Reform of primary healthcare (PHC) organisations is underway in Canada. The capacity of various types of PHC organizations to respond to populations needs remains to be assessed. The main objective of this study was to evaluate the association of PHC affiliation with unmet needs for care. Population-based survey of 9205 randomly selected adults in two regions of Quebec, Canada. Outcomes Self-reported unmet needs for care and identification of the usual source of PHC. Among eligible adults, 18 % reported unmet needs for care in the last six months. Reasons reported for unmet needs were: waiting times (59 % of cases); unavailability of usual doctor (42 %); impossibility to obtain an appointment (36 %); doctors not accepting new patients (31 %). Regression models showed that unmet needs were decreasing with age and was lower among males, the least educated, and unemployed or retired. Controlling for other factors, unmet needs were higher among the poor and those with worse health status. Having a family
Canadian cancer care has evolved under systems of provincial and federal fiscal control and aims to optimize the management of patients within each province. In Manitoba, patients are initially assessed at one centre but can subsequently receive chemotherapy at an outreach centre closer to home. In British Columbia, two outreach mechanisms exist: consultative clinics provide consultative and follow-up care by radiotherapists and medical oncologists who travel to outreach centres, and community-based regional oncologists provide diagnostic and treatment services in small communities. In Ontario, peripheral clinics are staffed by oncologists from the two provincial cancer institutions, but efforts have been made to find newer outreach models that will better serve certain communities in that province. Two basic models of cancer care organization in the community presently exist in Canada: one stresses the extension of resources outward to the community from major cancer centres, and the other ...

Studies of email-based telemedicine have shown that some roles - namely administrative and supervisory - are more automation friendly than clinical roles [11]. Purpose-written email systems - through automation - can reduce or eliminate these roles. This study has determined that when more than ~5200 email consultations are undertaken annually, purpose-written software for use in counselling settings reduces costs through freeing up staff time compared with using COTS. The potential savings increased as workload increased above the threshold value. The threshold was most sensitive to changes in key-input costs (administrative staff costs, capital costs and service fees) affecting only one of the service models. Costs, which affected both service models, resulted in lesser change - for example, counselling staff salary had no effect on the threshold. An adjustment of 50% in costs resulted in maximum change of 4.5% to the threshold hence, effect was considered small. However, the annual volume of ...

Lack of staff nurse participation in hospital decision-making has been cited as a major reason for the dissatisfaction in nursing. Shared governance has been proposed as an organizational model that provides staff nurses with both the structure and the mechanism for having increased decision-making authority. The purpose of this study was to investigate the relationship of organizational culture, perceived importance of involvement and actual involvement in decision-making, the discrepancy between importance and involvement, staff nurse years of involvement in shared governance, control over nursing practice, and work satisfaction among staff nurses working in hospitals with shared governance. A descriptive, correlational design was used to investigate 188 full-time RN staff nurses from three hospitals with shared governance. Organizational culture was eliminated from analysis because of the large amount of missing data. Three multiple regression models were tested. In the final prediction ...
According to Schein (1992),[3] culture is the most difficult organizational attribute to change, outlasting organizational products, services, founders and leadership and all other physical attributes of the organization. His organizational model illuminates culture from the standpoint of the observer, described at three levels: artifacts, espoused values and basic underlying assumptions. At the first and most cursory level of Scheins model is organizational attributes that can be seen, felt and heard by the uninitiated observer - collectively known as artifacts. Included are the facilities, offices, furnishings, visible awards and recognition, the way that its members dress, how each person visibly interacts with each other and with organizational outsiders, and even company slogans, mission statements and other operational creeds. Artifacts comprise the physical components of the organization that relay cultural meaning. Daniel Denison (1990) describes artifacts as the tangible aspects of ...
The healthcare reform in Bulgaria brought about major changes in both undergraduate and postgraduate (PG) programs in medicine and especially in General Medicine. The PG program in General practice of the Medical University - Plovdiv was designed to help the new kind of doctors - GPs improve the quality of their work not only by enhancing their clinical competence but also by introducing a new organizational model. One of the topics in the program was dedicated to the implementation of social research methods in general practice for assessing the needs of the patients and the possibilities for quality improvement ...
As the largest and most prominent centre in Sweden, the Transplant Institute is also unique in Scandinavia in that a full programme for all organ transplants is offered here. It is the only unit that performs multivisceral transplants. And it all started in 1965 when the first kidney transplant took place right here. Ever since then, Sahlgrenska University Hospital has continued to contribute to development in the field of transplantation both locally and globally.. Some years ago Sahlgrenska University Hospital created a new organizational model where all organ-transplanted patients are gathered in one place - the Transplant Institute. Applying a cross-disciplinary approach, all medical professionals involved in the care of a patient belong to the same unit. The outcome: shorter in-ward treatment periods and shorter transplant waiting times. And more importantly, an integrated team can develop a closer bond with the patient and contribute to an enhanced treatment experience.. During 2015 the ...

PY - 2009/9. Y1 - 2009/9. N2 - The commitment to fight against cancer is a part of European Community health strategies and of Italian health services planning. Interesting and useful results have been achieved in Europe and in our country where most of the population can participate to screening programmes. In Italy there are two main organisational models to deliver intervention for secondary prevention: population based and individual access. Several experimental data show population-based prevention is more fair and appropriate and achieves better results in reducing incidence and mortality rates; data are summarized. Because of this differences we go trough the different characteristics of the two models to handle them better planning and managing more efficiently a sustainable ...

Background: Suicide bereavement can have a lasting and devastating psychosocial impact on the bereaved individuals and communities. Many countries, such as Australia, have included postvention, i.e., concerted suicide bereavement support, in their suicide prevention policies. While little is known of the effectiveness of postvention, this review aimed to investigate what is known of the effects of postvention service delivery models and the components that may contribute to the effectiveness.Method: Systematic review and quality assessment of peer reviewed literature (Medline, PsycINFO, Embase, EBM Reviews) and gray literature and guidelines published since 2014.Results: Eight studies and 12 guidelines were included, with little evidence of effectiveness. Still, providing support according to the level of grief, involvement of trained volunteers/peers, and focusing the interventions on the grief, seem promising components of effective postvention. Conclusions: Adopting a public health approach to

Cloud computing is widely considered as an attractive service model because it minimizes investment since its costs are in direct relation to usage and demand. However, the distributed nature of cloud computing environments, their massive resource aggregation, wide user access and efficient and automated sharing of resources enable intruders to exploit clouds for their advantage. To combat intruders, several security solutions for cloud environments adopt Intrusion Detection Systems. However, most IDS solutions are not suitable for cloud environments, because of problems such as single point of failure, centralized load, high false positive alarms, insufficient coverage for attacks, and inflexible design. The thesis defines a framework for a cloud based IDS to face the deficiencies of current IDS technology. This framework deals with threats that exploit vulnerabilities to attack the various service models of a cloud system. The framework integrates behaviour based and knowledge based techniques ...
Aims and method. The Norfolk Youth Service was created in 2012 in response to calls to redesign mental health services to better meet the needs of young people. The new service model transcends traditional boundaries by creating a single, youth friendly service for young people aged 14-25 years. The aim of this study was to investigate the effect of the transition to this new model on patterns of referral, acceptance and service use. We analysed routinely collected data on young people aged 14-25 years referred for secondary mental healthcare in Norfolk before and after implementation of the youth mental health service. The number of referrals, their age and gender, proportion of referrals accepted and average number of service contacts per referral by age pre- and post-implementation were compared. ...

Background: Counselling is considered a prerequisite for the proper handling of testing and for ensuring effective HIV preventive efforts. HIV testing services have recently been scaled up substantially with a particular focus on provider-initiated models. Increasing HIV test rates have been attributed to the rapid scale-up of the providerinitiated testing model, but there is limited documentation of experiences with this new service model. The aim of this study was to determine the use of different types of HIV testing services and to investigate perceptions and experiences of these services with a particular emphasis on the provider initiated testing in three selected districts in Kenya, Tanzania, and, Zambia.. Methods: A concurrent triangulation mixed methods design was applied using quantitative and qualitative approaches. A population-based survey was conducted among adults in the three study districts, and qualitative data were obtained from 34 focus group discussions and 18 in-depth ...
The industry-defining alliance with AU Health, based on a managed services model, was the first ever partnership of this type in the U.S. and one of the largest in the world when it was signed in June 2013. Here are a few examples that highlight how Philips has made a difference since then to the health systems people and patient groups.. Prior to teaming with Philips, AU Health had one primary ventilator platform, and not all units were equipped with a comprehensive software package to provide all ventilation modalities. When a patients respiratory status changed for better or worse, precious minutes were wasted as respiratory therapists were forced to scramble to find and disinfect an available ventilator. Working together on this issue through our joint Technology Working Group, Philips helped AU Health update and standardize its fleet of ventilators to provide all modes of ventilation: invasive, noninvasive, and CPAP modes for neonates. This enabled Augusta University to not only reduce ...

Currently, the occupational profile of those responsible for educational guidance (counsellors) is based on psycho-educational training in line with the functions of current educational interventions; however, an increasing call for a systemic and global epistemological perspective in educational and psychosocial situational analysis requires a rethinking of the praxis of current professional profiles and an incorporation of new approaches in the development of guiding practices. \nThis research consists of a comparative analysis of the results obtained in the academic variables found as a consequence of the application of three basic models of educational guidance: 1) the social mediation model; 2) the counselling model; and 3) the service model, delivered through programs (services-programs), developed over a three year period, distributed across three experimental groups corresponding to each of these guidance models.\nA total of N: 194 secondary school students of different ages and genders ...
through the Early Intervention Psychosis Program. What we found was that for every family who qualified under the Early Intervention Psychosis mandate, we had another four families who sought our help for loved ones with varying degrees of mental illness, including those with histories of trauma and concurrent disorders. We had the capacity to provide only brief support to these families; unfortunately, they were unable to take advantage of our family recovery programs and clinical support because of the specific mandate of our funded programming.. We thought: wouldnt it be great if we could increase our funded capacity to assist these families? Our proposed service model would include comprehensive interventions for families and their loved ones within a recovery-oriented, peer, and family-supportive environment. In November of last year, our dream became a reality when we received additional funding from the TC LHIN to do just that, and our new Family Matters Peer Support and Recovery ...

The Business Model Canvas, developed by Alexander Osterwalder, is a Strategic Management and Lean Startup template for developing new or documenting existing business models. The canvas is presented as a visual chart with 9 elements spanning the areas of the organizations core offering, infrastructure, customers, and finances. These 9 elements are: 1. Key Partners 2. Key Activities 3. Key Resources 4. Value Propositions 5. Customer Relationships 6. Distribution Channels 7. Customer Segments 8. Cost Structure 9. Revenue Streams This tool can be used in a collaborative setting, e.g. strategy workshop. It helps organizations align their activities by illustrating and discussing potential trade-offs in a structured manner. Business Model Canvas is a framework to literally bring everyone on the same page. Many consulting firms have developed their own canvases for specific niches, based on the principles of the Business Model Canvas. This presentation also discusses the

The Régie régionale de la santé and des services sociaux de Montréal-Centre became involved in 1998 in a project to examine integrated services within its territory to assess the modifications in configurations of use and costs of social services and healthcare for frail elderly people attributable to the integrated system, with a view to expanding it throughout its territory or adopting the positive aspects. The demonstration project successfully implemented the clinical and organizational principles of the integrated services model, excluding responsibility for healthcare and all services for the entire frail elderly population and financial responsibility for these services.. The population targeted by the demonstration project, the methods for implementing the demonstration project, the financial and human resources invested, the choice of implementation sites, the assessment approach and the work calendar were developed and selected by the Solidage group and the Régie régionale in ...
Findings show that using community participation can lead to designing new service models that fit within existing budgets and address local aspirations and healthcare priorities.. Participant communities were similar on dimensions of rurality, health status, aspirations and health priorities. They proposed a consistent set of requirements for remote Scottish primary healthcare. These were: resident practitioner(s), expert emergency triage, monitoring and anticipatory care of vulnerable people, community volunteering for health improvement, and leadership of community (health) volunteering.. Despite community similarities, community participation led to different service designs for three communities and one community did not participate at the final service design stage. These diverse outcomes suggest the influence of local contextual factors. This implies that different communities do want different things so suggesting the same standard model for all, apparently alike, rural communities is ...
Evaluation in design processes means to assess a wide range of ideas against the goals and constrains of project in order to decide upon the most appropriate final concept [20]. Four types of evaluation were observed in this study: staff input, piloting, rapid improvement, and public consultation. New service development and service integration projects usually started with capturing staffs concerns on new service models proposed in policy documents. Then, small-scale and short-term piloting was carried out. Service improvement projects had rapid improvement in which new ideas were quickly tested, adjusted and implemented into practice. Service closure projects had staff and public consultation as a formal mechanism for gaining input from affected stakeholders.. The focus of the evaluation methods seen in this study, mainly use some form of controlled implementation. This is in line with a common approach to service development in healthcare organisations, Plan-Do-Study-Act [36]. While this is ...
The NHS 2012 Programme does not have a pan-London strategy for mental health planning for the Games. Modelling specific to mental health service demand has not been conducted. Prima facie this may not be imprudent, as data from previous Olympics do not suggest there will be a substantial rise in demand for mental health services. There may even be a decrease. However, the lack of data may be more reflective of monitoring practices, priorities in health surveillance systems or variations in service models resulting in people with mental illness being less likely to access or be referred to services.. Difficulty in estimating visitor numbers to the Games adds further uncertainty. Modelling by the Office for National Statistics suggests there will be an additional 510 000 visitors in London each day during the Games period.3 This projection needs to be cautioned, as predictions for visitor numbers for previous games have been unreliable. Post hoc estimates of visitor numbers show large variations ...
Project Summary. Human aging is a challenge for many health care systems. For instance, while we observe a breath taking surge of chronic diseases in many countries we also observe that health care systems are surprisingly ill-equipped to answer to this challenge. This project proposes that health care providers can explore the disruptive potential of information technology to improve care delivery by pooling information technology with business models, which are adapted to the idiosyncrasies of health care. ...
Of the many service models that might deliver antidementia drug treatments, most are likely to be clinic-based. To our knowledge, this is the first description of a service that employs experienced nursing staff solely to ensure the effective drug treatment of dementia and which delivers the intervention in patients homes. The benefits of a home-based service are considerable; the most obvious is the combination of enhanced convenience for patients and reduced non-attendance rates (Anderson & Aquilina, 2002). The tasks delegated to the specialist nurses enabled the sharing of a large workload, which may have otherwise significantly prolonged patient waiting lists. We believe that employing experienced community nurses benefits both patients and other members of the services. The CATS nurses provide continuity in assessment and have an important pastoral and practical role in directing patients and carers towards appropriate services. Using these common and comprehensive response measures makes ...

One of the hardest things to do as a consultant is to figure out how much to charge for your services. And we spent over half of our three hour course talking about this.. The first way to figure out how much to charge can be called cost-plus.. You decide how much money you need, how much time youre likely going to work to earn it, and divide to get a daily or hourly rate. They you add an amount you think that you can get on the market, and at the end of the year, this is your companys profit. Profit is handy, since its what allows you to grow - to hire new people, take on new projects, and maybe move from consulting to a product-based business model.. A well-known rule of thumb (which Stephen Walli recently restated to me) is that a successful consultant will bill roughly 50% of the available time in a year. Some years will be worse, some years will be better. But between looking for new clients, down time between projects, and handling everything that you need to do when youre at the head ...
