OBJECTIVE: To measure the proportion of nosocomial diarrhea cases associated with Salmonella and Shigella species. DESIGN: Prospective 6-month survey. SETTING: Tertiary care center in a developing country. PATIENTS: Pediatric and adult patients admitted with the previous 24 hours and all consenting adult or pediatric medical patients with nosocomial diarrhea. OUTCOME MEASURES: Prevalence of Salmonella and Shigella species isolated from rectal swabs at admission and among subjects with nosocomial diarrhea. RESULTS: Salmonella species and Shigella species were isolated from 3.0% and 2.5%, respectively, of 667 patients screened on admission. All admission Salmonella isolates were identified in children under 13 years of age; Shigella prevalence was similar for children and adults. Children with Salmonella at admission were significantly older and more likely to have diarrhea, fever, and some indicators of malnutrition than those from whom Salmonella was not isolated. We have identified new microbial pathogens and want to study their molecular biology and how they interact with human cells, develop markers for the infection etc. For all these purposes, we need to know what kind of proteins the organism has. I was amazed to hear recently that in a big majority of cases (Even for well-known pathogens) it is really not known if the microbe produces that particular protein. In other words, there is no experimental evidence. For example, a proteogenomic study of MTB identified 3176 proteins with approx 250 novel peptides. Doctors and scientists from MMM, Public Health England and The Royal Sussex County Hospital, Brighton published their whole-genome sequencing based tool to track the spread of Neisseria gonorrhoea infections in The Lancet Infectious Diseases today. Using samples collected in Britain and the USA they found genetic evidence from the infections showing how cases were linked to each other. INFECTION CONTROL TEAM The team usually consists of a Consultant Medical Microbiologist (CMM) and one or more well qualified and trained Infection Control Nurses (ICNs). Whilst the CMM is often a Member of the Royal College of Pathologists (MRCPath) he/she is often less qualified as an Infection Control Doctor (ICD) although he/she is often called upon to take the lead in Infection Control and act as the ICD. Most of the Earths surface is colder than the inside of your refrigerator. Deep sea temperatures are almost universally 2-4C. Thats already 73% of the Earths surface. Then add to that the polar regions, mountain ranges and permafrost and you have about 85% of the Earths surface constantly at refrigerator or freezer temperatures. While there are many microbes adapted to life at those temperatures, microbiology has historically been dealing with mesophiles ("middle [temperature] loving") that with psychrophiles ("cold lovers"). This is mostly for historical and practical reasons: medical microbiology has always been dominant and our body temperature is 37C. Another historic branch of the development of microbiology was the food industry, and most processes in that branch take place at temperatures between 10 and 40 C.
