In low and middle-income countries (LMICs), where the rates of maternal mortality continue to be inappropriately high, there has been recognition of the importance of training traditional birth attendants (TBAs) to help improve outcomes during pregnancy and childbirth. In Guatemala there is no national comprehensive training program in place despite the fact that the majority of women rely on TBAs during pregnancy and childbirth. This community case study presents a unique education program led by TBAs for TBAs in rural Guatemala. Discussion of this training program focuses on programming implementation, curriculum development, sustainable methodology, and how an educational partnership with the current national health care system can increase access to health care for women in LMICs. Recent modifications to this training model are also discussed including how a change in the clinical curriculum is further integrating TBAs into the national health infrastructure. Midwives save lives. Well-trained midwives could help avert roughly two thirds of all maternal and newborn deaths, according to the most recent State of the Worlds Midwifery report. They could also deliver 87 per cent of all essential sexual, reproductive, maternal and newborn health services. Yet only 42 per cent of people with midwifery skills work in the 73 countries where more than 90 per cent of all maternal and newborn deaths and stillbirths occur. Since 2008, UNFPA has worked with partners, governments and policymakers to help build a competent, well-trained and well-supported midwifery workforce in low-resource settings. UNFPA focuses on four key areas: strengthening competency-based midwifery training; developing strong regulatory mechanisms to ensure quality services; raising the voices of midwives by establishing and strengthening midwifery associations; and advocating for increased investments in midwifery services. BACKGROUND: current efforts to reduce maternal and newborn mortality focus on promoting institutional deliveries with skilled birth attendants (SBAs), and discouraging deliveries at home attended by traditional birth attendants (TBAs). In rural Kenya, semi-nomadic pastoralist communities are underserved by the formal health system, experience high maternal and neonatal mortality, and rely primarily on TBAs for delivery care, despite Government proscription of TBA-assisted births. This study examined the knowledge, attitude and practices of TBAs serving these communities to assess the potential for collaboration between TBAs and SBAs. METHODS: a cross-sectional, interviewer-administered survey was conducted among 171 TBAs from Maasai and Samburu pastoralist communities in Laikipia and Samburu counties, Kenya, as part of a larger mixed-methods study in partnership with a local service provider. RESULTS: TBAs were relatively elderly (mean age 59.6 years), and attended an average of 5-6 deliveries per year.

The maternity care system in the Netherlands is well known for its support of community-based midwifery. However, regular midwifery practices typically do not offer caseload midwifery care - one-to-one continuity of care throughout pregnancy and birth. Because we know very little about the outcomes for women receiving caseload care in the Netherlands, we compared caseload care with regular midwife-led care, looking at maternal and perinatal outcomes, including antenatal and intrapartum referrals to secondary (i.e., obstetrician-led) care. We selected 657 women in caseload care and 1954 matched controls (women in regular midwife-led care) from all women registered in the Dutch Perinatal Registry (Perined) who gave birth in 2015. To be eligible for selection the women had to be in midwife-led antenatal care beyond 28 gestational weeks. Each woman in caseload care was matched with three women in regular midwife-led care, using parity, maternal age, background (Dutch or non-Dutch) and region. These two

This study shows that knowledge about HIV/AIDS is extremely low among TBA living in rural areas of Mysore Taluk. An overwhelming majority (88%) of surveyed birth attendants had not even heard of HIV/AIDS, a surprising finding given that the epidemic in India is more than two decades old. This is in stark contrast to high levels of knowledge found by the government of India among the general population in HIV sentinel surveillance surveys. The 2006 National Behavioral Surveillance Survey showed 75 percent of people living in rural areas were aware of HIV/AIDS[15]. Even the small number of TBA in our study, who had knowledge about HIV demonstrated poor understanding about how the virus was transmitted. Additionally, less than half of those were aware of the potential for preventing vertical transmission of HIV. We argue that although the number of couples in the UK experiencing fertility problems has risen, 1.4% of all births in the UK are as the result of infertility treatments (HFEA, 2005/2006); yet there is little research into the experiences of motherhood for women who successfully achieve pregnancy following IVF or GIFT. In contrast to other countries, Austria rarely offers alternative models to medical led-care. In an attempt to improve the facilities, a midwife-led care service was incorporated within the Department of Obstetrics and Fetomaternal Medicine. The aim of the present study was to analyze the maternal and neonatal outcomes of this approach. Over a 10-years period, a total of 2123 low-risk women receiving midwife-led care were studied. Among these women, 148 required obstetric referral. Age- and parity matched low-risk women with spontaneous vaginal birth overseen by an obstetrician-led team were used as controls to ensure comparability of data. Midwife-led care management demonstrated a significant decrease in interventions, including oxytocin use (p < 0.001), medical pain relief (p < 0.001), and artificial rupture of membranes (ARM) (p < 0.01) as well as fewer episiotomies (p < 0.001), as compared with obstetrician-led care. This article reports on a qualitative exploration of student midwives views of caseloading a known group of women, which formed part of a larger action research project through Bournemouth Universitys pre-registration, undergraduate midwifery programme. Analysis of the caseloading data revealed four themes: preparation to undertake a caseload; knowing your mentor; tri-partite meetings; and relevance of caseloading to their learning in becoming midwives.
