Background/Purpose: To establish EGPA/Churg Strauss inflammatory heart disease prevalence and develop an algorithm for heart disease screening in EGPA patients. Methods: An audit of all EGPA patients attending Addenbrookes was performed. Clinical presentation, cardiac studies, disease outcome measures and clinical course was noted. Values are given as percentages and median (IQR). Mann Whitney U was used. Results: 131 EGPA patients (47% men) were followed of whom 96 (73%) underwent cardiac evaluation. Endomyocardial biopsies were performed in 11 African women in Nairobi who presented with the clinical features of peripartum cardiomyopathy. The samples were studied by light and electron microscopy. In five patients there was evidence of a healing myocarditis, that is the presence of a mild inflammatory cell infiltration within the myocardium with foci of necrosis and variable amounts of hypertrophy and fibrosis. Of the nine patients who were followed up, three out of four with myocarditis had persistent heart failure and four out of five without myocarditis improved. Peripheral blood T lymphocyte cell subsets were measured in nine patients by means of monoclonal antibodies. A high helper:suppressor T cell ratio was found in three patients. Each patient will be randomized to receive either standard care and immunosuppression therapy (treatment group) or standard care alone (control group). To prevent bias, randomization will be stratified by recency of symptom onset to ensure that both the treatment and control groups are balanced with respect to it. Nonrheumatic streptococcal pharyngitis-associated myocarditis (SPAM) is infrequently reported, and its pathogenesis remains unclear. Lyme disease (LD), is the most common vector-borne illness in the US and Europe, with predominantly cutaneous, articular, cardiac and neuro-psychiatric manifestations. LD affects all layers of the heart and every part of the conducting system. Carditis is a less common manifestation of LD. Heart block (HB) as the initial and sole manifestation of LD is rare. Inducible HB has never been reported in LD. We report a case of heart block (HB) inducible with exercise and reversible with rest. A 37-year-old male presented to the emergency department after experiencing two episodes of syncope while at work. He presented, with a heart rate of 57 bpm, and the ECG showed sinus bradycardia with first degree AV block. The PR interval was 480 ms (NL 120-200 ms). Physical exam was unremarkable. The cardiologists initial impression was vaso-vagal attack. He developed high degree AV block during a stress test for the initial work up, which resolved on cessation of exercise. Anyway, DH is still hurting a bit in his chest but they gave him meds to help manage his pain. He is only to work & do activity as tolerable ... hes stubborn so this will take an act of God to get him to sit still. His 1sgt & commander are trying to get him convalesant leave or at least quarters. They know he isnt up to work but without it in writing DH will show up tomorrow morning bright & early.
