Detection of diseases in an early stage is very important since it can make the treatment of patients easier, safer and more ecient. For the detection of rheumatic diseases, and even prediction of tendencies towards such diseases, capillaroscopy is becoming an increasingly recognized method. Nail-fold capillaroscopy is a non-invasive imaging technique that is used for analysis of microcirculation abnormalities that may lead todisease like systematic sclerosis, Reynauds phenomenon and others.. The main goal of this master thesis project is to provide new tools and techniques for the analysis of capillaroscopy images from the nail-fold area. Image processing and machine learning techniques are applied to images obtained by digital microscopes, like Mediscope as produced by Optilia Instruments AB, Sollentuna. This thesis oers a novel way for segmentation of capillaries from images as well as (semi)automatic capillary width calculation and automatic annotation of capillaries. These tools provide new ...
Objective Investigation of the reliability of the qualitative and semiquantitative scoring of nailfold videocapillaroscopy (NVC) assessment between two raters in a systemic sclerosis (SSc) cohort.. Methods Two raters from different centres blindly assessed the NVC images of 71 consecutive patients with SSc qualitatively as belonging to the scleroderma spectrum (SDS) category (early, active, late scleroderma pattern or scleroderma-like pattern) or to the normal category and semiquantitatively by calculating the mean score for capillary loss, giant capillaries, microhaemorrhages and capillary ramifications. Inter-rater/intrarater agreement was assessed by calculation of the proportion of agreement and by κ coefficients. Rater agreement of mean score values of hallmark parameters was assessed by intraclass correlation coefficients.. Results The inter-rater/intrarater proportion of agreement to qualitatively assess an image as belonging to the SDS category or not was 90% and 96%, whereas ...

Results Seven-teen SSc patients were found lSSc (17/31, 54.83%; mean disease duration 8.70 yrs) the remaining four-teen were found dSSc (14/31, 45.1% mean disease duration 11.14 yrs). The predominant DC TA patterns observed were spot (violet and suffused pink sometimes associated to punctiform vessels) on the hands and reticular (alveolar and branched vessels) on the other sites (mainly face). Spot DC TA pattern (prevalent in the hands) was found mainly associated with early/active NVC patterns in lSSc patients (91% vs. 9% early/active vs late NVC pattern, respectively), whereas reticular DC TA pattern was found mainly associated with active/late NVC patterns in dSSc patients, (92% vs 8% active/late vs early NVC pattern, respectively). In addition, TA on the palms appeared as spot DC pattern in 15 of 31 patients, including 13in the ACA positive patients (13/20; ROC area 0.7, p=0.03). A DC TA pattern consisting of suffused pink was observed on the palms of 13 ...
NASCIF, Ana K. S. et al. Inflammatory myopathies in childhood: correlation between nailfold capillaroscopy findings and clinical and laboratory data. J. Pediatr. (Rio J.) [online]. 2006, vol.82, n.1, pp.40-45. ISSN 0021-7557. http://dx.doi.org/10.2223/JPED.1435.. OBJECTIVE: Nailfold capillaroscopy is an important tool for the diagnosis and follow-up of patients with rheumatic diseases, in particular dermatomyositis and scleroderma. A relationship has been observed in adults between improved capillaroscopic findings and reduced disease activity. Our aim was to correlate disease activity (clinical and laboratory data) and nailfold capillaroscopy findings in 18 patients with inflammatory myopathies. METHODS: This prospective study included 13 juvenile dermatomyositis patients (Bohan and Peter criteria) (mean age of 8.8 years) and five patients with overlap syndrome (mean age of 15.7 years). We evaluated disease activity (skin abnormalities and muscle weakness, muscle enzymes and acute phase ...
Dermoscopy (DC) is a noninvasive technique used in the early diagnosis of melanoma5 and nonmelanoma skin cancers, as well as inflammatory skin disorders (psoriasis, lichen planus, seborrheic dermatitis)6. The vascular structures of these conditions are detectable by DC, which has also provided data similar to NVC images commonly used in the study of nailfold capillaries in connective tissue diseases7.. In our study, DC was used to better appreciate vascular features of TA on selected anatomic sites (e.g., face, chest, hands, forearms) and to provide a morphological description of altered skin microcirculation in patients with SSc from a novel perspective. Using this approach, capillaries perpendicular to the skin, as well as the deeper portions of cutaneous microcirculation, usually not visible, become visible as they appear similar to TA.. DC patterns were investigated from 31 women with SSc (median age 55 yrs, median disease duration 8 yrs) showing skin TA and grouped into limited (lSSc) and ...
Capillaroscopy is a method used to examine a patients microcirculation and assess pathological changes. It is completely safe and noninvasive. Using some sort of microscopy, the clinician looks through the epidermis of the nailfold to view the capillaries of the distal papillae.3. Ariane Herrick, MD, is professor of rheumatology at the University of Manchester and honorary consultant rheumatologist at Salford Royal NHS Foundation Trust, England. She explains that nailfold anatomy makes it an ideal place to evaluate the microcirculation. In most of the skin, capillaries run perpendicular to the skin surface, and only the tip of the loop is visible.. Dr. Herrick explains, At the nailfold, capillaries run parallel, rather than perpendicular, to the skin surface, and they can, therefore, be visualized noninvasively when magnified. Capillaries themselves are invisible, but what can be seen is the column of red blood cells within the capillaries.. This is of interest to rheumatologists in ...
We sought to assess whether nailfold videocapillaroscopy (NVC) patterns are associated with levels of angiogenic factors in systemic sclerosis (SSc). Circulating endothelial progenitor cells (EPCs) and circulating endothelial cells (CECs) were measured in the peripheral blood of 60 consecutive SSc patients. Serum levels of eight endothelial markers were measured first in these 60 patients, and then in an independent replication cohort of 43 SSc patients in case of association with NVC patterns. NVC patterns were determined by four independent investigators blinded to vascular markers. Patients with the late-NVC pattern exhibited lower EPC levels (P | 0.0001) and higher VEGF levels (P = 0.03). Higher VEGF levels were confirmed to be associated with the late-NVC pattern in the replication cohort (P = 0.01). By multivariate analysis focused on biomarkers, lower EPC (P = 0.03) and higher VEGF levels (P = 0.001) were independently associated with the late-NVC pattern. In an alternate multivariate model

Although an early capillaroscopic pattern can occur in patients who have suffered from Raynauds phenomenon for many years, the term early is used because this pattern tends to occur initially; it also can be observed in patients with shorter disease duration than in those with active or late patterns. However, it must be recognized that capillaroscopic patterns are descriptive and that there is inevitable overlap between patterns. The differences between the pattern in SSc and the normal pattern of healthy subjects or patients with benign primary Raynauds phenomenon can be readily observed (see Figure 12).. The development of robust scoring systems to quantify the microvascular change is essential to the performance longitudinal studies. For this reason, a practical system to score these capillaroscopic alterations in patients with SSc has been recently proposed and validated.7,8 Interestingly, in studies on clinical findings over time, about 15%-20% of primary Raynauds phenomenon are ...

Systemic sclerosis is an autoimmune disease characterised by microvascular involvement and fibrosis affecting the skin, lung, and heart. To date, our understanding of the changes in MICs, T cells, and cytokines in SSc is still incomplete. In our study, we hypothesised that SSc is associated with alterations in MICs, T cell subsets (CD4/CD8), and cytokine production in the lung, blood, and skin. We also hypothesised that these alterations contribute to the genesis of the pulmonary and skin lesions and would therefore correlate with the clinicopathological characteristics of these lesions. We carried out this investigation to fill this existing gap in the literature and to test our hypotheses. Therefore, we examined 19 patients with SSc and six matched healthy individuals. Our results clearly showed the following: (1) SSc has a predilection for middle aged women; (2) MICs and cytokine concentrations (IL-1β and TNFα) are increased in the BALF, blood, and skin lesions of patients with SSc, (3) ...

Background: A small number of studies used nailfold capillaroscopy (NC) in the evaluation of nailfold capillary changes in Behçets disease (BD). The purpose of this study was to investigate the characteristics and frequency of nailfold capillary changes in BD by dermatoscopy and videodermatoscopy and to develop a scoring system for those capillary changes to predict the activity and severity of the disease. Methods: We performed NC on 40 BD patients and 20 healthy controls with dermatoscopy and videodermatoscopy. Capillary morphology, distribution, and density were analyzed qualitatively and quantitatively. We also assessed the activity and severity of the disease with BD Current Activity Form and BD Severity Score Classification and evaluated the relation of these scores with morphology scores and capillary density. Results: Capillary morphologic alterations were encountered significantly more in BD group (P , 0.05). Loss of continuity in capillary loops and irregularity of capillary ...
Editor,-Studies on visual field and peripheral circulation had led to the hypothesis that the eye might be involved in the vasospastic syndrome,1 a potential contributor to glaucomatous optic neuropathy.2 In some patients with peripheral vasospasms, visual field defects worsened after cold provocation, and, often, both peripheral vasospasms and visual field defects improved after calcium channel blocker treatment.1Because these and newer observations3 suggest some parallels in ocular and digital blood flow regulation, the relation between ophthalmic artery and nailfold capillary blood flow velocities was evaluated.. Fifty patients with primary open angle glaucoma with a mean age of 67 (SD 15) years were examined. Excluded were patients with previous filtering surgery or systemic and cardiovascular diseases. Blood flow velocity in nailfold capillaries was assessed in one randomly chosen finger of the right hand (totally arbitrary choice) by means of nailfold capillaroscopy.4 The velocities ...

Raynauds phenomenon (RP) is a vasospastic disorder characterized by episodic color changes of blanching, cyanosis, and hyperemia in response to cold and/or emotional stress. Although most typically noted in the fingers, the circulation of the toes, ears, nose and tongue is also frequently affected. Population studies have shown that RP in adults is more common in women than men, with prevalence estimates ranging from 4% to 30%. Geographic variations in the prevalence reflect differences in climate. RP may be a primary or a secondary process. LeRoy and Medsger suggested criteria for primary RP: symmetric attacks, the absence of tissue necrosis, ulceration or gangrene, the absence of a secondary cause, negative antinuclear antibodies, normal nailfold capillaroscopy and a normal erythrocyte sedimentation rate. Secondary RP is characterized by an age of onset of more than 30 years, painful and asymmetric attacks, ischemic skin lesions, positive autoautoantibodies, capillaroscopic abnormalities ...

Dear Editor,. Congratulations Canadian Family Physician and Dr Ran D. Goldman for the Child Health Update on Raynauds phenomenon (RP) [1]. I would like to point to a new study in childhood on this underrated functional vascular disease.. The pediatric research group of Gorski et al from Poland examined with the help of nail fold capillaroscopy and blood tests 66 patients with RG (34 undifferentiated RP and 32 secondary RP) aged 6-19 years and a control group with 20 healthy children [2]. Compared to the control group, significantly higher concentrations of endothelin-1 (p = 0.0134), sE-selectin and high-sensitivity C-reactive protein (hsCRP: p , 0.001) were found in the patients with RG, as well as a lower high-density lipoprotein cholesterol concentration in the lipid profile (p = 0.001). The group over 15 years also had higher levels of low-density lipoprotein fraction and triglyceride. As the analysis of multiple linear regression showed, among the capillaroscopic parameters, the more ...

To the Editor:. We read with great interest the recent Images in Cardiovascular Medicine article by Girod et al1 regarding a case with tako-tsubo-like transient left ventricular dysfunction. In their article, the authors well described a case of this rather unusual disorder and also provided an impressive picture of a tako-tsubo (octopus catcher). Although the exact mechanism for this entity has yet to be clarified, there have been suggestions of abnormal microvascular involvement after emotional or physical stress in this still-mysterious transient disorder.2. This apparently extraordinary phenomenon has several similarities to the left ventricular dysfunction seen in cases of subarachnoid hemorrhage3 as well as brain death. This is of particular importance as some of these individuals become organ donors. Findings such as mild elevation of cardiac enzymes without evidence of obstructive epicardial arteries, reversible nature of the clinical course, and focal pathological changes in myocardium ...
Results. PBMC from patients with SSc produced significantly more APRIL (961 ± 151 pg/ml/105 cells) than control PBMC (798 ± 219 pg/ml/105 cells; p , 0.01). In patients with SSc, increased production of APRIL was associated with the presence of diffuse skin involvement, scleroderma lung disease, peripheral vasculopathy, greater capillary damage on capillaroscopy, and presence of anti-topoisomerase I (anti-topo I) antibodies. Multivariate regression analysis revealed anti-topo I antibodies as the only independent predictor of high production of APRIL by PBMC. ...

ABSTRACT: In order to study the neonatal microcirculation, the capillary hemodynamics in skin was investigated in 43 full-term infants 2-7 days after birth. The nailfold capillaries of the thumb were visualized by means of television microscopy and the capillary blood cell velocity (CBV) was videophotometrically quantified in 107 microvessels. The skin temperature, mean arterial blood pressure, and heel puncture hematocrit were measured simultaneously to evaluate any relation with the CBV. The mean CBV in all infants was 0.38 ± 0.21 mm/s, with a range of 0.04 to 1.2 mm/s in individual capillaries. There was no correlation between CBV and skin temperature (27-33° C), mean arterial blood pressure (44-68 mm Hg), or postnatal age. However, a significant correlation was found between the log CBV and the skin prick hematocrit (r = −0.64, p | 0.001). It is concluded that the mean CBV during the 1st wk of life is not significantly different from the capillary velocity reported in adults. Normal variations

ROP remains a significant cause of vision loss in the pediatric population. Although some risk factors have been identified, its development is complex and involves both environmental and genetic factors. This study aimed to initiate the identification of genes that may play a role in ROP susceptibility. Using the murine OIR model, we developed a mapping cross between C57BL/6ByJ and BALB/cByJ inbred mouse strains. These mice were confirmed to have significantly different vascular responses to OIR on P16, with the BALB/cByJ strain having smaller avascular areas than the C57BL/6ByJ strain. These results support those published by Dorrell, et al. [20], which found that while both strains have similar vaso-obliterative areas upon their return to normoxia on P12, the BALB/cByJ strain revascularizes more rapidly than the C57BL/6J strain. Genetically determined differences in retinal ocular morphology between the two strains may play a role in their responses to OIR. Recent reports using confocal ...

RP is classified into two categories: primary and secondary. Primary RP are vasospastic attacks precipitated by cold temperatures or emotional stress. Primary RP, which occurs in the absence of an identifiable disease, is most common in otherwise healthy females between 15 and 30 years of age. A family history of first-degree family members is reported in about 30% of cases. These attacks usually occur symmetrically and bilaterally in the hands. There is no underlying sign of tissue necrosis or gangrene (eg, digital pitting) and the underlying vasculature is normal. Nailfold capillary microscopy (see below) and physical examination findings are normal. If a patient meets criteria for primary RP and no new symptoms develop over 2 years of follow-up, the development of secondary disease is unlikely. The finding of abnormal nailfold capillaries on microscopy or specific autoantibodies are strong predictors of secondary RP due to an underlying rheumatologic disease. ...
