SIXTEEN DEVELOPERS or institutions have shown interest in a 36-acre section of the Presidio of San Francisco that abuts the Richmond District, home to a large, active and influential neighborhood association that will oppose any plan deemed too dense and too busy. Seven of the 16 preliminary proposals that were solicited by the Presidio Trust, managers of the former Army bases transformation into a self-sustaining national park, suggest assisted living centers for seniors. In all probability the historic 1932 building will be used by a developer, but two seven-story wings that were added in 1952 and are not regarded as historic will be razed, said Jim Meadows, the trusts executive director. Whatever rises on the site will occupy space that is, at the least, unusual. Because of hilly topography and the Presidios own limited road system, it is somewhat isolated and difficult to get to from within the Presidio, said Ron Miguel, PAR president. Even when it was the Marine Hospital, it did not

A limited number of patients admitted to psychiatric acute wards or Psychiatric Intensive Care Units are admitted with a fluctuating array of psychiatric symptoms including brief depressive episodes. These patients do not fit into the present diagnostic systems due to short duration of affective symptoms and the fluctuating arrays of other symptoms. Case histories indicate that these patients have psychiatric conditions due to epilepsy or organic brain dysfunctions.. In the study acutely admitted patients filling criteria for two recent DSM-4 axis 1 diagnoses (one is a brief depressive period) during the last two weeks are compared to age- and sex-matched patients acutely admitted with a major depressive disorder. The patients are rated with a number of rating scales at admittance, day 5, after two weeks and three months. EEG and QEEG are taken three times the first week. MRI, drug screening and blood screening of medication-concentrations are taken. ...

Have you ever wondered why some places offer surgeries, such as spay or neuter, for under $100 and other places you feel like youre paying an arm and a leg? There are several reasons why there is such a difference in pricing which is important to understand when choosing where to have the procedure for your pet.. Low cost clinics are able to offer extremely affordable prices compared to full service hospitals. It does not mean low cost provides substandard care and it doesnt mean full service are over charging to profit. Understanding the differences between the two can help put in perspective why pricing is so different. They might be performing the "same" surgery, but the overall process and procedures are not the same. Low cost clinics exist to help keep the pet population under control by offering services to spay and neuter at affordable pricing. They are a great option for many scenarios like foster pets, shelter animals, the financially distressed, etc. One of the main differences ...

A biodegradable polymeric multiphasic release system of one or more biodegradable bursting units capable of delivering biologically active substances in a burst or pulse at predetermined times and a method for constructing those units is disclosed. The individual bursting units of the system may take the form of a biodegradable, membrane coated capsule surrounding a core material which may include an active agent. The membrane ruptures after a predetermined time upon exposure to water or bodily, or other aqueous environmental fluids. The bursting units may be used in any situation in which a controlled pulsed release of an active substance is desired. The predetermined time release of each bursting unit is controlled by the composition of the core material, the initial radius and wall thickness of the membrane and the porosity of the membrane. The core material may also contain a chemical composition which generates gas as fluid is drawn into the bursting unit thus adding an additional design
Do bacteria respond to the structure of the organic matter field? Large bacterial populations can develop on hotspots-for example, marine snow, dead and even living algae-and in association with the "dark matter." [In one study, 24 to 68% of bacteria were on transparent particles (13)]. Oceanic bacteria (14) exhibit sophisticated behavior-swimming speeds of hundreds of micrometers per second, unusual swimming (run reversals), and chemosensing. The organic matter field is dominantly polymeric and suprapolymeric, so most pelagic bacteria have a diverse repertoire of surface-bound enzymes (proteases, glucosidases, lipases, phosphatases, nucleases) to cause its hydrolysis (15). Small-scale variation of species composition can significantly change the distributions of enzyme activities (16). Pelagic bacteria also have multiphasic permeases for nutrient uptake (17), with nanomolar to millimolar half-saturation constants, suggesting their adaptations to life in a patchy environment. Considering the ...

During the past 15 years in National Health Service hospitals in North England, there has been a weekend effect in patients undergoing emergency general surgery based on day of operation, but not day of admission. When this mortality difference was analyzed over the 15 years of the study, differences in weekend mortality were most pronounced in the earlier time periods and no longer present in the most recent analysis. The British Journal of Surgery study also revealed significant reductions in mortality for all emergency general surgery admissions, from 5.4 percent in 2000-2004 to less than 2.9 percent in 2010-2014, a relative reduction of 46 percent. When considering only postoperative mortality, rates fell from 7.5 percent in 2000-2004 to 3.6 percent in 2010-2014, a 52 percent relative reduction.. "Emergency surgery has long been recognised as an area of surgical practice that is high risk, complex and facing significant challenges with an ageing population. In conducting this research we ...
A larger qualitative study by Woolhead, Calnan, Dieppe, and Tadd (2004) of seventy two older people in the United Kingdom (UK) demonstrated that older peoples dignity was often violated by hospital staff exposing patients naked bodies to strangers when, for example, a hoist was used for lifting. In one situation a person in a stroke rehabilitation unit said, "[I was] on the hoist and I used to say can I cover myself up and they just pulled your nightie down over you but the back view was wide open to anybody" (p. 167 ). In these situations in which patients were being taken to the toilet, the act of elimination was far from private, as is the case in much health care today. Whatever strides may have been made towards a classless society in the UK, society can still be said to be stratified in relation to which toilets one may use. The higher ones status, the more private the facility. In UK National Health Service Hospitals, it is commonly the case that staff would never use the patients ...
New revelations are coming from the UK that doctors are discharging sick and disabled newborn babies, from National Health Service hospitals in England only to die slowly at home or in hospices in an unfathomable manner. The innocent children are being put on controversial "death pathways," once only thought to have involved elderly and terminally ill adult patients.. According to the UK Daily Mail, one doctor has admitted starving and dehydrating ten babies to death in the neonatal unit of one hospital alone. Writing in a leading medical journal, the physician revealed the process can take an average of ten days during which a baby becomes smaller and shrunken. The LCP - on which 130,000 elderly and terminally-ill adult patients die each year - is now the subject of an independent inquiry ordered by ministers.. ...

NICE has recommended idelalisib, when given with another cancer treatment rituximab, for adults with CLL who have a specific genetic change and have not received any other treatment or for all adults with CLL if their cancer has come back less than 24 months after previous treatment.. The decision follows a preliminary decision earlier this year where NICE asked for the company to provide further information on the cost effectiveness of the drug. Gilead Sciences responded by submitting new economic analyses. A simple discount agreement to the list price of idelalisib was included in the initial submission. Professor Carole Longson, NICE health technology evaluation centre director, said: "We are delighted that Gilead Sciences responded to our consultation by providing further information for our independent appraisal committee and that they had agreed to provide idelalisib to the NHS at a reduced price in the initial submission. For people whose cancer has returned less than 2 years after their ...

Objective personality tests, such as the Minnesota Multiphasic Personality Inventory (MMPI), might be more sensitive to reflect subclinical personality and be more state-dependent in an individuals lifetime, so they are good scales to predict the psychological distress regarding certain states. The aim of this study was to identify the specific pattern between body mass index (BMI) and psychological distress using the objective personality test. For this study, we investigated BMI and the Korean Military Multiphasic Personality Inventory (MPI). A retrospective cross-sectional study was conducted with 19-yr-old examinees who were admitted to the Military Manpower Administration in Korea from February 2007 to January 2010. Of 1,088,107 examinees, we enrolled 771,408 subjects who were psychologically apparent healthy possible-military-service groups. Afterwards, we reviewed and analyzed directly measured BMI and MPI results. In terms of the validity scales, the faking-good subscale showed an ...

The Nebraska Diagnostic and Evaluation Center (D&E) is a maximum custody, reception, diagnostic, evaluation, assessment, classification and assignment facility for the Nebraska Department of Correctional Services (NDCS). Located in Lincoln, Nebraska, D&E was established by the Nebraska State Legislature in 1976 as part of Legislative Bill 984. Construction was completed in June 1979 and the new institution was opened in August 1979 as a 176-bed facility. As provided by State Statute, all adult males sentenced to the NDCS by the County and District Courts of the State of Nebraska are received into the correctional system at the Diagnostic and Evaluation Center. In addition, the institution houses ninety-day evaluators sentenced by the court for assessment, county safe keepers, interstate transfers and returned parolees and escapees. Initially the facility was set-up to handle one inmate per bed (176 inmates), but overcrowding has forced the prison to add beds to the single person rooms and has ...

Studies of premenopausal (6) and postmenopausal (7) women have correlated serum estradiol levels with increased endometrial thickness. To validate transvaginal ultrasound measurement of endometrial thickness as a surrogate marker of estrogen status, we examined the association between endometrial thickness and other factors, including some factors known to be associated with estrogen status in postmenopausal women. In postmenopausal women undergoing screening transvaginal ultrasound for early detection of ovarian cancer, we observed a strong and consistent association between endometrial thickness and factors reflecting exogenous (current HRT use) and endogenous (BMI) estrogen exposure. Other studies have described similar associations involving endometrial thickness, HRT use, and BMI (7-11). In postmenopausal women not receiving HRT, serum estrogen concentrations increase with body weight (12) due to peripheral aromatization of ovarian and adrenal androgens in adipose tissue (13, 14). In women ...

This is a good case of mine, hes a 68 year old male. He has alcohol cirrhosis and esophageal varices, hes not a prisoner. But he doesnt speak English, so just putting that out there. Hes a child Pugh A5, he had a total bilirubin of 0.7 his AFP was 43 so not indicative, and his ECOG was 0-1 when he saw me for the first time in clinic and this is what his initial CT scan showed, to from my clinic, he had not been followed up appropriately and just came in with a belly ache and they noted this very large mast in his right hepatic lobe. Further down you can see that the tumor extends more inferiority and you are getting a very avid enhancement of the portal vein and you can see theres some portal vein thrombosis in there. So the arrow is really pointing at what I was reading as an arterial portal shunt. So knowing that this is what you are going to face ahead of time sort of can help you plan. So arterial portal shunting diagnosis can very often be made on multiphasic CT scans so you know Im ...
The disposition of morphine was investigated by means of radioimmunoassay after a single intravenous dose (10 milligrams per 70 kilograms) was administered to 10 adult normal male subjects who had not received other drugs for 2 weeks preceding the study. Although the cAMP and phosphoinositide (PI) second messenger systems are involved in olfactory signal transduction, aspects of their roles remain unclear. We have further examined the rapid kinetics of cAMP fluctuations in response to odorants in rat olfactory cilia isolated by calcium shock. Odorants cause a rapid and transient subsecond elevation of cAMP levels, as well as a more sustained signal lasting 5-10 sec. Basal cAMP levels demonstrate a biphasic calcium dependence; calcium enhanced both adenylyl cyclase (AC) and phosphodiesterase (PDE) activities. The odorant-induced cAMP response also demonstrated a biphasic dependence on calcium, with peak activity at 10 microM free calcium. All odorants tested were found to stimulate cAMP accumulation, and the dose-response curves were multiphasic, with less stimulation seen at higher concentrations. Dose-response curves performed for isovaleric acid at two free calcium concentrations demonstrated that calcium can influence cellular responsiveness ...

TDMH, as an organization, is committed to team management and shared leadership. It is in this spirit that the vision of consolidating all diagnostic outpatient services was realized. The consolidation achieved the dream of team members that patients could access all outpatient diagnostic services in one area, during one pre-scheduled visit and be served by a core group of well-trained, patient focused multi-skilled health-care professionals. Adjacent to the Emergency Room, this consolidation offers increased convenience for patients, nurses and physicians. Located at the outpatient entrance, this service also ensures easy access for all patients and facilitates convenient access to patient parking areas. On September 24, 1996, a team was formed to further investigate Consolidated Outpatient Services. Their goal was "To provide outpatient Diagnostic and Treatment Services that are patient focused, efficient and effective, using a multi-skilled team approach." All decisions were arrived at ...

Relationships at work were positive both among COs and with others with whom we worked. We worked five days a week and our two days off were usually together. Once we were familiar with the routine on the various wards it was easy to trade shifts and days off. That way we could line up four or even six consecutive days off with which to go home on leave. Mr. Inglis [the medical superintendent] was very lenient with us about this. He said that he knew one of us would be on duty in each place even if he did not always know who. Soon we started trading with the regular staff too and soon they started helping each other out as well. This never used to happen before, they said. [ASM, 138-153] ...
