Reform of primary healthcare (PHC) organisations is underway in Canada. The capacity of various types of PHC organizations to respond to populations needs remains to be assessed. The main objective of this study was to evaluate the association of PHC affiliation with unmet needs for care. Population-based survey of 9205 randomly selected adults in two regions of Quebec, Canada. Outcomes Self-reported unmet needs for care and identification of the usual source of PHC. Among eligible adults, 18 % reported unmet needs for care in the last six months. Reasons reported for unmet needs were: waiting times (59 % of cases); unavailability of usual doctor (42 %); impossibility to obtain an appointment (36 %); doctors not accepting new patients (31 %). Regression models showed that unmet needs were decreasing with age and was lower among males, the least educated, and unemployed or retired. In Manitoba, patients are initially assessed at one centre but can subsequently receive chemotherapy at an outreach centre closer to home. In British Columbia, two outreach mechanisms exist: consultative clinics provide consultative and follow-up care by radiotherapists and medical oncologists who travel to outreach centres, and community-based regional oncologists provide diagnostic and treatment services in small communities. Studies of email-based telemedicine have shown that some roles - namely administrative and supervisory - are more automation friendly than clinical roles [11]. Purpose-written email systems - through automation - can reduce or eliminate these roles. This study has determined that when more than ~5200 email consultations are undertaken annually, purpose-written software for use in counselling settings reduces costs through freeing up staff time compared with using COTS. The potential savings increased as workload increased above the threshold value. The threshold was most sensitive to changes in key-input costs (administrative staff costs, capital costs and service fees) affecting only one of the service models. Costs, which affected both service models, resulted in lesser change - for example, counselling staff salary had no effect on the threshold.
