Objective Investigation of the reliability of the qualitative and semiquantitative scoring of nailfold videocapillaroscopy (NVC) assessment between two raters in a systemic sclerosis (SSc) cohort.. Methods Two raters from different centres blindly assessed the NVC images of 71 consecutive patients with SSc qualitatively as belonging to the scleroderma spectrum (SDS) category (early, active, late scleroderma pattern or scleroderma-like pattern) or to the normal category and semiquantitatively by calculating the mean score for capillary loss, giant capillaries, microhaemorrhages and capillary ramifications. Results Seven-teen SSc patients were found lSSc (17/31, 54.83%; mean disease duration 8.70 yrs) the remaining four-teen were found dSSc (14/31, 45.1% mean disease duration 11.14 yrs). In our study, we hypothesised that SSc is associated with alterations in MICs, T cell subsets (CD4/CD8), and cytokine production in the lung, blood, and skin. ABSTRACT: In order to study the neonatal microcirculation, the capillary hemodynamics in skin was investigated in 43 full-term infants 2-7 days after birth. The nailfold capillaries of the thumb were visualized by means of television microscopy and the capillary blood cell velocity (CBV) was videophotometrically quantified in 107 microvessels. The skin temperature, mean arterial blood pressure, and heel puncture hematocrit were measured simultaneously to evaluate any relation with the CBV. The mean CBV in all infants was 0.38 ± 0.21 mm/s, with a range of 0.04 to 1.2 mm/s in individual capillaries. There was no correlation between CBV and skin temperature (27-33° C), mean arterial blood pressure (44-68 mm Hg), or postnatal age. However, a significant correlation was found between the log CBV and the skin prick hematocrit (r = −0.64, p | 0.001). RP is classified into two categories: primary and secondary. Primary RP are vasospastic attacks precipitated by cold temperatures or emotional stress. Primary RP, which occurs in the absence of an identifiable disease, is most common in otherwise healthy females between 15 and 30 years of age. A family history of first-degree family members is reported in about 30% of cases. These attacks usually occur symmetrically and bilaterally in the hands. There is no underlying sign of tissue necrosis or gangrene (eg, digital pitting) and the underlying vasculature is normal. Mounting evidence indicates that inflammatory mechanisms drive systemic sclerosis (SSc) vasculopathy and fibrosis, especially early in the disease. Therefore, patients with very early SSc could benefit from early treatments targeting inflammation. Glucocorticoids are among the most potent anti-inflammatory and immunosuppressive agents. Several studies have demonstrated a mixed response to treatment with glucocorticoids in SSc, probably because it is seldom initiated at very early stages of the disease. We hypothesise that by inhibiting the inflammatory process driving SSc disease progression, glucocorticoid treatments will induce remission in patients with very early SSc. This study is a 12-week, randomised, double-blind, placebo-controlled trial analysing the effects of high-dose intravenous methylprednisolone in very early SSc. Eighteen eyes of 9 consecutive patients with a median age of 41 years (range: 19-54) with electrophoretic confirmation of sickle cell disease were included and analyzed.
