Although recent advances have been made in antiviral therapy against AIDS, there is currently no cure for AIDS. It is likely that ultimate control of the disease depends on the development of safe and effective vaccines against HIV.. Healthy, adult volunteers without identifiable high-risk behavior for HIV-1 infection are vaccinated. In phase 1, at each participating unit, four volunteers receive a dose of gp160 (12.5 mcg); two volunteers receive placebo. Volunteers are monitored 1 month before proceeding to the second phase. In phase 2, four volunteers receive gp160 (50 mcg); two volunteers receive placebo. Primary immunization and two booster immunizations at day 30 and day 180 are done in an outpatient setting. Volunteers are closely monitored for the first 2 weeks postimmunization (primary and boosters), and extensively followed for a minimum of 2 years. Volunteers may be offered an additional boost of the same preparation at 18 months. Per 07/28/92 amendment, 18 volunteers will receive 200 ...

Study to demonstrate added value of Unyvero™ P50 pneumonia application in clinical routine -. Curetis AG today announced the start of an additional prospective, multicenter clinical trial of its marketed Unyvero™ P50 Pneumonia Application to demonstrate its clinical and health economic value.. The CE performance evaluation completed last year already demonstrated 81% sensitivity at 99% specificity for detecting pneumonia-causing pathogens. Following the market launch in April 2012, Curetis in 2013 presented data from more than 1,000 patient samples showing overall sensitivity of its pathogen panel of 80.6% at a specificity of 96% (ECCMID European Conference of Clinical Microbiology and Infectious Diseases, Berlin).[1]. The new multicenter study aims to establish real-world data on the health economic impact of fast, patient-near molecular testing and subsequent therapy adjustments (if necessary). It will be conducted in five leading European centers and will involve several hundred ...

Results At baseline all groups were comparable in their demographic, clinical and radiographic characteristics. One hundred twenty-seven pts completed the study. At 12 months there was no radiographic progression in the majority of pts (,75% in each group). Increase of the number of erosions and joint space narrowings was maximal in MTX group (mean, 35 and 80 respectively), and minimal in MTX-P group (mean, 12 and 20 respectively), p = 0.066. Slowing of disease progression with Lef was similar to MTX. There were no significant differences in radiological score between groups MTX-P and MTX-P-MP at 12 months Relative slowing of disease progression was noted in groups MTX-P and MTX-P-MP in comparison with MTX group (p = 0.08) but not with Lef group (p,0,1). Of note, among 20 pts with evident radiographic joint progression 9 pts had good clinical response according to EULAR criteria. RF and ACCP had no predictive value for the radiologic progression.. ...

The importance of unbiased comparative research trials to answer clinical questions that guide best treatment practices is unquestionable. Large multicenter clinical trials are critical for answering important clinical questions that cannot be answered by observational data. These studies depend on adequate recruitment from unbiased samples of patients from the population of interest. Investigators participating in such trials must also have clinical equipoise that the treatment under study may or may not be better than the standard treatments available. If such clinical equipoise does not exist, then recruitment will be biased and potentially delayed. In recent years, stroke clinical trials have struggled to enroll patients to acute trials for a variety of reasons, including narrow eligibility time windows and, most recently, to the growing list of available, although not necessarily safe or efficacious, therapies.. In January 2008, the US Food and Drug Administration cleared the second device ...
Purpose: Recent developments in rare-cell technology have led to improved blood-based assays that allow for the reliable detection, enumeration, and more recently, genomic profiling of circulating tumor cells (CTCs). We evaluated two different approaches for enumeration of CTCs in a prospective therapeutic study of patients with metastatic triple-negative breast cancer (TNBC). Experimental Design: The CellSearch system, a commercially available and U.S. Food and Drug Administration (FDA)-cleared assay for CTC enumeration, and IE/FC, an alternative method using EPCAM-based immunomagnetic enrichment and flow cytometry that maintains cell viability, were used to enumerate CTCs in blood of patients with metastatic TNBC. CTC numbers were assessed at baseline and 7-14d after initiation of therapy with cetuximab ± carboplatin in a Phase II multicenter clinical trial (TBCRC 001). Results: CTC numbers from two methods were significantly correlated at baseline (r=0.62) and at 7-14d (r=0.53). Baseline ...

Planned as a repository of information on potential sites for multicentre clinical trials in neuromuscular diseases, the Clinical Trial Site Registry (CTSR) has attracted significant interest from both industry and academic institutions.. Currently, the CTSR contains data on over 200 registered sites in 41 countries worldwide and in the last two years nine official requests for the identification of potential trial sites have been received, many along with data requested from the TREAT-NMD Global Patient Registries.. In addition to this primary function, the CTSR has also developed as an international network of centres caring for many types of neuromuscular disorders. Furthermore, this network can also be used to disseminate care recommendations, and to evaluate current practice in different countries and individual centres. This is currently underway as part of the EU-funded CARE-NMD project to address care practices for Duchenne muscular dystrophy.. At a recent TREAT-NMD meeting the ...

Cellular immune responses elicited by vaccination are complex and require polychromatic analysis to accurately characterize the phenotype and function of rare, responding cells. Technical challenges and a lack of instrument standardization between research sites have limited the application of polychromatic cytometry in multicenter clinical trials. Two previously developed six-color T cell subset immunophenotyping reagent panels deliberately designed to accommodate three additional low frequency functional measurements were compared for their reproducibility of staining across three different flow cytometers. We repeatedly measured similar T cell subset frequencies between the two reagent panels and across the three different cytometers. Spectral overlap reduced sensitivity in two of the three open measurement channels (PE [IL-2] and APC [IFN gamma]) for one reagent combination, particularly in subsets with low cytokine expression. There was no significant interassay variation for measurements ...
After the announcement of the judgment, people cheered in the street [LAT report], lighting fireworks and chanting for Kasabs death. Kasab can still appeal to Indias highest court, and, if that is unsuccessful, he can petition the government for clemency. Kasab will likely be hanged, but the appeals could delay his execution for years. Kasab filed his appeal in June after being convicted [JURIST reports] in May for his role in the attack, which was allegedly coordinated by Pakistani militant group Lashkar-e-Taiba (LeT) [CFR backgrounder]. He was sentenced to death after the prosecution sought the death penalty [JURIST reports], citing eight aggravating circumstances. Judge ML Tahiliyani, specially appointed in January 2009 to preside over the trial of the three suspects detained after the attacks, heard closing arguments [JURIST report] in the case last March. In January 2010, Tahiliyani denied [JURIST report] Kasabs request for an international trial after Kasab claimed that he would not ...

Quality of life in preoperative patients with sacroiliac joint dysfunction is at least as depressed as in other lumbar spinal conditions Daniel Joseph Cher, W Carlton RecklingSI-BONE, Inc., San Jose, CA, USABackground: Pain from the sacroiliac joint (SIJ) is an under-recognized cause of low back pain. The degree to which SIJ pain decreases quality of life has not been directly compared to other more familiar conditions of the lumbar spine. Methods: Multivariate regression analysis of individual patient data from two prospective multicenter clinical trials of SIJ fusion and three prospective multicenter clinical trials of surgical treatments for degenerative lumbar spine conditions. Results: Controlling for baseline demographic parameters as well as a validated disability score, quality of life scores (EuroQOL 5-D and SF-36) were, in most cases, lower in the SIJ cohorts compared to the three other spine surgery cohorts. Conclusion: Patients with SIJ dysfunction considering surgery have decrements in quality
Conclusions The poster campaign failed to increase awareness of local research amongst people using musculoskeletal clinic waiting areas. This was despite significant input to design and content by patient research partners and high approval of final drafts by this group. Few people reported seeing the posters, which may be due to the high volume of competing posters on display. This concurs with existing data that people fail to see and be influenced by posters in clinic waiting areas (1). Those people who had seen the posters were significantly more aware of local research, which may be due to the impact of the poster. Another explanation is that people already aware of local research disproportionately noticed and remembered the posters. To increase the success of a research awareness campaign, tools should be developed and tested with their target audience (i.e. patients not aware of or involved in research) in addition to patients already actively involved. ...

for the management of proteinuria. Several studies have determined its efficacy in decreasing blood pressure in cats. This study was designed as a prospective, double blinded, randomized, placebo controlled multicenter clinical trial. An initial 28 day parallel group phase was followed by a 154 day open label extended use phase.. Client owned cats with indirect systolic blood pressures between 160-200mmHg by doppler were recruited from participating clinics. Cats were stratified by underlying disease (CKD, hyperthyroidism, both, or neither), and all concurrent disease was required to be stable. Cats were excluded for the use of blood pressure modifying drugs or a range of conditions known to effect blood pressure.. Telmisartan was administered to cats at 1.5mg/kg PO q12h for 14 days, then 2mg/kg q24h. Placebo cats were administered an equal volume of visually identical inactive solution. Blood pressure was rechecked at 14 and 28 days, and then cats in the treatment group with a blood pressure ...

How close is medicine to a world in which your treatment for hypertension, coronary artery disease or pain is fine-tuned to your genotype? Weaving a path through the thorny issues surrounding "personalized medicine," Julie Johnson and her colleagues are now showing that a genotype-driven approach to drug dosing can work. An example is the blood thinner warfarin, where the therapeutic daily dose can be anywhere from 1 to 20 milligrams, and patients must have frequent blood tests to prevent dangerous bleeding and strokes. People of different ancestry vary broadly in how they metabolize these drugs, and new research has identified several genetic factors that explain much of the varying dose response. International trials are looking at the practical application of the findings and beginning to examine the big payoff: using genotype to predict the long-term outcome of a given treatment for a particular individual.. ...
