Pathologic T stages were: T1, 45 (59%); T2, 21 (28%); T3, 5 (7%); and T4, 5 (7%). Five (7%) patients had neck dissections. Five (7%) patients received adjuvant radiotherapy. Mean follow-up was 42 months. N2 - Since the successful advent of in vitro fertilization and embryo transfer (IVF-ET), reproductive surgeons have been forced to compare pregnancy outcomes of surgical procedures for tubal infertility with those of IVF-ET. The current study reports pregnancy outcomes, with extended follow-up, in patients who had proximal oviductal obstruction. Twenty-seven patients who had proximal oviductal disease with limited distal disease were evaluated after microsurgical repair. The conception rate at the end of 3 years of observation was 69.3%. The live birth rate was 53.2% at 3 1/2 years. The results indicate that microsurgical reconstruction of proximal oviductal occlusion is a workable option for patients with infertility.. Squamous cell carcinoma of the head and neck can present as a cervical metastasis from an unknown primary site. Recently, transoral robotic surgery (TORS) and transoral laser microsurgery (TLM) have been incorporated in the workup of unknown primary tumors. We searched MEDLINE, EMBASE, Cochrane, and CINAHL from inception to June 2015 for all English-language studies that utilized TORS, TLM, or lingual tonsillectomy in the approach to an unknown primary. Of 217 identified studies, eight were reviewed. TORS/TLM identified the primary tumor in 111/139 (80 %) patients overall, and 36/54 (67 %) patients with no remarkable findings following physical exam, radiologic imaging, and panendoscopy with directed biopsies. Lingual tonsillectomy identified the primary tumor in 18/25 (72 %) patients with no findings. Hemorrhage (5 %) was the most common perioperative complication. Lingual tonsillectomy using new approaches such as TORS/TLM may improve the identification of occult primary tumors.

A controlled clinical trial was performed to investigate the safety and efficacy of the hyaluronate-based gel polymer Hyaloglide after microsurgical reconstruction of peripheral nerves of the hand. Thirty patients were randomized to receive either no postsurgical treatment (n = 16) or Hyaloglide (n = 14) and were clinically evaluated at various intervals for 1 year. The application of Hyaloglide posed no safety concerns. Efficacy was assessed by the recovery of sensitivity, measurement of pain, and progression of Tinels sign. The Hyaloglide-treated group showed better improvement in recovery from pain, approaching statistical significance during the first 3 months postsurgery. Likewise, recovery of sensitivity was also higher in the Hyaloglide-treated group throughout the entire follow-up period, and the distance of Tinels sign was longer in the Hyaloglide-treated group (P , 0.05 at day 30). We report the utility of fluorescent indocyanine green (ICG) during the microsurgical resection of a giant perimedullary cervical arteriovenous fistula (GPMAVF) in a young man with hereditary hemorrhagic telangiectasia. ICG angiography is a useful adjunct for understanding angioarchitecture and drainage patterns in spinal arteriovenous malformation (AVM) surgery. Cervical GPMAVF is a rare spinal lesion found in association with hereditary hemorrhagic telangiectasia. This vascular malformation is associated with venous varices in a perimedullary location causing cord compression and vascular steal. This rare case highlights the crucial role of ICG during complex spinal AVM/AVF surgery to obtain angiographic cure.

OBJECTIVE To evaluate the impact of transanal endoscopic microsurgery (TEM) on postoperative anal function and quality of life in patients with benign rectal tumor and early rectal cancer. METHODS Clinical data of 50 patients with rectal adenoma and early rectal cancer undergoing transanal endoscopic microsurgery in our hospital from October 2008 to June 2013 were retrospectively analyzed. The technique involves a low anterior rectal or colo-anal resection, very often associated with a protective stoma or abdominal-perineal resection with permanent colostomy. 41 patients underwent transanal excision using TEMS (KARL-Storz) or single port TAMIS (Covidien) kits. All patients went through the colorectal MDT with standardized MRI reporting. Average age was 70yrs (range=40-86). Mean hospital stay was 2.7 days. In 30 years, 1,669 patients underwent open microsurgery for tubal diseases. Several techniques like adhesiolysis, reanastomosis, fimbrioplasty, salpingoneostomy, proximal reconstruction, isthmo-ostial anastomosis and reimplantation are described. Results were excellent for patients with a favourable prognosis (1,517 patients) and with very high pregnancy rate: 80% pregnancies with delivery for tubal reversal, 68% for proximal diseases, 75.1% for fimbrioplasty and 55% for salpingoneostomy. Risks of ectopic pregnancy were very low: 1.5% for tubal reversal (because the tubes were healthy), 4% for proximal diseases, 4% for fimbrioplasty and 6.7% for salpingoneostomy. Results were very low for patients with a poor prognosis (152 patients): 10% pregnancies with delivery for distal diseases, less than 20% for proximal diseases and 22% ectopic pregnancies. Objective To study the therapeutic effects for cerebral hematomas in basal ganglia region by microneurosurgery via transsylvian fissure-insular approach. Methods Retrospective analysis was performed in 23 cases with cerebral hematomas in basal ganglia region which were treated by microsurgery via transsylvian fissure-insular approach in our department from 2003 to 2005. Results The patients were evaluated by activities of daily living(ADL) after the operation, GradeⅠ was found in 3 cases, Ⅱ in 11 cases, Ⅲ in 7 cases, Ⅳ in 1 case, death in 1 case. Conclusion Microneurosurgical treatment via transsylvian fissure-insular approach for cerebral hematomas in basal ganglia region is a kind of minimally invasive method with less damage, better effects and more favorable prognosis.
We present our experience of microsurgical treatment of pericallosal artery aneurysms (PCAA) in three neurosurgical centers in Abidjan (Ivory Coast) from 1990 to 2016. This present study aimed to evaluate characteristics of 6 patients with PCAA treated during a 26-year period and to establish the rate, clinical nuances, anatomical variations and respective microsurgical approaches of PCAA in Abidjan. We analyzed medical files of all 93 patients admitted for an intracranial aneurysm between 1990 and 2016 and focused on the 6 patients who were treated for a PCCA. The mean age of patients was 37 years, half of whom were less than 30 years old. They were 3 men and 3 women. The time from first symptom to admission was more than 3 days, but less than 16 days. Five out of six patients had ruptured aneurysms and the clinical condition on admission was WFNS grade 0 one patient (16.67%) and WFNS I-III five patients (83.33%). Facial palsy is a relatively common clinical condition with a variety of causes. Irrespective of its etiology, facial palsy always represents a very serious problem for the patient. This underlines the need for more effective treatment procedures. Retrospective evaluation of a clinical material of 16 patients with facial palsy treated at the University Hospital of Linköping during the period 1990-2000 showed that to improve the results of microsurgical nerve repair experimental research - controlled studies on homogeneous materials - is imperative.. Purpose: We evaluated the effectiveness of microsurgical ligation for painful varicocele and predictive factors of pain resolution. Materials and Methods: Between January 2006 and March 2009, a total of 114 patients (mean age, 30.2 +/- 18.9 years), who underwent microsurgical inguinal varicocelectomy for painful varicocele, were included and followed up for 1 year after the surgery. The quantity of preoperative and postoperative pain was assessed by means of 11-point numeric rating scale (NRS). We retrospectively analyzed the outcome of surgical ligation and predictive factors of pain resolution using patient age, height, weight, body mass index, grade and location of varicocele, duration, quantity and quality (dull, dragging, aching) of pain, and postoperative pain resolution. Objective: An increasing number of fusiform vertebral artery aneurysms (FVAA) were treated by endovascular interventions. Microsurgical clipping of these lesions disappeared from routine operative theatre even in neurovascular centers. Still, microsurgery may be an option in patients with FVAA in very experienced hands.. Method: During the last 6 years, seven patients with FVAA underwent microsurgical clipping procedures. Accurate chromosome segregation depends on proper kinetochore- microtubule attachment. Upon microtubule interaction, kinetochores are subjected to forces generated by the microtubules. In this work, we used laser ablation to sever microtubules attached to a merotelic kinetochore, which is laterally stretched by opposing pulling forces exerted by microtubules, and inferred the mechanical response of the kinetochore from its length change. In both mammalian PtK1 cells and in the fission yeast Schizosaccharomyces pombe, kinetochores shortened after microtubule severing. Interestingly, the inner kinetochore-centromere relaxed faster than the outer kinetochore. Whereas in fission yeast all kinetochores relaxed to a similar length, in PtK1 cells the more stretched kinetochores remained more stretched. Simple models suggest that these differences arise because the mechanical structure of the mammalian kinetochore is more complex. N2 - In this paper, knotted suture threads for microsurgery are evaluated from the view point of tension and elongation by using a developed testing machine. The suture threads for microsurgery are thin and easy to rupture. That makes a suture difficult. If a surgeon pulls the thread too strong when he or she makes a knot, it will rupture. On the other hand, if a surgeon pulls it too weak, it will become untied. Therefore, we want to know the mechanical properties of the thread with a knot. That will be a reference when a surgeon handle the thread or when a microsurgery support robot is designed. BACKGROUND AND OBJECTIVES: Lymphedema complicating breast cancer treatment remains a challenging problem. The purpose of this study was to analyze the long-term results following microsurgical lymph node (LN) transplantation. METHODS Twenty-four female patients with lymphedema for more than 5 years underwent LN transplantation. They were treated by physiotherapy and resistant to it. LNs were harvested in the femoral region, transferred to the axillary region, and transplanted by microsurgical procedures. Long-term results were evaluated according to skin elasticity, decrease, or disappearance of lymphedema assessed by measurements, isotopic lymphangiography, and ability to stop physiotherapy.
