle and can bring about infection. Typical symptoms of nasal polyps include stuffy nose, postnasal drainage, nasal obstruction, and decrease or complete loss of smell. Continue reading →. ...
We describe a long-range optical coherence tomography system for size and shape measurement of large hollow organs in the human body. The system employs a frequency-domain optical delay line of a configuration that enables the combination of high-speed operation with long scan range. We compare the achievable maximum delay of several delay line configurations, and identify the configurations with the greatest delay range. We demonstrate the use of one such long-range delay line in a catheter-based optical coherence tomography system and present profiles of the human upper airway and esophagus in vivo with a radial scan range of 26 millimeters. Such quantitative upper airway profiling should prove valuable in investigating the pathophysiology of airway collapse during sleep (obstructive sleep apnea).. ©2003 Optical Society of America. The purpose of this research study is to better understand how this potential treatment option, oral steroids, affects biochemical substances that have been associated with the development of chronic sinusitis and polyps. In order to do this, we need to study people with different forms of chronic sinusitis and compare them to individuals without allergies or sinus disease. We will also look at patients with chronic sinusitis who are treated with oral steroids and compare them to chronic sinus patients who have not received oral steroid therapy prior to surgery. This study may help pave the way to new treatments that address specific parts of the chronic sinus inflammatory pathway.. Hypotheses. ...
Results from a study by Park et al indicate that [6]-Gingerol, from Ginger, decreased the production of reactive oxygen species in Nasal polyp fibroblasts. Myofibroblast differentiation, collagen production, and other key biochemical processes were prevented by the antioxidant activity of [6]-gingerol. These data suggest the potential of [6]-gingerol in inhibiting the production of extracellular matrix…
BACKGROUND: Nasal polyposis is a chronic inflammatory disease of the upper respiratory tract that affects around 2% of the population and almost 67% of patients with aspirin-intolerant asthma. Polyps are rich in mast cells and eosinophils, resulting in high levels of the proinflammatory cysteinyl leukotrienes. OBJECTIVES: To better understand the role of the proinflammatory leukotrienes in nasal polyposis, we asked the following questions: (1) How do nasal polyps produce leukotriene C(4) (LTC(4))? (2) Can LTC(4) feed back in a paracrine way to maintain mast cell activation? (3) Could a combination therapy targeting the elements of this feed-forward loop provide a novel therapy for allergic disease? METHODS: We have used immunohistochemistry, enzyme immunoassay, and cytoplasmic calcium ion (Ca(2+)) imaging to address these questions on cultured and acutely isolated human mast cells from patients with polyposis. RESULTS: Ca(2+) entry through store-operated Ca(2+) release-activated Ca(2+) (CRAC) channels
The clinical trial population consists of patients presenting with two or more typical symptoms of CRS and duration of symptoms of at least ≥12 weeks. The diagnosis should have been confirmed with the initial diagnosis of CRS by nasal endoscopy and/or computer tomography (CT) and by obligatory nasal endoscopy including exclusion of nasal polyps during screening phase.. A total of 885 patients will be included in this clinical trial, which will last for approximately 12 months, including screening, treatment and medication-free follow-up period as well as final reporting. The duration for the individual patient is approximately 22 weeks (Visit 1 - Visit 7). After a 2-week screening phase, treatment will be started by application of either 240 mg or 480 mg daily dose (t.i.d.) or placebo for 12 weeks followed by a medication-free follow-up period.. Symptoms often do not correlate to the findings in nasal endoscopy or CT. Therefore, efficacy in this clinical trial is defined as per clinical ...
|b|I am suffering with chronic sinusitis and polyps|/b|. I am not able to breathe freely due to the polyps. The sinusitis causes the bacterial or fungal discharge. I got operated for polyp removal once but I was relieved from the blockage for only 2-3 months. I use Ortrivin (Xylometazoline) 0.1% daily 4-5 times, even then I face problem in breathing. I have lost my sense of smell. When the infection is high I resort to Itraconazole 100mg and Ciprofloxacin 500mg with Tenidazole 600mg along with Xylometazoline. I tried all other conventinal methods like ayurveda, homeopathy and others but none was able to relieved me. I even have slight DNS. What can be done to atleast breath uninterruptedly?

Rarely, masses such as nasal polyps and even cancer may cause symptoms of nasal obstruction. As such, an initial important part of any examination investigating a persons nasal obstruction is nasal endoscopy. Nasal endoscopy enables the physician to look into the back half of the nose which is not able to be seen by just looking into the nose with a flashlight. Beyond the examination, an exact description of a persons complaints is helpful to determine what type of treatment whether medical or surgical would be most beneficial. The most common scenarios are described below:. If the patient is a child younger than 5 years old,. I cant breathe through my nose mainly one side.. ...

A. Nasal endoscopy offers the physician specialist a reliable, visual view of all the accessible areas of the sinus drainage pathways. First, the patient s nasal cavity is anesthetized; a rigid or flexible endoscope is then placed in a position to view the nasal cavity. The procedure is utilized to observe signs of obstruction as well as detect nasal polyps hidden from routine nasal examination. During the endoscopic examination, the physician specialist also looks for pus as well as polyp formation and structural abnormalities that may cause recurrent sinusitis.. ...

In airways, a proliferative effect is played directly by cholinergic agonists through nicotinic and muscarinic receptors activation. How tumors respond to aberrantly activated cholinergic signalling is a key question in smoking-related cancer. This research was addressed to explore a possible link of cholinergic signalling changes with cancer biology. Fifty-seven paired pieces of head and neck squamous cell carcinoma (HNSCC) and adjacent non-cancerous tissue (ANCT) were compared for their mRNA levels for ACh-related proteins and ACh-hydrolyzing activity. The measurement in ANCT of acetylcholinesterase (AChE) and butyrylcholinesterase (BChE) activities (5.416 ± 0.501 mU/mg protein and 6.350 ± 0.599 mU/mg protein, respectively) demonstrated that upper respiratory tract is capable of controlling the availability of ACh. In HNSCC, AChE and BChE activities dropped to 3.584 ± 0.599 mU/mg protein (p = 0.002) and 3.965 ± 0.423 mU/mg protein (p | 0.001). Moreover, tumours with low AChE activity and high BChE

Background A subgroup of patients with severe asthma have persistent eosinophilic airway inflammation despite treatment with high intensity corticosteroid treatment. One possible explanation for this pattern of disease is involvement of a type 2 innate lymphoid cells (ILC2s) dependant and relatively corticosteroid resistant pathway generating type 2 cytokines such as IL-5 and Il-13. The presence of high numbers of ILC2s in the nasal polyps commonly associated with severe eosinophilic asthma supports this view. We have carried out a cross-sectional study testing the hypothesis that ILC2 counts are increased in peripheral blood of patients with severe eosinophilic asthma. ...

There are two major ways that ENT allergy treatment varies from traditional methods: first, the testing is done for quantitative as well as qualitative information about inhalant sensitivities. This allows a patients starting allergy shots to be mixed at the strongest dose possible for each antigen without compromising safety. Because of this, patients usually experience symptom relief faster, and only require one shot per week rather than two. Additionally, patients with lower level skin reactions still can attain significant relief of their symptoms (patients that usually are not offered shots in a traditional allergy setting.) These treatments can also be combined with surgical treatment of obstructing nasal polyps or other anatomic causes of upper respiratory disease.. The second major difference is the testing and treatment of non-IgE mediated food sensitivities. In contrast to anaphylactic shock type reactions, cyclic food allergy (or food intolerance) to commonly ingested foods can cause ...

Review question We reviewed the evidence for the benefits and harms of systemic (given by mouth) or topical (given by nose) antibiotics for people with chronic rhinosinusitis.. Background Chronic rhinosinusitis is a common condition that is defined as inflammation of the nose and paranasal sinuses (a group of air-filled spaces behind the nose, eyes and cheeks). Patients experience at least two or more of the following symptoms for at least 12 weeks: blocked nose, discharge from their nose or runny nose, pain or pressure in their face and/or a reduced sense of smell (hyposmia). Some people will also have nasal polyps, which are grape-like swellings of the normal nasal lining inside the nasal passage and sinuses.. Study characteristics We included five randomised controlled trials (RCTs) with a total of 293 participants. The studies were small (43 to 79 participants). Four recruited adults and the fifth children. Three studies only included people with chronic rhinosinusitis without nasal polyps, ...
Eosinophilic chronic rhinosinusitis (ECRS), a subgroup of chronic rhinosinusitis with nasal polyps, is recognized as a refractory eosinophilic disorder characterized by both upper and lower airway inflammation. In some severe cases, disease control is poor, likely due to local steroid insensitivity. In this study, we focused on protein phosphatase 2A (PP2A), a key factor regulating glucocorticoid receptor (GR) nuclear translocation, and examined its association with local responses to corticosteroids in eosinophilic airway inflammation. Our results indicated reduced responses to corticosteroids in nasal epithelial cells from ECRS patients with asthma, which were also associated with decreased PP2A mRNA expression. Eosinophil peroxidase stimulates elevated PP2A phosphorylation levels, reducing PP2A protein expression and activity. In addition, mRNA levels of inflammatory mediators (TSLP, IL-25, IL-33, CCL4, CCL5, CCL11, and CCL26) associated with eosinophilic airway inflammation in epithelial cells were

BACKGROUND: Topical corticosteroid is used as part of a comprehensive medical treatment for chronic rhinosinusitis (CRS) without polyps. Nevertheless, there is insufficient evidence to show a clear overall benefit. Trials studying the efficacy of topical corticosteroid use various delivery methods in patients who have or have not had sinus surgery, which directly impacts on topical delivery and distribution.. OBJECTIVES: To assess the effects of topical steroid in patients with CRS without nasal polyps and perform a meta-analysis of symptom improvement data, including subgroup analysis by sinus surgery status and topical delivery methods.. SEARCH STRATEGY: We searched the Cochrane Ear, Nose and Throat Disorders Group Trials Register; the Cochrane Central Register of Controlled Trials (CENTRAL); PubMed; EMBASE; CINAHL; Web of Science; BIOSIS Previews; Cambridge Scientific Abstracts; ISRCTN and additional sources for published and unpublished trials. In ARS, it is common to find high IL-8 (which is the neutrophils main chemotactic agent), TNF-alpha and total protein levels. Neutrophil and T lymphocyte levels are also usually increased (both in the epithelium and in the lamina propria). There is more information available on the existence of inflammation in chronic rhinosinusitis (CRS) with nasal polyposis. Among other findings, there is one that is particularly interesting: the persistence of high eosinophil levels. Numerous inflammation mediators have been found in the sinonasal mucosa of subjects with CRS and nasal polyposis which may play an important role and may be caused both by specific substances and by other non-specific factors (cold, pollution,…). Glandular secretion is an important component of inflammation, manifesting as rhinorrhoea, and is regulated not only by inflammatory regulators but also by the epithelial and inflammatory cells themselves and by classic neurotransmitters. A landmark study performed by Roca Ferrer et ...

N2 - Background: The role of microbiota in sinonasal inflammation can be further understood by targeted sampling of healthy and diseased subjects. We compared the microbiota of the middle meatus (MM) and inferior meatus (IM) in healthy, allergic rhinitis (AR), and chronic rhinosinusitis (CRS) subjects to characterize intrasubject, intersubject, and intergroup differences. Methods: Subjects were recruited in the office, and characterized into healthy, AR, and CRS groups. Endoscopically-guided swab samples were obtained from the MM and IM bilaterally. Bacterial microbiota were characterized by sequencing the V3-V4 region of the 16S ...

Verapamil, a calcium channel blocker, has shown significant efficacy in alleviating symptoms of patients with chronic rhinosinusitis (CRS) with nasal polyps.
In total, 96 CRSwNP and 125 CRSsNP patients were included. Eosinophils were found in 78% of CRSwNP patients compared to 42% in CRSsNP. Eosinophilic mucin was observed in 52% of the CRSwNP group versus 20% of the CRSsNP group. Furthermore, secretion analysis revealed FH in 11% CRSwNP patients compared to 3% CRSsNP patients. Recurrence in the total group was 22% over 3 years. CRSwNP patients with eosinophilic involvement showed a recurrence rate of 48%. When the airway mucus secretions were positive for EM and FH the recurrence rate was even 73%. ...
Chronic rhinosinusitis (CRS) is defined as an inflammatory condition involving the paranasal sinuses and linings of the nasal passages, which persists for 12 weeks or longer. CRS can be divided into three subtypes:CRS without nasal polyposis (CRS wit
Data Availability StatementNot applicable. BAFF, GNE-7915 ic50 AID, and germline transcripts had been elevated in CRSwNP sufferers. As opposed to IgA2 amounts, IgA1 amounts had been significantly elevated in the sinonasal tissues of CRSwNP sufferers (Pv? ?0.01). TGF- was elevated in the sinonasal tissues of sufferers with CRSsNP significantly. Conclusions Increased proteins degrees of IgA subclasses and related antibody-producing cells had been associated with raised eosinophils in CRSwNP sufferers which may bring about eosinophil pathological features. Several therapeutic strategies might be created to modulate the IgA creation to ameliorate the inflammatory systems in CRSwNP sufferers. Results 135 polyps ,2 cm were detected in 127 patients with a mean size of 3 cm (spread 2-7 cm). 17.8% (24/135) were thought to be invasive by the endoscopist and all of these were biopsied. 12.5% of these biopsied polyps were found to be adenocarcinoma/carcinoma in situ. 11.9% (16/135) of all polyps were adenocarcinoma.. 31.9% (43/135) of polyps were sessile, and 68.9% (92/135) were pedunculated. 94.1% (127/135) of polyps were left sided. 82.2% (111/135) of polyps were removed at the time of the initial endoscopy. Of these 22.5% were sessile and 77.5% pedunculated. 20% of the sessile polyps had further treatment by means of either surgery (12%,3/3 benign disease) or repeat polypectomy (8%) for residual polyp tissue. In comparison 8.1% of pedunculated polyps went on to have either surgery (5.8%, 4/5 adenocarcinoma) or repeat polypectomy (2.3) for residual polyp tissue.. There were no carcinomas detected in those polyps measuring ,5 cm. However the greatest percentage of adenocarcinomas were ...

Fluticasone propionate belongs to a class of drugs known as corticosteroids, specifically glucocorticoids, which are hormones that predominantly affect the metabolism of carbohydrates and, to a lesser extent, fat and protein. It is used to treat asthma, allergic rhinitis, nasal polyps, various skin disorders and Crohns disease and ulcerative colitis. It is also used to treat eosinophilic esophagitis. Fluticasone is used by powder or aerosol inhalation for the prophylaxis of asthma, the nasal spray is used for prophylaxis and treatment of allergic rhinitis, nasal drops are used in the treatment of nasal polyps, and creams and ointments are applied topically in the treatment of various skin disorders. It can be given orally in the treatment of Crohns disease and ulcerative colitis. Some benefit was also reported in coeliac disease.[citation needed] If taken correctly, the nasal spray and oral inhaler formulation have less corticosteroid side effects than the tablet formulation because they limit ...

In this study, bacterial ball occurred in 2.4% of CRS patients who underwent FESS and it showed lower prevalence than fungal ball or bacterial biofilm. Fungal and bacterial balls are common in CRSsNP and looked very similar, and therefore, we compared two kinds of bioball occurred in CRSsNP. They occurred mainly in the unilateral maxillary sinus of CRSsNP patients. In both, peripheral eosinophils were within the normal range but significantly lower in bacterial ball. This means that bacterial ball is independent of the Th2 response. Moreover, CT scans showed a clear difference between them. Most of fungal ball was accompanied by calcification (88.5%), whereas it was not in bacterial ball (25%). Nevertheless, it was impossible to discriminate between them at the time of surgery. Both were seen as amorphous semisolid materials with green or brown color (Fig. 2). Therefore, all suspected fungal balls should be referred for biopsy. Gram stain will be essential for the diagnosis of bacterial ball ...
If the physician determines a patients loss of smell to be caused by allergies or other inflammatory diseases, he/she may recommend nasal or oral steroids and antihistamines to decrease the inflammation. Surgery may be suggested if the patient is found to have nasal polyps, chronic sinusitis, deviated septum, or other surgically treatable disorders. However, our knowledge in recommending surgery to improve olfaction is limited, as there is no good evidence currently to suggest either surgery or medical management has a better outcome with regard to smelling ability. As mentioned before, despite medical or surgical treatments, certain inflammatory diseases like chronic rhinosinusitis can cause permanent loss, and return of smell to a totally normal state may not be possible. Although there is some controversy, many patients who have obstructive causes for their loss of smell usually do report significant improvements after both medical and surgical treatments. Patients with a permanent loss of ...
Antrochoanal polyps (ACP) are benign polyps that arise from the mucosa of the maxillary sinus, grow into the maxillary sinus and reach the choana. The common presentation of ACP is nasal obstruction.

I am a lifelong sufferer of seasonal allergies. All my childhood photos show a kid with a red nose and puffy eyes, while the rest of the family looks fine. By the time I was in my twenties, I was up to four to six sinus infections a year, along with all the trappings: postnasal drainage; sore, congested throat; itchy eyes; and at least one blocked nostril at all times. Worst of all, it led me to lots of antibiotics, which were really hard on my stomach. I just plain didnt feel well most of the time. I thought this was my lot in life, and something that would never improve. Never say never! By March 2008, it was time for a referral to a specialist.. In my initial examination, Dr. Wyatt explained to me that what I had were nasal polyps, and they could be surgically treated through endoscopic sinus surgery. Naturally the word surgery brings up concerns, but in my initial visit, Dr. Wyatt explained all the long-term benefits of the surgery, as well as the improvements that had been made in the ...

Q: After taking aspirin recently, I began to get asthma symptoms. Ive since read online that some asthmatics are sensitive to aspirin. Can you tell me why? Dr. Bassett: A small number of those with asthma can be adversely affected by aspirin and non-steroidal anti-inflammatory drugs (NSAIDs), such as ibuprofen.. In some cases, the resulting wheezing, shortness of breath and related respiratory distress can be serious, even life-threatening.. There is an association between the following conditions: aspirin intolerance, nasal polyps and/or sinusitis and asthma.. It is essential for your specialist to give you a medical evaluation to determine the best course of coordinated care. Your doctors can help you to identify the names of the various NSAIDs that you should avoid.. An allergist may need to discuss additional therapies (e.g. aspirin desensitization) if its appropriate. That may not only improve the sinus symptoms, but your asthma as well.. Dr. Clifford Bassett, allergist and asthma ...

A discussion of the hypotheses that eosinophils deposit toxic major basic protein (MBP) in the mucus and that MBP reaches concentrations able to damage the sinonasal epithelium
Type 2, or allergic, inflammation in the lung requires the cytokines IL-5, IL-13, and IL-9, which collectively elicit eosinophilia, alternative activation of macrophages, goblet cell hyperplasia, smooth muscle hypercontractility, and tissue remodeling, and may contribute to wound healing (Licona-Limón et al., 2013; Gour and Wills-Karp, 2015). As long-lived tissue-resident cells, group 2 innate lymphoid cells (ILC2s) are a critical early source of these type 2 cytokines before type 2 helper T cell (Th2 cell) recruitment. The role of ILC2s in promoting type 2 lung inflammation has been established in mice using models such as parasitic worm infection, chitin, papain, fungal, or house dust mite challenge, and allergic asthma (Neill et al., 2010; Price et al., 2010; Barlow et al., 2012; Bartemes et al., 2012; Halim et al., 2012; Klein Wolterink et al., 2012; Van Dyken et al., 2014). In humans, ILC2s accumulate in nasal polyps of patients with chronic sinusitis (Mjösberg et al., 2011; Ho et al., ...
Replied on 04/19/2011 Snoring and snorting generally point toward nasal congestion. In animals allergies usually cause itchy skin not increased nasal mucous production or nasal congestion. This congestion can be secondary to a primary bacterial, fungal or viral infection. It can also be caused by a nasal polyp, infected tooth root, foreign body (like a piece of grass or plant awn) in his nose or a cancerous mass. I recommend your dog see his veterinarian for a thorough physical examination and bloodwork. If his teeth are in poor condition and his white blood cell count is very high that may point toward a bacterial infection or a tooth root abscess. If his bloodwork looks fine otherwise your veterinarian can anesthetize him and thoroughly examine his nose and behind his soft palate for foreign bodies, polyps or other tumors. At that time he can clean and examine his teeth and/or take radiographs of his sinuses and nose to look for a reason for his congestion as well.. ...
METHODOLOGY: A prospective cohort study was performed. Patients with CRS received roxithromycin (150 mg) once daily for 12 weeks. Nasal secretions and serology were collected. Nine predictors for LDM response were assessed: nasal secretion IgE, nasal secretion IL-5, serum IgE, serum eosinophils, serum neutrophils, nasal polyps, asthma, allergy, and aspirin hypersensitivity, using receiver-operating curve analysis and multivariable logistic regression. Macrolide responders were those with sino-nasal outcome test-22 improvement, symptoms visual analogue scale decreased to ≤5, and no rescue medication ...

By Johanna Bennett] Shares of biotechnology giant Regeneron Pharmaceuticals (REGN) have climbed more than 4% today after Piper Jaffray analyst Ed Tenthoff upgraded the stock from a neutral to an overweight, with a $446 price target.. The upgrade comes just a few days after the drug maker released soft guidance for 2017 and forecasts called for slowing sales of the eye drug Eylea.. But Piper Jaffrays Tenthoff argues that anticipated approvals and deep pipeline should drive long-term value. Of particular importance: the atopic dermatitis drug Dupixent (dupilumab)..
