METHODS AND RESULTS The purposes of this study are to determine the short- and intermediate-term outcomes of patients undergoing BMC after previous surgical commissurotomy, to compare these patients with those undergoing balloon mitral commissurotomy as an initial procedure, and to elucidate the multivariate determinants of acute procedural and clinical outcome. Of 738 patients undergoing BMC as part of the National Heart, Lung, and Blood Institute Balloon Valvuloplasty Registry, 133 underwent BMC after previous surgical mitral commissurotomy. Prospective data obtained included demographic, hemodynamic, echocardiographic, and clinical follow-up. BMC after previous surgical commissurotomy produced a significant reduction in transvalvular gradient from 13 +/- 5 to 6 +/- 3 mm Hg (p less than 0.0001) and an increase in mitral valve area from 1.0 +/- 0.3 to 1.8 +/- 0.8 cm2 (p less than 0.0001). Background-Long-term follow-up after percutaneous mitral commissurotomy (PMC) enables predictive factors of late results to be identified. Methods and Results-Late results of PMC were assessed in 1024 consecutive patients. Good immediate results, defined as valve area ≥1.5cm2 without mitral regurgitation ,2/4, were obtained in 912 patients (89%). These 912 patients were randomly split into 2 cohorts comprising 609 and 303 patients, used respectively to develop and validate a scoring system predicting late functional results. Patients with myelodysplastic syndrome (MDS) most commonly have refractory anemia accompanied by various degrees of granulocytopenia and thrombocytopenia. At the time of cardiac surgery, both major infections and bleeding are severe complications in patients with pancytopenia due to MDS. However, there were very few patients with MDS who had undergone open-heart surgery. We reported a case of mitral valve replacement in a patient with MDS. A 68-year-old man with valvular heart disease and MDS, with a platelet count of 1.9 ~104/mm3, underwent successful mitral valve replacement. The mitral valve was replaced by an SJM25A prosthesis after resection of left atrial thrombosis using cardiopulmonary bypass. Platelets were transfused after the bypass. Results Although most TR regressed in these patients immediately after the procedure, 56 patients developed significant TR during follow-up (median 12 years, IQR 8.0-18.0 years). Patient #2 was a 34-year-old woman referred for refractory heart failure secondary to a failing mitral annuloplasty. She expressed her desire for future pregnancy, and the heart team decided to perform TMVR using a 26-mm SAPIEN 3 THV. The final mean transmitral gradient was 2 mm Hg. Anticoagulant therapy with intravenous heparin was initiated immediately after the procedure, but the results remained suboptimal during the first 24 h.
