A 70-year-old man was admitted with generalized seizures. The initial laboratory evaluation showed normal blood counts (platelet count, 303 109/L) and normal coagulation studies. Dilantin therapy was started and a left parietal hemorrhage was diagnosed. Nine days later, a left lower lobe pneumonia developed that was treated with gentamicin and ceftriaxone. On day 19, because of persistent fever, this antibiotic treatment was discontinued and mezlocillin therapy was started. The platelet count on that day was 388 109/L. On days 20 to 22, platelet counts were between 368 and 421 109/L; blood counts were not checked on days 23, 24, and 25. On day 26, the platelet count was 1 109/L, with a normal leukocyte count, hemoglobin concentration, prothrombin time, and partial thromboplastin time. The patient was clinically stable with no overt bleeding. Six units of random donor platelet concentrates were transfused; the platelet count 1 hour after transfusion was 83 109/L. Therapy with mezlocillin and ...
MK-417, a potent carbonic anhydrase inhibitor capable of reducing intraocular pressure after topical application, is currently under investigation for the treatment of glaucoma. The purposes of this study were to characterize dose-dependent pharmacokinetics of MK-417 and to determine the accumulating effect of the drug during chronic topical administration in rabbits. Because the drug resided primarily in the erythrocytes, kinetic analyses were performed on whole blood concentration data. Following i.v. administration, both total blood clearance and apparent volume of distribution for MK-417 increased disproportionately between the low and high dose, while the half-life of the drug appeared to be independent of dose. Total blood clearance and apparent volume of distribution increased from 0.993 +/- 0.224 ml/hr/kg (mean +/- SD) and 88.6 +/- 9.4 ml/kg at a dose of 0.05 mg/kg to 2.73 +/- 0.17 ml/hr/kg and 272 +/- 5.5 ml/kg at a dose of 1 mg/kg. The dose-dependent kinetics of MK-417 are probably due ...
Complications or urinary tract infection may lead to renal failure in persons with neurogenic bladders secondary to spinal cord injury (SCI). Safe, broad-spectrum, oral antibiotics are needed for use in this population. This study evaluates the susceptibility of 980 gram-negative urine isolates, including 68 from SCI patients, to norfloxacin and eight other drugs. Ninety-four percent of isolates from SCI patients and 98% from non-SCI patients were susceptible to norfloxacin. The percentage of strains susceptible to norfloxacin was comparable to that of aminoglycosides and ceftazidime, and was greater than that of mezlocillin, tetracycline, nitrofurantoin, and trimethoprim/sulfamethoxazole. Norfloxacin was the only oral drug showing significant activity against Pseudomonas aeruginosa and was as effective as the injectable drugs. Bactericidal levels of norfloxacin are easily achievable in urine and the drug is generally well tolerated. Norfloxacin may prove to be useful for management or ...

Two different strains of Escherichia coli exhibiting unusual patterns of resistance to beta-lactam antibiotics were isolated from patients at Cochin Hospital. Both isolates showed a low level of resistance to amoxycillin, ticarcillin and ureidopenicillins but were susceptible to cephalosporins, aztreonam and imipenem; beta-lactamase inhibitors potentiated the activities of the beta-lactams to only a limited extent. All resistance characteristics of the strains were transferable by conjugation to E. coli K12. Resistance was shown to be due to beta-lactamases of pI 5.20 and relative molecular masses of 24,000. The hydrolytic and inhibition profiles of these enzymes were similar to each other but differed from those of broad-spectrum beta-lactamases (TEM-1). The rates of hydrolysis (Vmax) of amoxycillin (c. 200%) were higher than that for TEM-1 (84%). Ticarcillin, ureidopenicillins and cephaloridine were hydrolyzed slowly. However, as for TEM-1, no hydrolysis was observed with cefoxitin, third generation
TY - JOUR. T1 - Ritonavir decreases the nonrenal clearance of digoxin in healthy volunteers with known MDR1 genotypes. AU - Penzak, Scott Robert. AU - Shen, Jean M.. AU - Alfaro, Raul M.. AU - Remaley, Alan T.. AU - Natarajan, Ven. AU - Falloon, Judith. PY - 2004/6/1. Y1 - 2004/6/1. N2 - Our objective was to examine the influence of ritonavir on P-glycoprotein (P-gp) activity in humans by characterizing the effect of ritonavir on the pharmacokinetics of the P-gp substrate digoxin in individuals with known MDR1 genotypes. Healthy volunteers received a single dose of digoxin 0.4 mg orally before and after 14 days of ritonavir 200 mg twice daily. After each digoxin dose blood and urine were collected over 72 hours and analyzed for digoxin. Digoxin pharmacokinetic parameter values were determined using noncompartmental methods. MDR1 genotypes at positions 3435 and 2677 in exons 26 and 21, respectively, were determined using PCR-RFLP analysis. Ritonavir increased the digoxin AUC0-72 from 26.20 ± ...

The Office of the Inspector General (OIG) investigates criminal activity for HHS. The special agents who work for OIG have the same title series 1811, training and authority as other federal criminal investigators, such as the FBI, ATF, DEA and Secret Service. However, OIG Special Agents have special skills in investigating white collar crime related to Medicare and Medicaid fraud and abuse. Organized crime has dominated the criminal activity relative to this type of fraud. HHS-OIG investigates tens of millions of dollars in Medicare fraud each year. In addition, OIG will continue its coverage of all 50 states and the District of Columbia by its multi-agency task forces (PSOC Task Forces) that identify, investigate, and prosecute individuals who willfully avoid payment of their child support obligations under the Child Support Recovery Act. HHS-OIG agents also provide protective services to the Secretary of HHS, and other department executives as necessary. In 2002, the department released ...

Bisphosphonates are clinically useful for the treatment of bone disorders; however, there is some controversy concerning the extent to which the design of the dosage regimen influences the efficacy of these drugs. The effect of different rates of infusion of [14C] pamidronate (APD; 3-amino-1-hydroxy-propylidene-1,1-bisphosphonate) (1 mg/kg infused over 4 or 24 hr) on its pharmacokinetics was investigated in rats by the measurement of tissue disposition and plasma clearance. The pharmacokinetic parameters, including total clearance, renal clearance, and nonrenal clearance, were found to be not significantly affected by the rate of infusion. The concentration of pamidronate in tibia, liver, kidney, and spleen was also unaffected by the two infusion rates. The bone (tibia) contained the highest concentration of all the tissues sampled, and the kidney accumulated the highest concentration among the soft tissues measured; this was in contrast to previous bolus administration studies where the liver ...
